Employee was attempting to remove a steel plate from the metal rack when the plate slipped out of their hands landing on their right foot.

Employee alleges that while operating the water truck on 8/25/23, employee hit a bump causing a back pain. Employee did not report the incident until 09/05/23 and sought medical treatment on 09/06/23. While this incident is still under investigation, this 7000-1 is being reported within the 10 business days of receiving medical treatment.

Employee was setting the water jacket in a 7 foot crusher. The water jacket shifted and pinned the employee's leg between the water jacket and socket causing a fracture.

While torqueing wheel fasteners on a 785 truck the technician placed hand in the path of the reaction arm causing the hand to be crushed between the arm and the wheel.

Employee was cleaning up with S925tx. The employee stated material fell from the conveyor frame striking the employee.

Employee pinched a thumb while operating a chain hoist.

The employee was working in the primary box when they struck left shoulder injuring it.

The employee was injured while removing a impact feed plate on a screen. When the employee was removing the plate, the plate slipped above gloved hand and lacerated forearm. Employee was taken to the ER where employee received nine stitches.

Employee attempted to pull a pump out of mud using discharge hose when his hand slipped and contacted the hose clamp resulting in a laceration to the ring finger.

On August 21,2013 employee was exiting the conveyor frame when he slipped an caught himself with his arm on the handrail hurting his shoulder. He did not seek medical treatment until November 7,2013.

Our boom truck contacted overhead powerlines, resulting in injuries to an employee on the ground near the load being lifted.

Employee was using a hand grinder with a brush attachment to remove rubber material from a screen support tube being resurfaced. As material was being removed, the attachment caught on the rubber causing the grinder to kick back on EE. The results from this action caused a laceration to the upper left arm area requiring seven stitches to close the wound area.

EE was lifting a bearing overhead when he felt pain in left shoulder. Not determined job related until 7/18/07.

A driver was driving haul truck from pit to primary crusher. Another employee was driving empty haul truck from primary crusher to pit. Due to inattentiveness of the driver, the empty truck slid into loaded truck.

reported 8/8/06 - Stemming holes, an operator at the quarry dropped a stemming box, hitting back, head & shoulders.

USING A UTILITY KNIFE TO CUT LINATEX RUBBER TO LINE A FLUME. THE KNIFE SLIPPED OFF LINATEX RUBBER, CUTTING INTO FOREARM.

EE STEPPED OFF OF BACKHOE TO LOAD HEEL PLATES FOR LOADER AND TWISTED ANKLE.
