Over-exertion in pulling or pushing objects

Employee was erecting a scaffold. Employee was attempting to pull two aluminum planks together and felt a ""pop"" in left arm.

Bodily reaction, (Not Elsewhere Classified)

Employee was outside inspecting the area north of the T2 Concentrate Thickeners for a reported leak. While walking in this area, the employee's right foot slid on the ground and the employee reached up with their left arm for balance. The employee felt left shoulder pain. The employee continued to work full duty until they received medical treatment on 11/28/2023.

Struck against a moving object

While operating a 988K Wheeled Loader (Co # SL22), during normal working tasks, Employee stated was experiencing back pain with pain to the right leg. Employee indicated that the pain had become more intense throughout the shift.

Fall to the walkway or working surface

Employee was walking in the contractor parking lot from their car to the company van. As employee was walking, employee slipped and fell onto knee.

Over-exertion (Not Elsewhere Classified)

Equipment operator reported feeling a burning sensation in left shoulder during normal operation of a Cat 24M Grader. Employee reported no traumatic event leading to the reported pain.

Run crew mechanic was laying on their side and reached for a 1-inch drive electric impact. Mechanic reported had felt pain in their rt shoulder.

Over-exertion in pulling or pushing objects

Employee was compressing brake caliper piston using a large pair of channel lock pliers and reported pain in the left elbow area.

Fall to the walkway or working surface

An employee slipped and fell on ice that accumulated in the lower tunnel entrance to the plant building while arriving to work. The employee experienced left knee pain and was sent to the hospital for further evaluation.

Pit electrician was picking up a 13.8kV cable head when force released from the cable and pushed EE to the ground, injuring EE's shoulder. EE is currently in PT.

An employee was kneeling down on a foam pad using a 6"" handheld grinder to remove welds in the #2 dryer when the grinder kicked back, contacting the employee's left inner thigh above the knee resulting in 15 stitches.

Contact with hot objects or substances

Employee was working on Line 10 for 3 conveyor cutting bolts when a hot bolt fell and burned through the employees pants and into their boot. Employee sustained a full thickness burn to the left foot.

Employee reported to supervisor a sore elbow with no explanation before the shift started on 1/11/23. Employee was given light duty. Employee was let go for erratic behavior on 1/15/23. Employee reported on 1/17/23 that elbow was broken. Employee believes it is from jackhammering the night of 1/10/23.

Struck by flying object

Employee reported left eye irritation after drilling on concrete. Employee reports having been wearing PPE at the time.

A Dozer operator reported (2) chipped teeth following routine dozing activities.

Struck by flying object

A welder, removing a lip shroud from a loader bucket, received a laceration to the rt thigh from flying debris resulting from steel-to-steel contact when striking the lip shroud with a sledgehammer. Employee received further evaluation and treatment in the ER.

Fall onto or against objects

An employee was transferring from a fixed ladder to 28B conveyor platform when the employee slipped, contacting their right shin on the platform grating causing a laceration to the shin. The employee was evaluated at the ER and received 3 stitches for the laceration.

Inhalation of radiations, caustics, toxic and noxious substances

Operator was running a D65 Dozer #2 when operator felt ill after the machine started emitting larger than normal amounts of exhaust.
