Fall to the walkway or working surface

Unlocking gate @ jobsite entrance - slipped & fell on icy ground.

Absorption of radiations, caustics, toxic and noxious substances

Employee stepped on bees nest & had multiple bites. Employee is allergic to bees. Injected with epi-pen & transported to hospital.

Fall down stairs

Emp tripped & fell down (4) steps leaving scale house

Contact with hot objects or substances

Welder shut off. Checked fluid levels to see the reason. Burned with the radiator fluid.

Fall onto or against objects

Lifting cable w/chain block hoist - hoist slipped while emp was pulling chain - Emp fell forward and struck ribs off dirt chute

Fall from machine

EE was climbing ladder to get into truck. Weld on ladder step broke causing ladder step to fall. EE fell 4-5' striking left knee on ground.

Over-exertion (Not Elsewhere Classified)

Changing screen on Findley & twisted knee - no lost time until 7/20/16.

Struck by falling object

Removing rear axle from Mack Dump Truck - axle dropped crushing lt hand between it & truck frame.

Over-exertion in pulling or pushing objects

Installing 14 inch plastic pipe. Pushed on pipe and felt pain/heard pop.

Fall from ladders

Changing light bulb on truck. Ladder kicked out. Fell on wrist.

Over-exertion in lifting objects

Stemming drill holes - shoveling rock w/shovel. Experienced pain in low back.

Repetitive motion injury due to working with hand tool and operation equipment
