Employee was cutting left sump out of #7 head, cable pulled under the miner. Employee shut off miner and walked up to remove cable a piece of draw rock fell hitting EE on the outside of EE's left foot. Rock was approx. 3' long, 3' wide and 5" thick. Employee was seen in the ED where x-rays revealed two broken metatarsal bones and EE received 7 stitches.

While running cable employee was lying on is back, a piece of draw rock fell out 8"thick 2'x2', striking right upper leg. EE received a small cut & bruising. No stitches. EE has loss time.
EE was spotting a corner bolt LOCC #5 entry inby right corner, when a piece of draw rock measuring 39"x10"x10" at widest point fell and cut left forearm. EE received stitches at the ER.EE has no loss time and return to work next day.
Employee was filling in on a roof bolter when a piece of draw rock fell & cut right shoulder. EE went to ER received 4 stitches. No loss time, full duty return to work.
Employee was removing steel from hole after drilling hole, a piece of draw rock fell & hit employee's hand cutting pinky finger on left hand. Employee crushed the tip of little finger & had tip amputated. Employee returned to work on 04/21/2017.
EE was installing roof bolts, EE swung drill head out,a hose caught one of the bolts, EE reached across drill head when piece of rock (2'x2'x2")fell striking the right forearm against the boom, rock slid down & struck the hand. EE was taken to ER,received 3 sutures in forearm,3 in the hand. EE fractured the long bone in the right hand just before the knuckle of the pointer finger.
Employee was in process of installing roof support in #4 entry on 004 section. EE just had got drill steel started in the top. EE engaged the up pressure and rock fell striking left elbow. Causing a cut which at ER was glued but ee followed up with physician who took out the glue and put in 3 sutures and having ee off work.
Employee positioned roof boting machine, set ATRS sounded top and began drilling to install a bolt. A piece of roof rock 5"x5"x2'7" fell from roof, hit ATRS & struck ee on left side of head & neck.
EE was traveling back outside from a remote area of the mine. In location inby Spad #1551 a roof rock 12x16x3 fell striking EE on left side of face & shoulder. Impact of rock caused a 1" cut on side of face did not require stitches. EE lost consciousness for a period & regained consciousness and called for assistance.
Employee was dusting travel way & the hose busted on rock duster. EE went to get repairman. EE was waiting on car to dump on feeder & noticed a piece of rock hanging loose on top. EE didn't feel comfortable leaving it, so used a slate bar to pull the piece that was hanging. EE did not anticipate what was coming with it. An 80"x 44"x 10" piece of rock fell on him.
Employees were moving a feeder to the 004 section a piece of draw rock fell landing on ee left hand.
