Accident type, without injuries

On Tuesday the 8th of September one of our mechanic's had a trouble call from Tilden Mines that number 2 pellet plant elevator is not working. EE arrived on site at 2:30pm, found debris in first floor hoistway door track, causing doors not to close. EE was inside the elevator assessing the doors, when they went fully closed, trapping EE in the car for 1 hour. .

Struck against a moving object

An employee was jostled when his loader contacted a toe at the shovel site. He was evaluated by a Physician & taken off from work. It was made known on December 18th that it was a lost time injury.

Over-exertion (Not Elsewhere Classified)

The employee filed an alleged report stating he felt a pain in his shoulder while wiping a windshield. He was evaluated by a Physician and it was made known on January 5, 2010 that Phyical therapy had been started.

Caught in, under or between a moving and a stationary object

Employee was punching holes in a 3/8"" plate and pinched his thumb between a stripper bar and plate that he was punching. He fractured his thumb and was taken off of work.

Caught in, under or between a moving and a stationary object

Employee was replacing a support shaft. While putting the shaft into place, the shaft shifted and his fingers were caught between the shaft and the wall resulting in fractures to 2 fingers. He was evaluated by a physician and was taken off of work.

Caught in, under or between running or meshing objects

Employee climbed onto a stopped conveyor belt without it being locked out. The belt started and he was caught by a material hopper, resulting in mulitiple injuries.

Fall to the walkway or working surface

Employee slipped and fell while pulling on the power cable to a shovel. He was evaluated by a Physician and started Physical Therapy on 1-30-09.

Over-exertion in pulling or pushing objects

Employee was pulling on a charged hose and felt pain in his right hip. He was evaluated by a physician and was taken off of work on 6/18/09.

Struck by falling object

Employee was knocking the dust off of a fan when the front guard fell off. His gloved hand hit the fan blades resulting in a fracture to his thumb. Lost time started on 6/18/09.

Over-exertion (Not Elsewhere Classified)

Employee was tightening bolts with a 1/2"" ratchet and felt pain in his right wrist. He was evaluated by a physician on 12/10/09. He was given a splint to wear.

Struck by flying object

Employee alleges that he got a piece of metal in his left eye from air chipping on 12/6/09. On 12/7/09 he reported this and was evaluated by a physician. An imbedded foreign body had to extracted from his eye.

Over-exertion in pulling or pushing objects

Employee alleges that he has back discomfort related to positioning a barrel of grinding balls on 11/24/09. He reported this on 11/30/09. He was evaluated by a physician and on 12/7/09 physical therapy was started.

Employee was cutting conduit with a sawzall. The sawzall slipped and went into his wrist. He was evaluated by a physician and on 11/16/09 he had a medical invasive procedure resulting in sutures.

Struck by flying object

Individual was welding when matter was blown into eye.

Contact with hot objects or substances

After arc airing, the employee received a burn to his Right hand. He was evaluated by a Physician on July 22nd, and has received a series of debridements.

Employee was removing a density gage cover. The cover came off and slipped out of his hands. He tried to catch the cover; however, his right index finger was caught between the cover and the pipe below. He received stitches to finger.

Fall to the walkway or working surface

Employee was walking in to work. He fell on the landing at the bottom of the stairs from the parking lot. He was evaluated by a physician and physical therapy was prescribed.

Struck by falling object

Employee was clearing a jammed grizzly. He cleared the rock and the grizzly fell abruptly back into position and the angle iron of the grizzly frame cut his right thumb. He received 4 sutures.

Employee was performing welding and grinding tasks. Accumulated material on his hard hat fell on his face when lifting the shield. He was evaluated on 1/19/09 and material had to be extracted from his eye and eye medication was prescribed.

Inhalation of radiations, caustics, toxic and noxious substances

During a routine screening, it was discovered that there were changes consistent with pneumonconiosis; probable asbestosis of unknown etilogy.

Employee complained of numbness and tingling in his hands after using a pneumatic drill. He was evaluated by a physician and on 3/18/10 it was determined that his diagnosis of carpal tunnel syndrome was occupationally related.

Employee complained of numbness and tingling in his hands from his work related activities. He was evaluated by a Physician and on 4-14-09 the diagnosis of Bilateral Carpal Tunnel Syndrome was determined to be occupationally related.
