Caught in, under or between two or more moving objects

Employee was transporting a conveyor impact roller assembly on a cart. The load shifted and his left ring finger was caught between the assembly and the edge of the cart. Employee's distal end of left ring finger was amputated with bone involvement. He started to miss time on 2/7/08.

Over-exertion (Not Elsewhere Classified)

Employee was removing a support pipe when the pipe unexpectedly moved and the employee grabbed the pipe. He felt a strain in his groin. He was evaluated by a physician on 1/13/09 and diagnosis with a hernia. He started to lose time on 1/21/09.

Employee complained of pain to his hands after disconnecting some hydraulic lines. He was evaluated by a physician and was diagnosed with carpal tunnel syndrome. On 11/21/08, it was determined to be occupationally related. (per call surgery on shoulder 3/10/09

Employee was a passenger in a golf cart that rolled over. The cart landed on top on his legs. He was evaluated by a physician and was taken off of work on 8/21/08.

EE was removing drive mounting bolts.When rmoving the 7th of the 8 bolts the remaining bolts sheared off.The stored energy in the drive caused it to rotate rapidly.The belt guard on the drive struck employee in the left shoulder-throwing him forcefully into a handrail and structural beam.He was evaluated by a DR and taken off of work.

Struck by flying object

The brake assembly at the Kiln Aux drive was disassembled as it was being repaired. The drive was started and the assemblies spun and hit the bolts that held the drum on. The brake assembly broke and hit the employee in the leg, who was standing approx. 12 ft away. This resulted in loss of time.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was working in outside storage area, set up with a temporary enclosure. Employee stated that the temporary heater burned eyes and caused coughing.

Drilling a harness clamp when glove became caught in machine. Injured left hand when pulling hand free. Employee was seen in ER. Fracture.

Contact with hot objects or substances

Employee was cleaning the internal butterfly valve of a fuel injection system. The engine backfired, causing flames to shoot up into the air. He received burns to his eyes. He was evaluated by a physician and eye medication was prescribed.

Struck against stationary object

Employee dislocated his right pinky finger while descending stairs. He caught his finger on the mounting bracket for the handrail. On 10/23/08, it was made known that he was given an immobilizer for his finger.

Employee was removing a bolt when the wrench slipped off of the bolt and struck him in the mouth chipping a tooth. He was seen by a dentist for repair on this tooth on 10/06/08.

While shoveling out a launder, employee struck his right elbow against a horizontal floor support. He was evaluated by a physician and on 1-9-09 physical therapy was started.

EE was removing material in a plugged chute. After the EE was finished, he closed the top door.As he closed the top door,the door caught the bottom door which struck the outside part of his lower leg.The force made EE twist,resulting in him striking his other knee on the guard rail.Evaluated by a Dr. & physical therapy started on 7/14/08.

Employee was needle scaling a new weld, when he felt discomfort in his eye. He was evaluated by a physician and 2 foreign bodies were removed from his eye. Eye medications were prescribed and used.

Employee was removing line 9 primary mill starter from it? cabinet when he felt a pain in his left shoulder. He was evaluated by a physician and physical therapy started on 6/27/08.

Fall to the walkway or working surface

Employee tripped and fell on a charged washdown hose-catching himself with his hand. Upon falling, employee fractured his left ring finger.

While grinding a spacer shim, the employee felt an irritation in his eye. He was evaluated by a physician on 3/22/08. A piece of metal was removed from his eye and he was prescribed eye medication.

Caught in, under or between a moving and a stationary object

He pinched his left middle finger between the seat lever and the seat base when adjusting the seat. He was evaluated by a physician and diagnosed with a finger fracture.

Struck by flying object

Employee alleges he got a piece of metal in his eye while he was using a grinding wheel (on 2/14/08 & 2/15/08). He was uncertain to actually when it went in to his eye--he felt irritation in his eye on 2/15/08. He was evaluated by a physician and a foreign body was removed and he was prescribed eye medication.

A rope and pulley system was being used to raise a temporary trap door. An employee pulled on the rope and the pulley ancorage failed. The pulley struck the employee in the face requiring stitches (left side of nose and right side of chin).

Over-exertion in pulling or pushing objects

Employee was using a two wheel cart and while pulling down on the cart handles he felt a pull in his groin. He was evaluated by a physician and on 4/17/08 he started physical therapy.

Struck by flying object

Employee was repairing stripper rods when he felt an irritation in his eye. He was evaluated by a physician and eye medication was prescribed.

Over-exertion (Not Elsewhere Classified)

Employee complained of tingling to his hands and arms after using a plastic sledge hammer to install pebble mill liners. He was evaluated by a physician and on 12/22/08 it was determined his diagnosis of carpal tunnel syndrome was occcupationally related.

Employee complained of right hand pain from computer work. She was evaluated by a physician and diagnosed with DeQuervain's Syndrome. On 9/27/08 it was determined to be occupational related.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was diagnosed with asbestosis possibly due to asbestos exposure.

Over-exertion (Not Elsewhere Classified)

Employee complained of numbness in his hands related to using tools as a mechanic. He was diagnosed with Carpal Tunnel Syndrome and on 4/11/08 it was made known that it was occupational related. Lost time started on 4/10/08 due to surgery.
