Employee was using elevator to change levels, an extension cord that he was carrying caught in the elevator door disrupting the operation of the elevator at 8 ft below the initial level. Employee contacted his coordinator to report the stop. Elevator vendor was notified and MSHA was contacted immediately. Elevator vendor arrived and was freed employee in 40 minutes.

Employee was picking up a seal wear plate and received a laceration on his ring finger from the sharp edge of the plate. He received 3 sutures.
While driving T16 truck (240 ton) the operator drove on to the left hand berm causing the truck to tip onto it's right hand side. Employee was taken off of work per physician.
While walking carrying a 48# wt. he felt his ankle give and he fell to the ground.

While entering a storage building a piece of ice fell off the roof and landed on his hard hat. He was later started on PT 2/4/2008).
Employee was removing a tail shaft wheel at Unit 2 gate. When the wheel did not come loose, the employee used wedges to help pry. When the wheel did come loose, it hit the employee in the left face and shoulder, resulting in stiches to lip and cheek. Employee returned to work for next scheduled shift.
Employee was opening his locker and a wrench fell out and landed on his pinky toe. He was evaluated by a physician and was diagnosed with a fracture.
Employee was loosening a bolt from a motor mount. The wrench broke free, causing him to strike his hand against a shim. He received a laceration, requiring sutures.
When employee was removing his face shield to put his welding helmet on, he felt dirt hit his face. At the end of the day, he felt irritation in his eye and went to a physician for an evaluation. A foreign body was removed from his eye and eye medication was prescribed.
While stripping back the outer jacket on a cable with a knife, the knife slipped, cutting his right index finger. 3 sutures required.
Employee was moving a piece of 16"" diameter 34 feet long plastic pipe by using a sling and pitman. The end of the pipe tilted towards the ground and the connecting ring on the pipe slid down striking the end of his thumb. Employee was diagnosed with a fracture to his thumb.
Employee was taking an oil sample on a reducer. He was loosening the plug with a wrench and the plug broke free and he experienced pain in his right shoulder. He was evaluated by a physician and physical therapy started on 8/13/07.
Employee was attending the morning tool box meeting when he felt an irritation in his left eye. He was evaluated by a physician and a foreign body was removed and he received prescription eye medication.
Employee was descending the stairs when he struck his little finger against an upright on the handrail resulting in a fracture to his left finger.

Employee was lifting a tire into a tire rack and felt a twinge in his lower back. He was evaluated by a physician and physical therapy started on 5/24/07.
While using a 3' pry bar to loosen a screen panel, the panel suddenly came loose and employee's finger was pinched between the pry bar and the panel hold down angle. He received a small cut and bruise of the right 4th finger. On 4/02/07 it was diagnosed as a subtle fracture of the distal tip. No treatment was ordered.
Employee was removing a magnet when a puff of air blew dust into his face. The speck was removed and he was given ointment prophalactically.
Employee was cleaning his safety glasses and adjusting his heater and fan controls on the loader that he was operating. Employee complained of dust going in his eye when the fan started to blow. He was evaluated by a physician and eye medication was prescribed.
Employee was removing a cutting edge from a 992D. In the process of cleaning the recessed area around the retaining bolt nuts, the small pry bar hit the employee in the mouth causing a chip to his tooth. He had his tooth repaired on 1/22/07.
Employee reported pain & numbness in his fingers and hands. He relates this to his job duties. He was evaluated by a physician and diagnosed with carpal tunnel syndrome and trigger finger. It was determined on 2/28/08 that the carpal tunnel syndrome is occupational related and the trigger finger may be occupational related.
Employee was diagnosis with silicosis related to dust exposure.
