Was replacing a hydraulic hose when it slipped out of their hand and hit them on the nose.

Employee was cutting liner material with a utility blade, the blade slipped from the cutting path and cut the employee's hand.

Employee was changing screens on the wet shaker when the rail fell on EE's right hand injuring EE's middle finger. Employee went to clinic for treatment for possible fracture. Employee was found to have a BAC of 0.04.

EE lifted the hood to replace a welding rod and a foreign object landed in EE's right eye.

EE was in the employee parking lot after finishing EE's shift and 2 co-workers noticed that EE was acting confused and suddenly fell to the ground. 1 worker took EE's shirt off and put it under EE's head, they noticed EE was going into some sort of seizure so the other worker called 911, the paramedics arrived in a few minutes.

Was dislodging a rock with pry bar, rock dislodged and pushed bar and hit hands on feeder box.

Injured right forearm while shoveling.

While welding a hot piece of metal fell onto his shoe. Burned through his shoe and burned the top of his left foot.

While walking toward water truck, EE tripped over a pile of concrete and fell on his left knee and hand.

While servicing screens on a dry shaker a 48' x 40" section of deck was removed to gain access to a lower deck, about 4.5 feet below the top deck. This section was located at the bottom of a stairway. EE came down stairway, forgot that the section was removed and fell forward catching himself with his right forearm on the edge of the opening.

While working on loader, Injury was operating the truck mounted crane, when it moved unexpectedly, pushing a metal plate that he was holding and catching his left ring finger between the metal plate and bolt, pinching his finger.

While shoveling, he strained his left shoulder.

coming down from loader he hit the bent step and hit his lower right leg.

Working at the rock plant. When a rock came down and hit him on the top of his neck.

Cutting a piece of angle steel, piece swung down and cut his right leg. He was given a tetanus shot and that made the injury a recordable accdient.

MOVING METAL WHILE REPAIRING PLANT & BRUISED HIS RIGHT WRIST.

HE WAS CUTTING A TOOTH OF METAL OUT OF THE 5 1/2 CONE CRUSHER WHEN IT GAVE WAY AND SHOT OUT AND HIT HIM IN THE FACE.

EE WAS GRINDING A PIECE OF METAL. SOME GOT UNDER HIS PROTECTIVE EYE WEAR AND INTO HIS RIGHT EYE, CAUSING 2 SMALL AREAS OF CORNEAL ABRASION.

THE EMPLOYEE WAS STANDING ON A LARGE MOTOR AND STEPPED DOWN TO THE DECK BELOW AND MISJUDGED THE DISTANCE. HIS FOOT BENT UNDER HIS WEIGHT AND BROKE FOUR TOES ON HIS RIGHT FOOT.

EMPLOYEE WAS PERFORMING REPAIRS ON A GRANE. HAD REMOVED A SHAFT AND FAILED TO REMOVE A PULLY WHICH WAS STRUCK TO THE MACHINE WITH GREASE. EMPLOYEE STEPPED DOWN FROM CRANE AND PULLY FELL, STR IKING HIM ON THE HEAD.

EMPL PUSHING A PIPE UNDER WATER AND CUT RIGHT AHND ON PIPE CAUSING LACERATION WITH TENDON LACERATION.

A SMALL, PORTABLE FLOOR SPACE HEATER (ELECTRIC) WAS LEFT ON INSIDE OF THE CRUSHER/PLANT CONTROL TOWER AND AT APPROX. 6:00 PM (2 HRS AFTER END OF SHIFT), A FIRE IGNITED INSIDE OF THIS CONTROL T OWER.

REINJURED STRAINED HIS RIGHT KNEE PREVIOUS INJURY OCCURED ON 5-5-89. EE WAS SHOVELING WHEN HE FELT A SHARP PAIN IN HIS RIGHT KNEE.

FELL DOWN IN TUNNEL,HIT RIGHT LEG BELOW KNEE ON WALL.
