Transformer in tub of dragline was not properly guarded and locked out. 3rd degree burn on right arm from flash arc.

IE noticed that a battery cell was popping and attempted to cut the cell strap. A screwdriver contacted the case causing an arc flash. IE turned away lost balance, fell spraining IE's wrist as IE attempted to brace self from the fall. The IE was also experiencing eye irritation in both eyes. IE was taken to ER for treatment and prescribed an antibiotic ointment for eyes.

The serviceman started the light plant to service it when he noticed a cable lying underneath the plant. He knelt to move the cable when he received a shock from the damaged energized cable. He was able to free his hand but received burns to fingers.

IE was inspecting #1 drag generator on BE-1570W dragline when the generator brushes flashed. The flash resulted in eye exposure to the electrical arc. IE was seen by ER doctor. No blisters or foreign material were detected under ultraviolet light, but the ER doctor prescribed an antibiotic ointment as a precautionary measure until the IE could be seen by an optometrist.

Employee was hooking up a come-a-long in the revolving frame and came into contact with energized fuse clips.

Employee states while rock dusting he was shocked due to static build-up and discharge in the line. Employee was examined in the ER and per medical review he was not able to return to work.

Employee was changing batteries on 516 scoop. He was trying to pick up the charge battery, when the jumper cable got pinned between the scoop and the battery. The employee then tried to remove the jumper by hand. The cable arced and the employee's left arm was burned.

Two employees stated he was throwing material across the trolley wire when his hand contacted the trolley and another employee used a wooden crib to push him away from the power. He sustained an injury to his shoulder.

Testing pump motor high voltage with clamp-on meter. In the process, contacted energized phase conductor while switching meter from "on" to "off". Resulting damage - current flow through middle finger of right hand and exiting middle knuckle.

Two EEs were installing a new receptacle onto a scoop battery. One of the conductors feeding into the receptacle was misaligned. Without wearing protective gloves, he attempted to align it by gripping it with a pair of insulated slip joint pliers. Apparently the insulation failed and a short circuit occurred causing an electrical arc which burned his left index and middle fingers.

Injured employee and supervisor were hanging the loader cable while loading out a sump. Both employees received an electrical shock from the cable. The cable had been repaired earlier in the shift by the section mechanic.

Employee is a Roof Bolter and had just finished bolting a place. He backed the bolter up out of the way and him and the other bolter started hanging their cable up to the roof. The cable was laying in a small mud hole and when the injured picked up on the cable he recieved an electrical shock.

EE stated that he locked & tagged out the power to #94 Coal Hauler Charger & then replaced the receptacle on #3 Battery & receptacle on #94 Charger. He then connected jumper cable to battery & charger. He stated he then was attempting to bolt a fuse to a shunt when an electrical arc caused electric burns to both hands.Result of grounded lead & power from battery

Mechanic was jump starting a D-10 dozer. After hooking up jumper cables from his service truck to the dozer battery, he mounted the dozer to start. As he stepped up onto the cab platform, one of the batteries blew the top out of it, spraying him with acid. Some entered his right eye. Was flushed out and taken to the E.R. Was released to follow up with an ophthalmologist.

EMPLOYEE REPORTED: BREAKER FOR RAM CAR BATTERY WAS DEFECTIVE. WHEN INSTALLING A NEW BREAKER HE HAD FAILED TO DISCONNECT THE C-TAP LEAD (ENERGIZED). THE LEAD SLIPPED FROM HIS GRASP CONTACTED THE NEGATIVE BATTERY LEAD RESULTING IN AN ELECTRICAL FLASH BURN TO BOTH HANDS.

EE AND OTHER WERE CHANGING THE FEMALE BATTERY PLUG RECEPTACLE ON A SET OF #18 COAL HAULER BATTERIES. HE HAD CONNECTED THE POSITIVE LEAD AND WAS CONNECTING THE NEGATIVE LEAD. HIS RATCHET AND EX TENSION CAME IN CONTACT WITH THE BATTERY CASE CAUSING AN ARC. THE FLASH BURNED BOTH OF EE S HAND MOSTLY ON TWO FINGERS ON EACH HAND.

EE STATED THAT HE WAS SITTING APPROX 2 FT AWAY FROM THE SHEARER CABLE. THE CABLE WAS ENERGIZED AT THE MASTER CONTROL BOX. THE CABLE GROUNDED NEAR THE SHEARER AND THERE WAS AN ARC. THE EE RECEIVED 1ST DEGREE BURNS ON BOTH HANDS AND THE NECK AREA. LOST TIME DID NOT START UNTIL 10/12/02.

EE WAS PUTTING A COAL HAULER BATTERY ON CHARGE AT THE TIME OF THIS INCIDENT. HE HAD UNPLUGGED THE HAULER FROM THE CHARGER. HE WAS INT HE PROCESS OF PLUGGING THE CHARGE CABLE INTO A BATTERY NEE DING CHARGED. WHEN HE PLUGGED THE CHARGER CABLE INTO THE BATTERY THE CHARGER KICKED ON AND THE CHARGER CABLE PLUG SHORTED OUT AND ARCED CAUSING A BURN ON HIS HAND.

INJURED WAS JUMP STARTING A LIGHT PLANT WITH JUMPER CABLES HOOKED FROM THE WELDER ON A SER- VICE TRUCK TO THE BATTERY OF A LIGHT PLANT. AS HE DISCONNECTED THE JUMPER CABLES FROM THE LIGHT PLANT BATTERY IT ARKED, CAUSING THE BATTERY TO EXPLODE. HE HAD CHEMICAL EXPOSURE TO HIS (R) EYEAND FACE. HE WAS TAKEN TO THE HOSPITAL(ER) WHEREHE WAS EXAMINED. RELEASED BACK TO WORK W/O RESTR

EE WAS CHARGING BATTERIES ON NO.3 COAL HAULER & WAS DISCONNECTING THE COAL HAULER POWER LEAD FROM THE JUMPER CABLE WHEN AN ELECTRICAL ARC OCCURRED & BURNED HIS LEFT ARM.

EE WAS TROUBLE-SHOOTING HOIST/PROPEL MODES ON 333 SHOVEL; HE WAS IN PROCESS OF MANUALLY EXERCISING HOIST CONTACTOR, USING SCREWDRIVER, WHEN HE EXERCISED CONTACTOR, A ELECTRICAL ARC FLASH RESUL TED, CAUSING SECOND-DEGREE BURNS TO HIS RIGHT HAND.

ELECTRICIAN PULLED A WIRE FORM ITS LUG AND IT CONTACTED ANOTHER PHASE CAUSING A SHORT AND ELECTRICAL FLASH CAUSING BURNS TO CHEST, NECK, AND FACE REGIONS.

EE STATED: AFTER DE-ENERGIZING MBC POWER CIRCUIT, REPAIRS WERE MADE TO THE POWER CABLE, WHEN REPAIRS WERE COMPLETED, AN ATTEMPT WAS MADE TO REPOWER THE MBC CABLE. WHEN THE BREAKER WAS RESET, A N ELECTRICAL FLASH OCCURRED, BURNING HIS HAND.

THE EE STATED HE WAS CHECKING GROUND MONITOR SYSTEM WITH A JUMPER WHEN THE PHASE CAME IN CONTACT DUE TO ONE PHASE ON BREAKER WAS IN .THIS RESULTED IN ARC

EE REPORTED HE WAS REACHING OVERHEAD ATTEMPTING TO HAND A 6 TON COMALONG ON PADEYE. HE FELT PAININ LOWER ABDOMIN

EE SATE DOWN ON TRUSSBOLTER BUMPER AND LEANED BACK ON TRUSSBOLTER CABLE GETTING A SHOCK TO HIS BACK.

EMPLOYEE WAS ATTEMPTING TO CONNECT TEST LEADS TO EACH PHASE FUSE BLOCK. EITHER HIS HAND ORR THE TEST LEAD CLIP CAME IN CONTACT WITH TWO PHASES SIMULTANEOUSLY AND CAUSED A FLASH THAT BURNED HI S FACE, LEFT HAND, AND LEFT FOREARM.

EE WAS CHECKING BATTERY ON A GOLF CART WHEN THE BATTERY BLEW UP BLOWING ACID IN HIS FACE AND ON HIS RIGHT HAND.

PUTTING UP BREAKER ON A BELT DRIVE THE BREAKER TURNED ON THE POWER ON TWO CABLES ONE CABLE WAS UNPLUGGED & THE LOCK THAT WAS ON IT HAD THE PHASE LUGS SHORTED OUT EE RECEIVED A FLASH BURN ON HI S LEFT HAND & FOREARM

THE EMPLOYEE HAD REMOVED TO POWER SOURCE LID TO REPAIR A CONTINUOUS MINER. WHEN HE THOUGHT THE MINER WAS REPAIRED, HE TURNED THE POWER TO THE MINER ON THE CHECK TO SEE IF HE HAD SOLVED THE PR OBLEM. WITH THE POWER TO THE MINER ON AND THE MINER WORKING CORRECTLY, THE EMPLOYEE, ALONG WITH ANOTHER EMPLOYEE, BEGAN TO REPLACE THE POWER SOURCE LID. THE OTHER EMPLOYEE RELEASED THE LID A
