Showing all 31HE WAS ASSISTING A CERTIFIED ELECTRICIAN IN INSTALLING A 7200 VOLT PLUG (CATHEAD) WHEN HE APPARENTLY CAME IN CONTACT WITH AN ENERGIZED RECEPTACLE ON THE POWER DISTRIBUTION BOX (VCB). THIS RESULTED IN A FATAL.
EE was making electrical repairs to the load side loop on a deenergized pole mounted solid blade disconnect switch. He had finished the repair work and was starting to come down the pole. Some how he received an electrical shock and fell down the pole.
AFTER OVERSEEING THE ACTIVITIES OF CONTRACTORS WORKING ON THE BELT 614 LEVEL, EMPLOYEE LEFT AND WENT UP THREE FLIGHTS OF STAIRS TO THE 612 BELT LEVEL WHERE THE ELEVATOR TO THE SURFACE WAS LOCA TED. WITHIN A MINUTE OR TWO OF LEAVING, HE FELL THROUGH A 4X4 OPENING THAT HAD BEEN USED TO LOWER PARTS. THE HOLE HAD FLAGGING AT ALL ENTRIES AT LEAST 4' FROM THE EDGE. HE FELL 19' TO THE BEL
EMPLOYEE HAD FINISHED HIS SHIFT AS A DRAGLINE OILER. HE WAS SEEN LEAVING. AT SOME POINT, HE CAME BACK TO THE MACHINE. IT IS THEORIZED THAT HE CAME BACK FOR HIS CELL PHONE WHICH WAS DISCOV ERED LEFT BEHIND ON THE MACHINE. HE FAILED TO NOTIFY THE OPERATOR THAT HE WAS BACK AND HE WAS TO BOARD THE MACHINE. HE WAS APPARENTLY STRUCK BY THE RIGHT DRAGLINE SHOE WHILE BOARDING.
THE VICTIM WAS HAULING REFUSE FINES -100 MESH TO THE IMPOUNDMENT. HE WAS BACKING THE TRUCK TO THE DUMPING POINT WHEN FOR SOME REASON. HE TURNED THE TRUCK THROUGH THE BERM. THE TRUCK TURNED TO THE LEFT SIDE AND SLID DOWN THE EMBANKMENT AND CAME TO REST ON IT'S LEFT SIDE, SUBMERGING THE CAB INTO THE IMPOUNDMENT MATERIALS.
EMPLOYEE WAS WORKING ON A POWER DISTRIBUTION BOXTO DETERMINE WHY A CIRCUIT BREAKER WOULD NOT OPERATE. HE APPARENTLY CAME IN CONTACT WITH AN ENERGIZED CIRCUIT DURING THE TROUBLE SHOOTING PROCES S.
EE WAS CLEANING UP AROUND THE 8 SHOVEL. EYEWITNESS SAYS "A LARGE ROCK CAME DOWN AND HIT THE 86 DIRECTLY ON THE RIGHT SIDE OF THE ROPS...SMASHING THE CAB." EE WAS KILLED INSTANTANEOUSLY BY THE IMPACT ACCORDING TO THE DEPUTY CORONER.
EMPLOYEE WAS FOUND LAYING ON THE MINE FLOOR, APPROXIMATELY 25'INBY SPAD #15181 IN THE #4 ENTRY ON THE 3 RIGHT PANEL, 001 RETREAT MINING SECTION. HE HAD APPARENTLY BEEN RAN OVER BY A BATTERY POWERED COAL HAULER.
DRIVER GOT OUT OF TRUCK, NOT SETTING PARK BRAKES, PULLED HAND BRAKE. WAS FOUND PASSENGER SIDE OF TRAILER WITH SERVICE LINE DISCONNECTED. DRIVER WAS LAYING ON GROUND HEAD TOWARD FRON OF TRUCK, TANDEMS HAD RUN OVER LEGS, STOMACH AND CHEST AREA. TRUCK WAS RUNNING AND HAD MOVED APPROXIMATELY 16' HITTING TRUCK IN FRONT.
CONTRACTOR FELL THROUGH PUNCH PLATE DECKING INTO RAW COAL CLASSIFYING CYCLONE AND DROWN.
EMPLOYEE (MINER OPERATOR) WAS OPERATING A REMOTE CONTROL CONTINUOUS MINER ON THE 002-0 SECTION IN #4 ENTRY ON RETREAT MINING WHEN 3 ROOF BOLTS AND STRAPS BROKE RELEASING A PIECE OF ROCK APPROX 7' X 11' X 8" THICK THAT FELL ON EMPLOYEE WITHOUT WARNING. THE ROCK WAS STRAPPED AND BOLTED WITH 48" FULLY GROUTED RESIN BOLTS.
VICTIM WAS STRUCK BY A RIB ROLL WHICH MEASURED APPROX. 23 FT LONG X 0" TO 42" THICK AND 10 1/2 FT HIGH. HE SUSTAINED MULTIPLE INJURIES WHICH LED TO HIS DEATH.
EMPLOYEE HAD A MOTOR VEHICLE ACCIDENT. HE WAS DRIVING ON A DIRT AND GRAVEL ROAD. HIS VEHICLE OVERTURNED AND HE WAS THROWN FROM THE VEHICLE. THE VEHICLE THEN ROLLED ONTO EMPLOYEE.
EE WAS CUTTING A 22" DIA. OAK TREE, APPROX.45 FT IN HEIGHT. PRIOR TO CUTTING THIS TREE A 5" DIA. TREE LOCATED UP THE HILL FROM THE 22" OAK HAD BEEN CUT AND WAS LODGED IN THE TOP OF THE 22" OAK . WHEN THE 22" OAK WAS CUT AND BEGAN FALLING THE5" TREE DID NOT FALL FREE, BUT WAS LIFTED WITH THE 22" OAK. THIS ACTION CAUSED THE 5" TREE TO SWING AROUND HITTING EE IN THE HEAD AND KNOCKING
FALL OF ROOF STRUCK CONTINUOUS MINER OPERATOR.
THE EMPLOYEE POSITIONED A PICK-UP TRUCK APPROXIMATELY 10 FT. IN FRONT OF A 170 WABCO. HE APPARENTLY DID NOT MAKE HIMSELF KNOWN TO THE DRIVER. THE DRIVER PULLED FORWARD FATALLY INJURING THE VIC TIM.
USING A 110V AC SINGLE PHASE IMPACT WRENCH WHEN HE RECEIVED AN ELECTRICAL SHOCK.
A MUS SLIDE OCCURRED ON THE HAULROAD APPROXIMATELY 2000 FT UP FROM GUARD SHACK ON CANTENARY COAL CO'S RED WARRIOR MINE (4607178).THIS SLIDE RESULTED IN LIMBS BLOCKING THE HAULROAD PASSAGE.THE VICTIM GOT OUT OF HIS TRUCK AND WAS CUTTING THE DEBRIS OUT OF THE WAY WHEN ANOTHER TREE UPROOTED AND FELL STRIKING HIM ON THE HEAD.
VICTIM WAS DRIVING A LOADED COAL TRUCK DOWN A ELEVATED ROAD WAY AND APPARENTLY TRIED TO SHIFT GEAR HE GO THE TRUCK IN NEUTRAL AND COULD NOT GET IT BACK IN GEAR WHILE GAINING EXCESSIVE SPEED.HE DROVE THE TRUCK INTO A STOCK PILE KILLING HIM INSTANTLY.
AN OVER THE ROAD TRUCK DRIVER, WAS FATALLY INJURED WHEN A SECTION OF DRISCO PIPE 50 FT LONG 26INCH IN DIAMETER WITH WALL THICKNESS OF 21/4 INCHES AND WEIGHTING APPROX. 3800 LBS, ROLLED FROM TH E SECOND ROW OF THE LOAD. STANDING ON THE HAULAGE ROAD APPROX. 10 FT FROM THE LOAD ON THE DRIVER S SIDE OF THE TRUCK WAS STRUCK & CRUSHED WHEN THE PIPE ROLLED OVER HIM.
WHILE DOING PILLAR EXTRACTION MINING ON THE 001 SECTION A MASSIVE ROOF FALL OCCURRED. THE FINAL PUSH ON THE 2 LEFT BLOCK WAS BEING MINED. THE ROCK FELL ABOVE PERMANENT ROOF SUPPORTS AND OVERRO DE THE SUPPORT POST ALLOWING THE CONTINUOUS MINER AND COAL HAULER TO BE TRAPPED. THE INJURED WAS POSITIONED OUTBY THE MINER AND BETWEEN THE BREAKER AND ROADWAY POSTS OBSERVING THE ROOF. WHEN
A JOY CONTINUOUS MINING MACHINE CONTRIBUTED TO THIS FATAL ACCIDENT WHEN THE MACHINE INFLICTED CRUSHING INJURIES. DUE TO THE IMPROPER BODY POSITIONING OF THE MINER OPERATOR (THE FATAL VICTIM).

AT THIS TIME VICTIM CONTACTED

THE COAL HAULAGE TRUCK WAS HAULING COAL DOWN THE MAIN HAULAGE ROAD TO THE PREPARATION PLANT WHEN THE EMPLOYEE LOST CONTROL OF THE VEHICLE. THE TRUCK LEFT THE ROADWAY IN A CURVE ANS APPARENTLY ROLLED AT LEAST ONCE THROWING THE OPERATOR OUT OF THE VEHICLE.
