A contractor was stuck in the Pellet plant elevator for approx. 1 hour and 40 minutes. MSHA was notified. No injury.

The elevator door close limit switch was not making contact which caused the elevator to stop. An employee was stuck in the elevator for 57 minutes. MSHA was notified via phone call. No injuries.

An employee was stuck in the concentrator elevator for 1 hour 5 min. MSHA was notified. Repairman was contacted-no injury.

An employee was trapped in the warehouse elevator for approx. 49 minutes when the elevator car was unable to spot itself at the proper level.

Employee smelled abnormal odor while performing cleanup operations in the coal system building. Identified small amount of smoldering coal and promptly flushed with water. Outside emergency services was contacted and confirmed that smoldering coal was extinguished by mine personnel.

Two employees were stuck in an elevator for approx. 30 minutes. The elevator service was contacted. No injuries occurred. MSHA was informed at the time of the incident.

The sight glass area in the 1 1/2 vent line has 4 bolts that hold a glass tube in place. The bolts all snapped and the glass tube broke. Operators smelled gas, closed main gas valve and vented building. Followed up with air monitoring equipment to verify acceptable range.

The employee was operating a loader moving chunks for the chunk breaker operator. As the employee was backing up, debris flew through the open window of the loader striking the employee in the left eye. He was evaluated, surgery was performed and taken off of work on 9-29-10.

Employee was removing a tail shaft, when his hand was hit by the shaft, resulting in a finger fracture. **Lost time started 9/7/10.**

Over-exertion in lifting objects

Employee was lifting a panel and felt a pull in his abdomen. He was evaluated and diagnosis was a hernia.

Flash burns (electric)

Employee working on a 480 volt starter experienced an arc flash blast resulting in burns to his hands and face. He was evaluated by a physician and taken off of work.

Employee reported a low back strain, was evaluated by a Physician on 5-17-2010 and taken off of work resulting in Lost Time. This personal injury has been determined to be attributed to an event dated 12-12-96. *On 5/27/2010, Medical was obtained indicating the above.

Employee was pulling a chain and it struck her on the left wrist and fingers. She was evaluated by a Physician and a splint/sling was applied.

The employee grabbed a tool and turned and allegedly states he experienced pain in his lower back. He was evaluated by a Physician on 12-18-10 and placed on restricted duty.

The medical was reviewed and it was made known on 1-17-11 that it was work related. Physical Therapy was ordered and a series was completed. The employee was driving his dozer, backed into a feeder and was jostled, resutling in an injury to his shoulder.

Employee was walking over a snow bank when he sunk into the snow causing him to strain his left leg. He was evaluated by a physician and placed on restrictive duty starting 12/15/10.

Employee was standing between two sawhorses to cut a metal beam, the cut beam fell to the floor, striking the employee on the top of the foot. This resulted to fractures in the foot and restrictive duty starting 6/10/10.

Employee was removing the discharge hose from a conveyor sump. The hose was hooked to the crane. When the hose broke free from the pump it struck him in the right side of the head. He fell to the ground and was unconscious for a brief time. Was evaluated by a physician and returned to work on 12/2/10.

Employee pinched his left ring finger between the bolt driver and the tool holder resulting in sutures to his finger.

While sliding a piece of grating off of a pallet, employee's glove got caught on the grating. The grating fell pinching his right middle finger between the grating and the floor. This resulted in a fracture to the distal end of his finger.

Employee complained of a foreign body in his eye during his work activities. He was wearing safety glasses and face shield along with a half mask repirator. He was evaluated by a physician and a small piece of metal was removed from his eye and eye medication was prescribed.

Employee was removing bearings from a head shaft. While removing the rollers from the bearing cage, the employee ran his hand against the roller cage, resulting in a cut to his left index finger. He received sutures.

Employee was using a hand grinder when it kicked back cutting his left wrist area resulting in sutures.

Moving a light stand wired to a piece of steel structure while repositioning the light, finger got caught between the steel and light pole causing a laceration.

Employee was kneeling on the floor welding base plate for sheeting being installed. As he was starting to stand up, he said he felt something pull in his back.

Employee was using a hole saw to cut a hole in a metal box when he felt something go in his left eye. He flushed his eye; however, on 5/5/10 complained of an irritation in his eye. He was evaluated by a physician and a foreign body was removed from his eye and eye medication was prescribed.

While hammering in a feeder chain link pin, something chipped off the pin or a rock flew off the area and hit the employee in the right cheek. He was evaluated by a physician and received sutures.

Employee was stripping a wire. As the wire was stripped from the jacket, his hand came into contact with the edge of a cabinet door. This resulted in a cut to his right index finger requiring sutures.

Employee was changing idlers when he felt dirt go into his eye. He was evaluated by a physician. There was a non-imbedded foreign body removed from his eye. Eye medication was prescribed.

Employee was traveling on walkway with his hand on the railing. While his hand was holding onto the rail, his left ring finger struck a handrail support resulting a fracture to his finger.

Employee was sorting through hydraulic press extensions to press a bushing on a shaft. While handling turn he caught a sharp edge and received a superficial cut to his right index finger. Derma bond was applied.

Numbness off and on in bilateral hands and arms reported in 2010. Has treated conservatively since. On 9-13-19 under went surgical procedure for condition.

Employee complained of numbness/tingling in his fingers and discomfort in arms and shoulders. It was made known on 9-14-10 that he has a diagnosis of Carpal Tunnel Syndrome and it is occupationally related.

Employee complained of numbness in his hands radiating up into his shoulders and he related this to his work activities. On 9/14/10 it was made known that the employee was diagnosed with Carpal Tunnel syndrome and it was determined to be occupationally related.

Visitor was talking to someone in the parking lot when he became light headed and passed out due to a non-occupational illness.
