Struck against a moving object

The Euclid R-Haul truck Comp. No. 671 was hauling a load of stemming material to the drill bench in the Wadesville Pit. The truck went over an embankment, upset, coming to rest on the driver's side causing fatal blunt force trauma to the driver located inside the cab. The cause of this accident is yet to be determined pending further investigation.

Over-exertion in lifting objects

Lifting power cable for 7800 dragline, he felt pain in left shoulder and bicep area - torn bicep ligament at elbow.

Fall to the walkway or working surface

Emp. was walking on snow covered ground when he slipped & fell to the ground.
