Employee stated EE was in the process of bolting the first cut out of 3L on the 007 MMU. After EE finished the first row, draw rock feel from the roof in by the ATRS. Resulting in a piece of draw rock falling into EE's drilling station and striking EE on the right leg.

Employee stated, EE was walking by the corner in number nine entry, when the corner fell out striking EE on the left side of EE's upper back.
While loading in the number ten entry, a piece of coal rib rolled out striking the employee's left ankle.

The Operator was notified on 04/16/2026 that the employee has been diagnosed with Occupational Pneumoconiosis.
Employee stated they were in the process of helping another employee lift a piece of steel when they felt pain in their lower back. Continued to work until 4/15/26.
Employee was striking a claw hammer with another claw hammer in an attempt to pull a belt pin from a mechanical splice, when a piece of metal broke off and struck their right forearm, resulting in a laceration and four sutures.
Employee was hitting a conveyor chain with a 5lbs. hammer, when a piece of metal broke loose and struck them in the left side of their groin.
Employee was setting timbers for the next retreat mining lift inby spad 21417 when the slab cut entry that was previously cut fell to the roof bolts causing draw rock to strike the employee. Employee was coherent and alert while being transported outside. During transportation to medical facility they suffered a cardiac arrest. Incident remains under investigation.

EE was clearing some rocks off bed of EE's trailer that is used to haul block and fell off trailer resulting in injury to EE's shoulder
Employee stated EE was throwing cable over in #7 heading with another employee. After picking up the cable, EE twisted as EE was throwing the cable over and felt a sharp pain in EE's left lower back. EE continued to work until 3/31/2026.
Employee was finishing EE's inside bolt when EE went to drop EE's canopy. The canopy post collar dropped down striking EE's right pointer finger. This resulted in a small distal fracture of the right pointer finger.
Received a CWP diagnosis for employee on 3/12/2026.
Employee was trying to get a rock off the side of the miner using a slate bar. As the rock came off, the slate bar slid down the side of the miner and the employee felt a pop near right shoulder. They continued to work until taken off by a doctor on 3/20/2026.
EE was removing drill steel from a drilled hole. Pusher and extension separated in hole. Pusher stayed in drill chuck, extension stayed in roof. Extension fell out mashing left pinky finger between pusher and extension causing a tuft fracture to the end of the left pinky finger.
Approximately 1:10am, a roof fall occurred on the #1 Section in #8 entry at 12cc 1cc inby spad #4379 trapping 2 Mobile Roof Supports (MRS's) in the fallen material. There were no personal injuries.
EE has been diagnosed with Occupational Pneumoconiosis.
A non-injury roof fall was discovered in the scoop roadway 2 breaks inby punch out. Spad #382 across from fall in the belt entry. Fall measured approximately 25 feet long x 16 feet wide x 5 feet high. Fall has been timbered off and danger signs posted on all approaches. This mine is in non-producing status.
The employee was positioned at the inch control tram controls of the Fletcher HDDR Roof bolter and fell, striking the right side of EE's head, resulting in sutures to EE's right ear.
Employee was moving a miner cable when EE tripped over a rock measuring 6"x12 " landing on EE's right hand and shoulder, EE hasn't missed any work due to this incident but started physical therapy treatments on 4/14/26 which makes it a reportable incident, EE has continued to experience pain on EE's shoulder since EE's fall

The Operator was notified on 2/9/2026 that the employee has been diagnosed with Occupational Pneumoconiosis.
Employee was attempting to slide the forks together on the forklift when EE lost EE's footing causing EE to mash EE's left index finger and middle finger resulting in a fracture and lacerations. 3 sutures in EE's index finger and 2 in EE's middle finger.
Employee was diagnosed with Carpal Tunnel Syndrome
Employee was in the process of loosening a hydraulic hose on a low trac on the 004 MMU. While loosening the hose the pallet forks leaked off squeezing EE's head against the tilt arm of the low trac and the low trac frame. This resulted in 6 stitches.
Employee pulled bolting machine up in #1 heading and spotted a roof bolt, then sounded the roof and stated it sounded drummy and then tried to pull down a piece of rock but was unable to. EE then drilled a hole to bolt it up and was making up a bolt to installed when a piece of rock fell striking EE on the left forearm. This resulted in 5 stitches.
Employee was working on the RCM replacing core breaker and wear shoes while putting the pin through the shoe and core breaker with a hammer something came off hitting their left eye.
Employee dismounted grader to check tire chains, slipped and fell on ice, injuring left elbow. Employee worked at full duty until 2/2/2026, when EE was taken off work by physician.
Positioning bolt into 6" wrench. Operator inadvertently bumped rotation and burrs from wrench caught glove and operators hand was wrapped around bolt causing laceration to palm of hand underneath ring finger extending to middle finger.
Was removing rock from continuous miner when the corner broke off rock and struck them on right eyebrow and nose causing laceration requiring 8 sutures, employee was treated and released at hospital
On 1/25/2026 EE stated that while EE was bending a 5' bolt EE felt a pain in EE's right shoulder. EE was granted a full release and returned to work. On 3/30/2026 Mine Management was informed that EE started Physical Therapy for EE's shoulder.
The Operator was notified on 1/14/2026 that the employee has been diagnosed with Occupational Pneumoconiosis.
Icy conditions on deck of crane. Operator was leaving cab and slipped on deck of crane and fell to same level. The operator grabbed the cab door on way down causing minor strain to right shoulder. The operator's hard hat came off, EE's butt hit 1st causing bruising to right humerus, and back of head struck platform resulting in a contusion and hematoma.
Miner has been diagnosed with Pneumoconiosis.
Employee states EE was bolting in #4, in 4R entry, first turn. EE had just completed installing EE's first bolt on EE's 3rd row and went to swing in to install EE's 2nd bolt. When EE swung in, a piece of draw rock fell from the mine roof striking EE's right foot resulting in a fracture to EE's 1st metatarsal.
The employee was mounting a D11 Dozer, when EE lost EE's balance reaching for the grab rail, falling off the track to the ground.
Employee was sitting in the rear seat of the mantrip. During investigation it was determined EE re-adjusted EE's position in the seat and contacted a bolt installed in the mine roof.
Employee stated they were assisting an electrician to place a rub rail back on the miner, they smashed their left index finger between the rub rail and the frame of the miner causing a laceration, needing two sutures.
Operator finished the cut and was tramming to the next location when a piece of draw rock fell striking the operator on the left arm causing a laceration that required 5 sutures.
The Operator was notified on 12/28/2025 that the employee has been diagnosed with Occupational Pneumoconiosis.
Employee was diagnosed with Carpal Tunnel Syndrome. Safety was notified on 12/19/2025.

A non-injury roof fall was discovered on the #2 Section 1 brk inby spad no. 575. Fall measured approximately 25 feet long x 15 feet wide x 8 feet high. Fall has been timbered off and danger signs posted on all approaches. This mine is in non-producing status.
Employee was tightening lug nuts on EE's permissible 4 wheeler when the lug broke and caused EE to stumble and roll EE's ankle and jolted EE's right leg. EE hairline fractured EE's right fibula and was seen at hospital and was instructed to heal normal and is allowed weight bearing on that leg.
Hogg Contractor bent over to pick up hand tools. After EE stood up, a piece of draw rock dislodged from the roof falling striking EE on EE's right foot. This resulted in a fracture to the #3 and #4 metatarsals.
