Inhalation of radiations, caustics, toxic and noxious substances

Drilling test hole @ 95' in coal bed. Drill bit got stuck & ignited coal causing Co to escape up drill hole. Employee felt ill & was taken to local hospital.

Caught in, under or between a moving and a stationary object

Employee was operating bucket truck to weld on boom of Demag. When finished welding and lowering bucket, employee somehow caught himself between bucket and boom of Demag.

Fall onto or against objects

Employee was cleaning out RR car - slip & fell while inside car hitting back and side on i/s of car wall
