Accident type, without injuries

EE was driving away from pump when EE saw flames behind the cab. EE stopped the truck & exited by jumping off the front bumper. The truck rolled backwards about 150 yards and stopped up against a pile of coal where it continued to burn. Fire Dept. arrived on scene & extinguished the truck.

No injury - near miss accident - Employee was dumping load out of 785B truck - pulled out with the box up & made contact with overhead power lines.

EE was walking across uneven ground on mine site and slipped on ice and fell to ground landing on back.

Over-exertion (Not Elsewhere Classified)

While walking across uneven ground, employee stepped on a rock and twisted right knee. Employee has been working all this time till 9/30/2020, employee gets surgery today.

Struck against stationary object

While climbing down ladder, exiting loader, employee's foot slipped off 4th step causing employee to fall to the ground, striking head on ladder.

Fall from machine

While dismounting the truck employee climbed down the ladder to the bumper. EE did not realize that the under bumper ladder was missing. When EE stepped over the bumper from that missing ladder EE fell to the ground.

Employee fell getting out of back hoe causing left shoulder injury.

Employee was walking to designated parking area to dragline when ee slipped and fell on ice

Getting out of service truck. Gust of wind slammed door shut on left hand.

EE backed truck up to dump. Revved engine & pulled dump lever & immediately felt & saw flames around cab coming from engine. EE exited truck and went down ladder.

During repair to coal vessel, employees hand was pinched between flange & coal chute.
