Employee stated, EE was walking by the corner in number nine entry, when the corner fell out striking EE on the left side of EE's upper back.
While loading in the number ten entry, a piece of coal rib rolled out striking the employee's left ankle.
While putting a crib block under the continuous miner, the corner rolled striking the employee pushing their face into the side of the continuous miner causing swelling around the left eye. There was no treatment provided at initial doctor visit but they returned to the doctor on Monday 7/21/25 and was treated with prescription eye drops. We were notified of treatment on 7/28/25.
Employee was handling waterline for the continuous miner when a rib measuring approximately 5 feet long x 42 inches wide x 18 inches thick rolled off the corner of a pillar block striking the employee injuring EE's right lower back.
While monitoring MRS gauges, a piece of rib rock fell and struck employee on leg. Resulted in a fractured fibula, ankle and toe.
Employee was at the inch controls, tramming the machine when a rib roll struck employee in the legs. Employee was diagnosed with a non-displaced fracture of the right fibula.

Employee was assisting bolting partner retrieve the steel out of a rib bolt hole when a piece of loose rib fell and struck EE's thumb against the drill mast. This resulted in 13 sutures.
Rib rock fell striking miner on the back. Resulted in a fractured rib.
Employee was operating the off side of the roof bolter, installing the inside roof bolt at which time the rib rolled off, striking the employee's left foot, ankle.

EE upon exiting mine, traveled via POV to medical clinic. X-rays were inconclusive and EE was given time away from work. EE remains away from work at this time.
Employee was bolting on the left side of a full face miner. As employee reached for a bolt with left hand a piece of slate fell from the rib striking the employee in left hand causing a fracture. The employee continues to work. This is a NDL accident.
Employee was installing a rib bolt off of the miner when a piece of slate fell from the rib and struck EE in the left thumb as EE was inserting a pusher steel in the rib bolt hole causing a laceration and fracture to EE's left thumb.
Employee was installing a rib bolt when a piece of rock fell out from the top of the rib and struck the employee in the chest and shoulder. The employee was taken off work for a separated shoulder.
A portion of the coal / rock rib, measuring 8" x 8" x 18", separated from the top of the rib / brow and struck the employee in the shoulder and neck area.
Employee was hanging curtain in the face when EE seen the rib begin to move, EE instinctively put EE's hand up to keep it from hitting EE. The rib bent EE's wrist back resulting in a fracture that requires surgery.
Employee was installing a rib bolt off the miner when a piece of slate fell out and struck EE's right ring finger. The employee continued to work until EE went off work on 9/20/2019 for surgery to pin the fractured finger.
Employee exited scoop and rock fell from rib striking employee on the left knee causing them to fall forward. Incident was reported immediately to MSHA. Employee received full work release from medical facility.
Employee stated was walking by a rail supply car when a portion of rib rock measuring 24" in width, 18" in thickness and 20" in length fell hitting employee's lower left leg around the calf area.
EE was bolting alongside the miner. The EE was bent over putting a roof plate on an 8' roof bolt when a piece of coal (2' by 2' by 3') fell out of the rib and struck the employee in the back, shoulder and neck. EE suffered a hairline fracture of the T1 vertebrae.
Employee was drilling the hole when a portion of the coal rib rolled out behind the line curtain. The very end of the rib struck them in the left calf causing their leg to buckle and they rolled the left ankle. Employee did not receive the blunt force of the rib, only enough struck them to cause the ankle to roll. The rib did not hit the ankle.
Employee was backing the continuous miner when the cutter head of the miner contacted the coal rib on the right side causing the corner to become loose. Employee was unaware of this and went to the cutter head to move the miner cable over. The corner of the rib came out and struck employee's right ankle resulting in a fracture of the tibia and fibula.
Employee was standing in the middle of an intersection when a piece of rib rolled out and struck them in the left leg causing a sprain of the knee.
Employee was drilling in bottom of #11 shaft when a rock dislodged from the rib, striking EE on the head, rendering EE briefly unconscious and causing a concussion and abrasions to the back of the head.
Employee was side bolting on the continuous miner when a piece of rock ( 16" x 6" x 2" ) fell and struck the employee in the right hand. The employee continued to work until 10/25/2018 when they were taken off work to have hand surgery.
While moving hoses out of the way of excavator on shaft bottom, was struck by rib material, resulting in concussion to employee.
Employee was assisting an injured employee and a rib roll struck the employee in the leg.
EE was bolting on the longwall face when the rib rolled out and pushed the EE into the bolter causing a laceration and puncture wound to EE's right elbow area. The EE had surgery to clean out the puncture wound.
EE was working along stage loader at the LW when a piece of slate fell out from the rib and struck the employee in the left wrist, nose, lower body and head. The employee continues to work but started PT on 04/10/2017 and is now a NDL reportable accident.
The EE had pushed the slider tube forward and turned to go get another tube when a piece of rib (24"x18"x4") fell out and struck the EE in the back of neck and hard hat.
EE was hanging a vent tube when a piece of rib rolled out striking the slider tube and pushing it into the EE striking EE in the right shoulder causing a bad contusion.
The employee was bolting along side the CM when a piece of rib (3'x4'x4") rolled out and struck the employee in the back of the head, neck, and shoulders. The employee suffered a concussion and was taken off work.
Employee was cutting on curtain side. EE loaded 2 cars. As ee was waiting for a car to come back ee was cutting, filling ee pan. A piece of sandstone came back from the face at the miner head and struck ee near right eye.
Employee was bolting along side CM when a piece of rib (2'x3'x4") rolled out and struck EE in the back. The employee continues to work but is now receiving care from a chiropractor for EE's injury. This is being reported as a NDL accident.
