A single miner was involved in an elevator entrapment when an elevator malfunction occurred. Miner was safely removed and the elevator was repaired, inspected and returned to service.

A fire occurred on a Komatsu 930E Surface Mining truck. The area was secured to prevent harm from potential hazards for the duration of the exposure. External fire response agencies were immediately contacted and extinguished the fire when conditions allowed.
Employee was hosing in the crusher bowl when the employee tripped and fell landing on their left knee. Employee continued to work and felt something running down their leg when they observed a cut below the left knee. Employee was evaluated at the ER and received medical treatment (stitches).
Caterpillar 988K wheel loader was maintaining a shovel dig site when the left rear corner of the loader was contacted by loose material that fell from the dig face. The loader operator reported neck and back pain from being jostled in the cab. The left rear loader's fender and boarding ladder were damaged. The loader was taken out of service for repairs.
Employee arrived to work for the start of day shift. On their way into the truck shop from their personal vehicle, the employee slipped and fell on ice in the parking lot and broke their hip. Employee was transported to the hospital for medical treatment.
Employee was making a shovel cable connection when the cable rolled, pulling the employee's left arm forward. The employee experienced left arm pain and was evaluated at the local ED where images revealed an injury to their left bicep. The employee was prescribed work restrictions and is being accommodated.
Employee sustained and unknown injury to the left forearm while manipulating a lifter bar from the inside of the #1 Balling Mill.
An employee was ascending a stairwell prior to the start of their shift when the employee experienced a personal medical issue. The employee subsequently tripped and experienced a stair fall impact sustaining injuries to their left upper extremities.
Shovel operator stated jarring their left shoulder while operating the shovel mining out a ramp, and experienced left shoulder pain on 02/19/2025. Employee has a history of left shoulder injury. Employee continued to work full duty until receiving medical treatment for their left shoulder on 10/08/2025.
While positioning a pinion for install for the Pebble Mill, an employee's finger was pinched between the pinion and a liner bolt on the Pebble Mill. The employee sustained a laceration to their left index finger
Employee reported pain in their left shoulder while moving a case of boosters from a service truck to a cart. Employee was later prescribed restrictions 4-17-2025.
While installing the #8 Support shaft, the safety holdback rigging was in place and not utilized. This allowed the support shaft to slip and pinch the employee's calf between the shaft and shaft support wheels. Communication between the crew and foreman was poor. Crew complacency decreased awareness
While attempting to remove a wrapper rod, it fell down and struck employee in the right hand.
Employee was removing hardware from shovel dipper using a hammer when a piece of the hammer head broke off striking the employee in the left glute. The employee was seen in the ED where they received medical treatment and returned to work.
An employee attempting to clear a jammed chain for a chain fall sustained a fracture to their left index finger.
While moving a troughing roller frame assembly on 10 Conveyor, Line 3 in the Tilden plant, an employee pinched left thumb resulting in a laceration requiring 2 sutures.
On 4/5/2025, an incident was reported where an employee took off their hard hat to brush away dust, subsequently getting material in their left eye. The employee had reported the incident their following shift.
Employee was racking brick in kiln. While holding a brick, EE hit left hand with the rawhide hammer, breaking EE's middle finger.
Operator was cleaning tripper for maintenance. Operator was traveling on tripper walkway when the operator slipped and fell on water/ice formation. Operator sustained dislocation of their right fifth finger.
Plant repairperson was discarding worn return roller into dumpster when the employee pinched their left index finger between the return roller and the dumpster. The employee sustained crushing injuries to their left index finger resulting in medical treatment.
Employee experienced a presumed personal medical emergency at the start of their shift resulting in cardiac arrest. Emergency services responded, however, resuscitation efforts were unsuccessful.
