Employee was working on lining grinding #4. The liner was set on the mill floor on the tracks, it needed to be tipped slightly to reconnect to be put on the handler cart. It moved quickly and pinched his right index finger between the liner and the grapple tool. Lost time started on 5-5-04.

Fall onto or against objects

Employee was skirting on 17 conveyor when he fell backwards off the conveyor structure hitting his thigh on 2"" angle iron. He was admitted to the hospital on 9/24/04.

When reaching to check to see if seal water was on to 7B DTV pump, Ray slipped in mud and when trying to catch his balance he dislocated his right shoulder. He had surgery on 8-2-04.

The impact wrench used to re-torque the spider bolts in the crusher fell off the end of the monorail that it was suspended from. The wrench struck the employee on the foot causing a fracture.

Struck by falling object

WORKING AT PRECIPITATOR BOTTOM AT FALSE FLOOR IN HOPPER, JACKING FRAME TO ALIGN BOLT HOLES. HIGH VOLTAGE ""RIGITRODES"" FRAME CORNER LET LOOSE AND THE TEMPORARY SUPPORT MEMBER BROKE. THE FRAME CORNER CAME DOWN, STRIKING HARD HAT ON EE.

While repairing fans above the tripper in the ore barn, ee stepped on a trap door. He fell through the trap door, skinning his shin and catching himself, straining his right shoulder. He returned to the job and sought medical care.

EE PARKED HIS PERSONAL TRUCK IN THE DAY SHIFT PARKING LOT UPON ARRIVAL AT WORK. WHEN HE EXITED HIS VEHICLE HE SLIPPED ON THE SNOW COVERED GROUND AND TWISTED HIS KNEE. HE WAS EVALUATED BY A PHY SICIAN AND WAS TAKEN OFF OF WORK. KNEE SURGERY SCHEDULED FOR 2-16-04. (WAS OFF OF WORK STARTING ON 2-4-04.)

EMPLOYEE WAS WALKING FROM T-1 PRECIPITATOR BINS TO T-2 PRECIPITATOR BIN AREA VIA THE PELLET PLANT FAN FLOOR. HE WALKED THROUGH SOME MUD AND SLIPPED TWISTING HIS LEFT KNEE. HE WAS EVALUATED BY A PHYSICIAN AND KNEE SURGERY WAS SCHEDULED 2-19-04.

Struck against stationary object

While hosing under 28-2 conveyor on the washdown floor, employee was pulling a hose and struck his left shoulder on a beam. Physical therapy started on 1/3/05.

Over-exertion in lifting objects

Employee was changing a water jug on the water cooler and felt a strain in her shoulder. She was evaluated by a physician and physical therapy started on 1-11-05.

Employee was pounding in a panel retaining sleeve with a ball peen hammer. When he struck it with the hammer, the hammer slipped out of his hand and bounced up hitting his tooth. The tooth was broken off and required dental restoration.

Employee was lining inside 1B flux mill (2-3"" steel balls are inside). He took a step and the balls underneath his foot came loose causing his foot to slide and he felt a sharp pain in his back. He was evaluated by a physician and physical therapy started on 9-10-04.

Employee strained his right shoulder when raking back apron feeder 2C to clear a large slab. He was evaluated by a physician and physical therapy started on 9-9-04.

Fall from machine

Was a passenger in a van - while stepping out lost his balance & fell to the ground - chipping his front tooth.

Struck by flying object

Employee was welding and grinding on a screen when he got a piece of metal in his eye. he was treated with a prescription the following day.

Struck against a moving object

EE WAS OPERATING A LOADER - HE HIT SOMETHING SOLID IN THE TUNNEL AND THE LOADER STOPPED. HIS HAT FLEW OFF AND HIT THE WINDSHIELD BREAKING THE GLASS. HIS HEAD IMPACTED THE CAB - A LACERATION RESULTED REQUIRING 4 SUTURES.

EE was hooking up hoses to the north stock pile track cars and slipped hitting her head on pet cock. She complained of a pulled muscle in her neck. She was evaluated by a physician and physical therapy was started on 6-8-04.

WORKING AT BOTTOM OF PRECIPITATOR, HEARD LOUD CRASH OF STEEL FALLING, AREA FILLED WITH DIRT & DUST. SCRAMBLED TO GET DOWN AND OUT.

Employee complaining of pain in wrists and numbness in both hands related to operating heavy equipment. Diagnosis is carpal tunnel syndrome and it was determined on 3/8/05 that it was occupationally related.

EE COMPLAINS THAT HER HANDS HAVE BEEN SORE FOR THE LAST FEW WEEKS. SHE RELATES THIS TO HER WORK ACTIVITIES. DIAGNOSIS OF CARPAL TUNNEL SYNDROME. IT WAS DETERMINED ON 6-8-04 THAT IT IS OCCUPATIONALLY RELATED.
