Scrapped right shin on the basket of a manlift

The mechanic was attempting to reinstall a lift cylinder on an articulating dump truck in the parking area at Holliday Rock Jobsite. The strap that was attached to the crane and the cylinder slipped causing the cylinder to fall and strike the mechanic on the right-side causing accident to ribs and right arm.

Wet roads, and employee lost control of haul truck and ran into side of embankment.

Hosing down a conveyor belt when EE lost footing and strained the left ankle.

EE was opening the valve of a water tank when EE strained EE's right shoulder.

EE was walking down stairs when EE slipped and landed on EE's right knee.

EE was lowering a front door plate on a wet screen when it slipped out of EE's hands and hit right shin.

Employee was lifting a steel plate and twisted their back.

Employee was loading a cutting edge up onto the back of a truck and it cut their pinky finger.

Employee was walking next to the sand conveyor and hit knee on the tail pulley

Considered to be a medical issue/condition

Employee was loosening the bolt for a turn buckle of a motor to release the tension to replace the v-belt. When loosening the bolt, the turn buckle fell from its position crushing the tip of employee's left hand pinky finger crushing it between the turn buckle and the platform.

EE hit hand on a piece of angle iron cutting left hand.

EE was climbing down a ladder after working on a shaker screen when EE felt pain in EE's left bicep.

EE says lower back started to hurt while shoveling

Employee was cutting vegetation with lopping sheers. Employee twisted their body and strained their left ankle.

While monitoring the pit line, employee stood up and turned swinging hand into roller, employee's hand made contact with troughing roller. Roller pulled arm in up to elbow. Employee pulled arm out on own.

Miner was using pry bar to free up a chain on a sprocket. When miner pulled back on bar it slipped off the sprocket, hit a steel plate pinching finger between bar and plate.

Stepping down off of tractor onto level ground twisted left ankle.

While cutting a belt with a razor knife. Employee cut his left 2nd finger when the magnet above where he was working pulled at the razor knife, causing the knife to veer from its intended path.

While using the Stairs at Jadair, EE foot slipped from the stair causing his right ankle to twist.

Miner was making repair. He was turning a wrench in upward direction when he injured his back.

While repairing crusher, the chute that feeds the fine crusher fell onto ee hand, smashing his left ring finger. Crushing injury to left ring finger.

Shoveling material while under conveyor while bent over. Pulled back out while reaching for broom.

Stepped on uneven ground and twisted his ankle.

Working in cone crusher area, inspection plate came loose and cut inside of his leg, while on the platform

EMPLOYEE HAD JUST STARTED TO CHANGE A SHOUD HOOP. IN ORDER TO REACH IT, HE WAS STANDING ON THE LOADER BUCKET. THE BRAKES ON THE LOADER FAILED AND THE LOADER ROLLED BACKWARD ABOUT 4 FEET. HE JU MPED OFF THE LOADER AND LANDED ON A 8 INCH PIPE WHICH TWISTED HIS RIGHT FOOT CAUSING A BROKEN RIGHT FOOT.

EMPLOYEE WAS REPLACING A BEARING ON HEAD PULLEY ON MAIN CONVEYOR BELT. THE SHAFT SLIPPED OFF THE JACK, STRIKING THE EMPLOYEES LEFT INDEX FINGER.

WHILE ATTENDING TO DUTIES,SCRAPED RIGHT HAND ON SHARP HOOKS OF SCREEN CLOTH-CUT HAND.

EMPL FELT SORENESS IN RIGHT SHOULDER DUE TO REPETITIVE MOTION AND ACTIVITY OVER PROLONGED PERIOD OF TIME.ALSO HAD THE FLU WHICH ACCENTUATED SORENESS.

CLEARING OUT ROCK AROUND ROCK CRUSHER, LIFTING HEAVY ROCKS, FELT PAIN IN BACK. WAS SHOVELING NEAR BELTS @ CRUSHER.
