A JOY CONTINUOUS MINING MACHINE CONTRIBUTED TO THIS FATAL ACCIDENT WHEN THE MACHINE INFLICTED CRUSHING INJURIES. DUE TO THE IMPROPER BODY POSITIONING OF THE MINER OPERATOR (THE FATAL VICTIM).
