Employee was trapped in T-2 Elevator traveling from repair bay to balling floor. He was trapped for approximately 70 minutes. MSHA was contacted at about 01:45 AM. No injuries occurred.

2 employees were in Tilden 2 Pellet Plant elevator when the elevator stopped about 8 feet above the lowest level. The employees were entrapped for approximately 2 1/2 hours. MSHA was promptly notified.
An unplanned explosion of coal dust occurred in the Tilden 1 Raymond coal mill.
EE was changing filter sectors. When he lifted a filter, he felt pain in his groin. He was evaluated by a physician and was diag. with a hernia. EE started to miss time on 12/20/11.
Employee was setting up a ladder and the ladder tapped a dusty pipe. Employee was looking up and dust got in his eye. He was evaluated by a physician and taken off of work for one day.
Employee was lifting a pic into position and when it was chest high, he felt a burning in his bicep. He was evaluated by a physician and on 11/3/11 starting to miss time due to surgery.
Employee was drilling hole when bit got stuck. Torque on drill caused a clockwise quick motion of drill, catching his left pinky finger, resulting in a fracture and laceration of finger.
Employee was lining in the Primary Mill. He was using an alignment bar to position the liner in place, the liner popped up and the alignment bar struck the employee on the left side. He was evaluated by a Physician and released back to work. The employee missed one shift due to medication administration. He returned to work after missing one shift.
Employee was operating a loader and stated that he was jostled while mining the bank. He stated he was experiencing back and neck pain. He was evaluated by a Physician and taken off of work. He was released back to work on 9-22-2011.
Employee was using a sledge hammer -slugging down bolts in secondary crusher. He complained of pain in the groin area. He was evaluated by a physician-and on 7/20/11 he was diagnosed with a hernia. Loss time started 7/29/11 due to surgery.
Employee was moving trash when she felt pain in her back. She was evaluated by a physician and taken off of work on 6/14/11.
Employee was looking into the balling drum-not looking where he was going, when he stepped on a 4x4, twisting his knee. He was diagnosed with a knee strain and started to miss time on 4/29/11.
The employee was shoveling off snow from the deck of the drill when a piece of ice/snow allegedly fell from the mast striking him in the head/neck. He was evaluated by a Physician on 11-11-11 and taken off of work.
Employee was using a cart and bumped his knee on the steering column. He was evaluated by a physician and on 2/14/12 loss time began.
Employee was carrying some guarding when he tripped on grating and fell-twisting his back. He complained of back pain. He was evaluated by a physician and was taken off of work starting 7/12/11.
The operator was changing from clear glasses to sunglasses. He alleges that while leaning over and reaching to get the sunglasses he felt his back tighten up. He was evaluated on 2-16-11, and released back to his next scheduled shift (2-18-11) with restrictions. He took himself off for his first scheduled shift back, was re-evalauted on 2-18-11 and placed on restrictions.
Employee was walking down the ramp from the upper repair bay and rolled his ankle.
The operator was operating his loader in a confined area-the Tilden pit. He then alleged that his back started to feel sore. He was evaluated on March 3rd-continued to operated equip. On 3-29-11 he was re-evaluated with complaints of hip discomfort and given restrictions not allowing him to operate the loader.At this time it is unknown in complaints are related to 3-3-11 event.
Loader operator jarred neck while attempting to break frozen concentrate. She was evaluated by a Physician and given restrictions on 2-28-11.
Employee was struck by a pipe while walking through a trench causing an injury to his right ankle. He was evaluated by a Physician on 1-19-11, an air splint was ordered and placed on the right ankle.
Employee opened a door to the outside, the wind caught the door and his finger tip was pinched between the door and door frame. This resulted in a physician draining a hematoma under his nail bed on 12/28/11.
Employee reported pain in right wrist after turning pipe wrench while tightening a belt. He has been seen by a Physician and a splint was ordered.
Employee stated he felt something in his eye while at work; however, he doesn't know when or where this occurred. He was evaluated by a physician and eye medication was prescribed.
Employee was removing a countershaft arm guard. The employee was removing a port-o-power that he was using to guide the arm guard on the lift. The rigging broke and the arm guard fell on employee's left hand. Employee received a contusion to his hand and therapy was started on 11/28/11.
Employee's shift started on 9/6/11 at 10:45 p.m. Employee was lowering a plank to cover a pan feeder when the plank twisted and caught his finger. This resulted in a fracture to the distal end of his finger.
Employee was using a sawzall to cut conduit, the saw blade was pinched by the conduit. The saw then jumped causing the employee's hand to slip into the collect resulting in a fracture and sutures to his finger.
Employee was cutting conduit with a saws all and pinched his finger between the blade and frame of the saw resulting in a fracture to the end of his finger.
Employee was removing a cap from the outside of the cooler. The cap slipped and pinched his left ring finger between the cap and a rail. He was evaluated by a Physician on 3-10-11 and it was determined that his finger was fractured.
Employee was removing rigging when debris went into his left eye. He was evaluated by a physician and prescription eye drops were ordered.
