Over-exertion in pulling or pushing objects

Employee was cleaning tracks on drill rig with track shovel. Hit something hard with shovel and heard/felt left shoulder pop.

Fall onto or against objects

5/12 tripped and fell on an uneven sirive screen plant (S190) 5/25 slipped getting off a loader (Cat 981G loader co#1071)

Employee was exiting off the work vehicle when EE felt a pinch in the lower left side of back that had the pain go down the left side of EE's leg into the foot.

Fall to the walkway or working surface

inclement weather when employee was hosing down a haul truck that was recently repaired from a hydraulic hose busting. When they came around the front of the truck, they slipped and fell

This miner was found unresponsive in the back of a lube truck. 911 and MSHA were immediately notified. Upon further investigation it turns out this was the result of a non work related medical condition.

Fall from machine

Non-employee was crossing haul road on a motorcycle. Individual lost control; and crashed causing injury. EMS called to the scene. MSHA notified.
