Over-exertion (Not Elsewhere Classified)

Employee was using a 6 ton come-a-long on the transformer change out on grinding line 6. While using the come-a-long, he felt a pop in his right shoulder. Physical therapy started on 12/1/05.

Fall onto or against objects

Employee slipped on mud and hit his left knee on the handrail.

Struck against stationary object

Employee stepped off of a stair onto a rock and his right leg twisted out to the side. He was evaluated by a physician and on 10/12/05 he required surgery resulting in lost time.

Over-exertion in lifting objects

Employee was filling a headshaft oil reservoir with a 5 gallon pail of oil. While lifting the pail up, he felt a burning pain in his groin area.

Caught in, under or between a moving and a stationary object

EE was lifting a shaft out of a gear box and the shaft started to slide and his hand was caught between the shaft and an I beam. EE received several lacerations, resulting in surgery/lost time.

He was positioning an aluminum pick when the pick slipped. He pinched his upper arm on the handrail trying to hold on to it. Lost time started on 2/10/05.

EE was removing the feed chute liner from a primary mill 7 feed chute when the D-ring broke off of the liner-he was struck by the clevis attached to the D-ring resulting in facial trauma/fracture. Lost time started on 1/14/05.

Employee was cutting skirting for a conveyor line when the box cutter slipped, cutting his left thigh. He received 4 stitches.

Over-exertion in lifting objects

While lifting a roll of wire with the cable winder attached, employee felt a muscle pull in his right shoulder and a pop. He suffered a strain and was later prescribed physical therapy.

Absorption of radiations, caustics, toxic and noxious substances

Employee was sprayed with reuse water while cleaning seal water strainers for the andrits filter. He was sprayed in the right eye and was evaluted by a physcian and eye medication was prescribed.

Using a porta power jack repetitiously to remove bit from the chunkbreaker. The bit was coming out very slowly and it took many pumps to remove it. Employee felt soreness to his left shoulder and neck. He is receiving Physical Therapy

Employee was working in the north inlet of a fan. While climbing down, he put his hand on the donut resulting in a deep wound to his right thumb.

Employee complained of discomfort and decreased strength related to his work activities as a maintenance repairman. It was determined on 01/26/06 that his Carpal Tunnel Syndrome is occupationally related.

Employee complained of soreness in her elbows related to work activities as a plant repairman. She was diagnosed with epicondylitis and on 9/7/05 it was determined to be work related.

Experiencing pain in left elbow over a period of time. Diagnosed with Epichondylitis and determined to be work related.

Employee complained of tingling and lack of strength in his hands when working with 1"" pipe wrenches. He was evaluated by a physician and was diagnosed with carpal tunnel syndrome. It was determined on 2/18/05 that it was occupationally related.
