The employee received fatal injuries when he was struck by a rock that fell from the mine roof. He was involved with longwall shield recovery and was building wooden cribs in an area where a shield had been removed. While in the process of constructing the cribs, he was struck by falling roof material.
Victim, experienced miner working inside fan structure with another ee using grinder 8-10 ft from secure barrier to protect from shaft area. While grinding, other miner noticed ee missing. After search, his body was discovered at shaft bottom. Incident didn't involve non-compliance w/MSHA rules/regs. Investigation ongoing, but appears victim voluntarily jumped to his death
VICTIM WAS OPERATING REMOTE CONTROLLED CONTINUOUS MINER IN THE #4 ENTRY. PILLAR #131 ON THE 9 LT. SECTION WHEN THE TOP STARTED WORKING IN THE ENTRY. HE WAS RUNNING FROM POSSIBLE ROOF FALL AND WAS RUN OVER BY SHUTTLE CAR WHICH HE WAS LOADING.
EE WAS CUTTING A 22" DIA. OAK TREE, APPROX.45 FT IN HEIGHT. PRIOR TO CUTTING THIS TREE A 5" DIA. TREE LOCATED UP THE HILL FROM THE 22" OAK HAD BEEN CUT AND WAS LODGED IN THE TOP OF THE 22" OAK . WHEN THE 22" OAK WAS CUT AND BEGAN FALLING THE5" TREE DID NOT FALL FREE, BUT WAS LIFTED WITH THE 22" OAK. THIS ACTION CAUSED THE 5" TREE TO SWING AROUND HITTING EE IN THE HEAD AND KNOCKING
EMPLOYEE WAS WALKING UP THE BELTLINE TO TAKE UP AREA WHILE 3 OTHER EMPLOYEES WERE PULLING TO TRACK A ROLLED UP ROLL OF BELT.UNKNOWN TO THE (3) EMPLOYEES THE ROPE HAD BECOME CAUGHT ON AN ANCHOR .THE ROPE PULLED LOOSE STRIKING THE EMPLOYEE IN THE HEAD AND NECK.
55036NGWALL FOREMAN WAS LOWERING LONGWALL SHIELDS IN PREPARING THEM FOR TRANSPORTATION UNDERGORUND WHEN THE NO. 008 WHIELD COLAPSED WHILE HE WAS RELEASING YIELD CAPSULE PRESSURE ON THE TILT JA 32203HIS RESULTED IN PINNING HIM UNDER THE CANOPY AND INSIDE THE SHILED CAUSING INJURIES TO THE CHEST CAVITY AND OTHER INTERNAL INJURIES.
