Fall from machine
Cab of 785B truck caught on fire while emp was backed into rock dump. Emp jumped from platform to ground below.
Fall from ladders
Employee was climbing ladder to access jacked up truck. Jack kicked out. Employee fell onto cab deck of truck hitting head, ribs and tail bone.
Struck by flying object
Drill bit blocked up, Employee raised tower and separated steel to remove all pressure to dig out air holes. Drill steel still had pressure and blew air and debris into Employee's eyes and face. He did not wear his safety glasses.
Over-exertion (Not Elsewhere Classified)
Employee was climbing ladder to get into truck. Bottom of ladder swung in at emp. Strained wrist/hand on grab rail.
Struck against a moving object
Emp. was backing up ramp in truck to dump load. Emp. got to close to Rt side of ramp. bank gave way under weight of truck & slide sideways down bank & then rolled onto roof aprox 30 feet.

On 2/4/2015 employee twisted knee getting out of loader. Did not seek medical treatment until 02/25/2015.
