Accident type, without injuries

Coal barn attendant reported the smell of coal burning at 11:20 am. He inspect coal storage barn, noticed area appeared to be smoldering. He attempted to extinguish, air monitor alarmed. Operator notified supervisor 12:05pm. At 12:10pm, supervisor restricted entry. Contacted Ishpeming Township Fire Dept, ITFD extinguished the smoldering coal. ITFD off-site 2:45 pm.

Employee was trapped in T-2 Elevator traveling at the Balling Floor Level. He was trapped for approximately 50 minutes. MSHA was contacted at about 11:15 AM. No injuries occurred.

Control Rm received high CO alarm and Operator was called out to assess. Operator utilized a gas meter to assess and it triggered high CO levels. Coal barn was assessed. Coal barn showed signs of smoke. Fire dept was called, they provided services and fire was extinguished. On-site personnel monitored area. Investigation shows a damaged electrical cord on the C1 Conveyor.

EE was walking up a stair case and stepped on the 11 tread and it fell out from under his right foot on 10-25-12. He has been treating with a Physician and was taken off of work on 1-10-13.

The employee was walking up a flight of graded stairs, stumbled and his left hand landed on the grading and his fingers went through the grading. He was evaluated by a Physician, taken off of work and hand a surgical repair to the 2 fractured fingers.

Over-exertion in lifting objects

The employee tried to lift grate liners and didn't realized they were stuck together thus causing a strain to the left shoulder. The employee was evaluated by a Physician and had a surgical r epair to the shoulder on June 14, 2013 thus resulting in lost time.

This injury occurred to a driver while on mine property at the Tilden Flux dump site. As the driver was pulling his wheel chocks from underneath the tractor tires in preparation to leave the dump zone, the driver stumbled backwards and stepped on his left foot at an odd angle. After receiving treatment at a local walk in clinic, the driver diagnosed with a broken bone in his foot.

DRIVER WAS HOOKING PUP TRAILER WHEN LOADER BACKED INTO EQUIPMENT CAUSING EQUIPMENT TO STRIKE DRIVER THROWING DRIVER APPROX. 20 FEET. DRIVER SUFFERED BRUISING AND SLIGHT CONCUSSION. PUP TRAILER SUSTAINED HEAVY DAMAGE TO FRAME.

Employee was bent over to cinch a strap on a cable mat while working as a drill helper and he felt a sharp pain in his lower back. He was evaluated by a Physician on 4-13-12 and taken off of work. He returned to work on 4-19-12.

Employee was checking the positions of steam bypass valves. The employee alleges that when he grasped the valve handle and attempted to turn it in the closed direction, he felt a pain in his upper right back. He was evaluated by a physician and taken off of work.

The employee felt discomfort in his bike while removing a 2 ton chain fall from overhead after completing the installation of new liners in the feed chute on line 3 primary mill. The employee was evaluated by a Physician on 1-3-12 and taken off of work. He returned to work as scheduled on 1-9-12.

On 10-11-12 the employee was in seated position, putting a boot on his left foot when he allegedly reported back pain. He was evalauted by a physican and taken off of work. He was released back to work on 10-15-12 with restrictions. On 10-24-12 it was made known that this alleged claim would be accepted as work related.

Caught in, under or between a moving and a stationary object

The employee was exiting the manlift and left shoulder clothing or harness caught on manlift handrail causing a quick lifting of the left arm causing pain to the left shoulder. The employee was evaluated on 9-25-12 and given restrictions.

On 4/17/12, employee sustained a fractured bone in his right hand and a forearm laceration (2 sutures); while attempting to lift the corner of the primary mill vibrating screen to replace a rubber marshmellow, the hammerlock link failed on a loaded chain sling and chain struck employee. The employee was evaluated by a Physician and taken off from work.

Employee allegedly reported pain in his lower back while pulling on a come-along. Employee returned to work and did not seek medical attention at that time. He was evaluated on 4-11-12 and taken off of work by a Physician.

Employee was assisting on the stool change of a crusher. When the crane hook came out of the ring of the mantle lifting jig, it flipped over landing on employees right thumb. he was evaluated by a physician and missed one day of work-2/21/12. Restricted duty started 2/22/12.

Employee was cutting floor grating. The cutting saw bound up and when it freed up a piece of the steel gave away and pinched off the tip of his finger. He was evaluated by a physician and was taken off of work.

EE was hosing out a precip. hopper with the grating cover propped up. The cover fell down onto her left hand. She was evaluated by a physician at the end of her shift, diagnosed with a fracture and is working with restrictions.

Caught in, under or between a moving and a stationary object

Employee was removing a steam exhaust fan (wt 200lbs) from duct work. As it was being lifted, it ""rolled"", the employee attempted to stabilize it and at this point his finger got caught between the fan motor and the hand railing. He was evaluated by a Physician, diagnosed with a fracture and is working with restrictions.

Employee was blowing down a filter chute when concentrate dust got into his eyes. He was evaluated by a Physician and prescription eye medication was ordered.

The employee was stepping out of a cable truck and twisted his left ankle on a rock. He was evaluated by a Physician and was ordered to wear an ankle brace.

On 8-23-12, the employee was cleaning dirt from a tripper car when his rake twisted, causing a left forearm strain. Employee was evaluated by a Physician and Physical therapy was ordered. He completed his second session on 9-19-12.

On 8-13-12, employee felt right knee discomfort after picking up a 35 lb box from the floor. He was evaluated by a Physician and Physical therapy was ordered. Employee completed his second session on 9-5-12 making this reportable.

Employee allegedly complained of right eye irritation stating it started earlier in the week. There was no specific event associated with this. The employee was evaluated, diagnosed with a scratch to the lower eye lid and prescription eye medication was ordered.

EE was in the process of raising staging out of the cooler by hoisting them up with a rope. During the last lift, the staging caught the cooler wall and would not move. The ee was attempting to free the section of staging & when it had released, it came loose & the stored energy struck the ee's left thigh. He was evaluated on 5-17, rtw & given an immobilizer for his L knee.

The employee strained his lower back when maneuvering inside the boiler steam drum. He was evaluated by a Physician and Physical Therapy was ordered. The employee completed a series of sessions on June 11, 2012.

Employee was working on a counter weight of a conveyor belt when dirt went in his eye. He was lifting overhead when dirt went in his eye. He was evaluated by a physician and prescription eye medication was prescribed and used.

An 18"" pipe wrench fell approximately 36"" onto employees right elbow. He was evaluated by a Physician on 3-28-12 and Physical Therapy was ordered. He is currently participating in this.

EE complained of numbness and pain in both hands alleging it to was related to work activities. He was evaluated by a Physician and it was made known to be an Occupational Illness on 2-4-2013. He continues to work full duty.

The employee complained of numbness in both hands relating this to his work activities. He was evaluated by a Physician and diagnosed with CTS. It was made known on 10-2-12 that this diagnosis was occupationally related.

EE claims to have numbness in his fingers and his hand alleging it to his work activities. He was evaluated by a Physician, diagnosed with Left Cubital Tunnel Syndrome and it was made known on August 28, 2012 that his work activities appear to be a significant contributing factor to his condition.

An employee complained of pain/numbness in both wrists. He was evaluated by a Physician and on June 4, 2012 it was made known that he was diagnosed with Carpal Tunnel Syndrome which was occupationally related due to work activities.
