Accident type, without injuries

Employee in elevator car, when elevator car reached desired floor the elevator door would not open. Employee contacted operator via radio, communications maintained at all time, elevator car was locked out, fall protection worn and anchored, used the elevator door key and opened door, employee walked out of elevator. Time in elevator car approximately 35 minutes.

Accident type, without injuries

Two maintenance employees were riding in the Tilden Concentrator elevator when the elevator stopped. ThyssenKrupp elevator service was called. They opened the elevator door allowing the two to exit. ThyssenKrupp identified the 2nd floor call button to be broken. Call button was replaced. No Injuries or Illness. Radio and GAI-Tronics communications at all times.

Over-exertion (Not Elsewhere Classified)

EE's boot caught on L63's boarding step while boarding the loader causing EE to lose balance. EE felt discomfort in right forearm that was holding the handrail. Evaluated in the ER and taken off work.

Fall to the walkway or working surface

Employee slipped and fell in the night shift parking lot when reporting for night shift. Employee didn't want to be seen by an EMT, just wanted it reported. Later the employee was brought to the hospital for evaluation. Employee was removed from work.

Over-exertion in lifting objects

Alleged occupational illness. Employee changed batteries on GR18. Employee reported lower back felt sore, while changing batteries; once sat down felt shooting pain in lower back. Employee did not want to be seen by site responder.

Fall onto or against objects

Loader operator reported slipped on stairs, struck knee on grating while going to make oil checks on L48. Operator seen by on-site EMT, and then brought to hospital for further evaluation.

Struck against stationary object

Grader operator was boarding and left knee came into contact with left rear fender support. Evaluated at hospital. Bruised knee.

Bodily reaction, (Not Elsewhere Classified)

Employee stated walking behind haul-ruck, in fuel building felt back pain. Employee seen by responder, taken to hospital. Made no reference to work relatedness. Stated sought chiropractic care previous day. Sept. 5th, its reported employee stated had been performing several tasks that shift; shoveling, walking, placing wheel chocks for haul trucks, pulling fuel hose, greasing etc.

Over-exertion in pulling or pushing objects

Employee reported experiencing discomfort in a finger on the right hand 10-15 minutes after using a wrench to remove a dowel pin. On 4-23-19 EE saw a specialist and has been off work.

Employee was performing a count of shovel/loader teeth requiring to partially move the position of the teeth. Employee reported discomfort in left groin. Evaluated by surgeon and it was made known on 3-6-19 that employee had a hernia.

Fall to the walkway or working surface

Employee reported walking on Drill Pattern 6168 after snow cleaning was performed by a loader. The employee stated that EE lost footing and fell landing onto EE's right side. Employee was evaluated by onsite medical responder, and then later brought to hospital for further evaluation. Employee was removed from work.

Fall to the walkway or working surface

EE was removing item from shelf when EE tripped and fell backwards onto the floor. EE hit head causing a small laceration. Evaluated in ED, mild concussion.

Over-exertion (Not Elsewhere Classified)

While performing routine daily work assignments of walking, mounting and dismounting equipment, employee states stepping up into a service truck (from ground to rear bumper/truck bed) and felt a pop in right calf. Employee was seen by on site EMT and taken to a hospital for further evaluation.

Caught in, under or between a moving and a stationary object

EE reported working on stock feeder belt when a pulley slipped contacting EE's hand causing injury. Evaluated in Emergency room.

Struck by powered moving object

THREE MAINTENANCE REPAIRPERSONS WERE working behind LINE 8 PRIMARY MILL. AS THE FORKLIFT OPERATOR WAS BACKING UP, THE REAR OF FORKLIFT STUCK AN EMPLOYEE IN RIGHT foot and ultimately backing over right leg. Employee sustained bruising and skin abrasions to right leg calf and thigh. EMPLOYEE WAS TAKEN TO UP HEALTH SYSTEM- BELL FOR EVALUATION, X-RAYS WERE NEGATIVE.

EE was pulling a hose near header pipe when EE lost balance and fell injuring left hand. Evaluated at ER and diagnosed with a hand fracture.

Over-exertion in lifting objects

Employee was positioning cribbing and felt tightening of muscles in lower back. Evaluated by NP and given restrictions.

EE scoring wiper with utility knife held in right hand and cut index finger on the left hand with the utility knife. Evaluated by site responder and then taken to local hospital where EE received medical treatment.

A plant repairperson, wearing leather gloves while using an open blade utility knife to trim urethane liner material cut self with utility knife blade. Utility knife blade cut through leather glove and skin of left hand middle finger. Employee was taken to a Hospital, stitches were required to close laceration to left hand middle finger.

Struck by... (Not Elsewhere Classified)

Supervisor was assisting operator in the process of manually closing the South Decant Valve, after releasing the valve handle it sprung back striking employee above left eye. Employee was taken to UP Health System for evaluation. Received stitches to close laceration.

Struck against a moving object

Employee was operating a pick-up truck in load-out area at night while it was dark, drove over two, one foot high mounds of pellets resulting in being jostled in cab, chipping a tooth. Tooth repaired on 5-10-19.

Caught in, under or between a moving and a stationary object

Employee used a torch to cut a piece of 5/8"" mild steel plate in the lower repair bay. The plate employee was cutting was standing up vertically in the plate rack. When employee was finishing the cut, the piece of steel started to fall and employee attempted to catch it but it pinched employee's middle finger as it hit the floor. Employee was taken to a medical facility.
