Employee was standing on a ditch bank when it collapsed. He fell into the ditch and was covered/crushed by falling material
planning and communication issues contributed to the injury.
Got sand in EE's eye from wind swilling around. EE was wearing safety glasses
Employee was moving water cannon and strained shoulder.
Moving cannon direction. Sprain of left shoulder
Employee was cutting metal with a side grinder with a cutting disk when it grabbed the metal and kicked back hitting employee in the upper thigh cutting leg requiring stitches.
Impact fracturing right ankle.
Employee was using a cutting torch to cut a piece of pipe. The pipe shifted, knocking the torch toward employee's left hand and setting glove on fire. Employee suffered 1st degree burns on left thumb and hand.
Employee sat an excavator tooth on a handle of water cannon. Employee moved handle causing tooth to fall on top of head. Employee had removed hard hat for unknown reasons. Employee received a cut that required a couple of stitches.
EE WAS OPERATING EQUIPMENT TOO CLOSE TO EMBANKMENT WAS NOT WATCHING EMBANKMENT COLLAPSED ALLOWING DIRT TO SURROUND MACHINE EE SUFFERED BROKEN LEG.
EE WAS OPERATING MACHINE TOO CLOSE TO EMBANKMENT. THE EE WAS NOT WATCHING EMBANKMENT WH IT COLLAPSED ONTO MACHINE. EE WAS PINNED TEMPORARILY BY HIS FEET. EE THEN DISEMBANKED MACHINE AND WAS TA KEN TO HOSPITAL WHERE HEWAS TREATED FOR BRUISE AND RELEASED.
