EE was thawing a pipe when it released and swung back. The water that came from the pipe knocked EE down. EE braced self with hands upon falling and jolted right shoulder.

Walking to the truck from the load out area when EE tripped over a mesh screen and fell.2

Coming off the steps of a Haul Truck, employee slipped off the last step and felt right knee pop.

Moving the head on the drill up & down for mechanic to work on and the steel hammer came back and pinned EE's left elbow between it and the drill.
