Individual attempted to lift a 5-6 ft. long chunk of I-beam (weighing approximately 150 lbs.) and toss it over the handrail of an elevated screen deck to the ground below. The I-beam shifted and pinched their right thumb between the beam and handrail. Individual complained of extreme pain and was transported to Urgent Care where an X-Ray discovered a broken bone in the right thumb.

Team member was tying a boat up when their left hand was caught between the metal railings, resulting in fracture.

Team member got dirt and a small cut on their left eyelid on 6/24/2024. By 6/26/2024, the eye was swollen and became painful. Team member sought medical care at a local eyecare center.

Team member was removing washer bearing from back of log washing machine. While pushing the bearing flange through the wall, their finger was pinched between the wall and the flange causing a laceration to pinky finger which required stitches.

Team member was guiding the leg of a crusher into the foot pedestal. As the foot fell into place, it caught the right ring finger when the leg slid into the stand. Team member sustained a right ring fingertip amputation without bone loss.

MINER WAS INSTALLING A BRACKET ON THE WALL AND BRUSHED THEIR ARM AGAINST ONE OF THE PROTRUDING BOLTS THAT HAD NOT YET BEEN FULLY SECURED RESULTING IN A LACERATION TO THEIR ARM THAT REQUIRED SUTURES.

MINER WAS STRUCK IN THE LOWER BACK BY THE OVERFLOW CHUTE OF A PORTABLE CRUSHER AFTER IT SHIFTED WHILE BEING STOWED ON THE TRAILER FOR TRANSPORT. THE IMPACT KNOCKED MINER TO THE GROUND.

LIFTING 8" PIECE OF STEEL CHANNEL TO REPLACE A LEG/SUPPORT BEING INSTALLED ON AN OVERLAND CONVEYOR. CARRIED ITEM APPROX 30' AND SET IT DOWN. MINER EXPERIENCED RIGHT TESTICULAR PAIN.

TEAM MEMBER WAS LIFTING INSPECTION DOOR TO VSI. WHILE MOVING TO GET A BETTER LEVERAGE ON THE DOOR TO USE TWO HANDS, THE DOOR SLIPPED AND LANDED ON TEAM MEMBER'S LEFT THUMB. THE EVALUATION BY THE HAND SPECIALIST INDICATES NERVE DAMAGE AND BONE IS OFFSET AND WILL NEED SURGERY.

Miner was working on a conveyor and cleared the dust off the are they were about to work on and got dust in their eye.

WHILE PERFORMING MAINTENANCE ACITVITIES ON A CONVEYOR, EE DISLODGED MATERIAL THAT WENT BETWEEN THEIR FACE AND SAFETY GLASSES CONTACTING THEIR LEFT EYE. EE WAS EVALUATED AS HAVING A CORNEAL ABRASION AND PRESCRIBED EYE DROPS.

Team Member removed the mantel nut on an HP-400 Cone Crusher while performing regular maintenance. Team member lost grip of the mantel nut while setting it to the side causing there right hand / wrist to be pinch between the cone liner and mantel nut.

Miner was lifting guard under tail drum of VSI and heard a pop in their back, followed by pain.

Miner was lifting hood on fuel truck for pre-operation inspection and back gave out while attempting to lift hood.

A miner was changing a tire on a large piece of off-road equipment when the new tire tipped over as it was being staged in an upright position striking their foot. A groove/run beneath the tire contributed to this incident.

Miner climbed up stockpile to observe conditions of head roll on pea rock conveyor. Miner lost footing and right hand went through drive pulley pinch point, slicing three fingers through miners glove.

Employee was putting an eye on the end of cable. Cable slipped out of employee's hands, striking EE on the chin and causing lacerations to chin and breaking tooth.

Employee was taking down safety rails on screen plant while two other employees were holding rail. Rail slipped and hit employees hand, fracturing right hand.

Smashed finger connecting u-joints while installing drive line.

LEG PAIN/STIFFNESS IN KNEE FOR ABOUT A WEEK BEFORE FEELING A "POP" AND BURNING SENSATION DOWN KNEE AND MID SHIN

While setting up portable crusher, employee bumped right knee on leg of plant.

During the lateral adjustment of a set of forks on a Cat TL1255 Telehandler, the worker cut his left pointer finger while he was pulling and another worker was pushing one of the forks. The fork suddenly slid quicker than was expected, catching his finger between one of the hooks and the bar. He was wearing gloves at the time. his finger received nine (9) stitches.

While changing anvils on a VSI crusher he strained his lower back.

After removing 3'x4' plate steel, tripped on expanded metal protruding from storage rack. Plate steel bounced on concrete striking me on chin, cutting a gash on my chin.

Employee was changing cutting edges on a CAT 980G Wheel Loader. Bolt turned and cut finger.

Lifted 55lb bag of chemicals into hopper and felt pain in back when powder started coming out because powder weight shifted.

Crew was changing a punch plate on an incline screen. While aligning screen, the screen slid down existing deck, hitting employee in the right shin, cutting leg.

Employee was loading holes, he reached out to his right, to grab a 50# bag of ANFO and felt a sharp pain in his lower back. He strained his lower back.

Employee was shoveling rock with a hand shovel. Employee went to lift a pile of rocks with shovel and felt a sharp pain in lower back and fell to ground.

While loading a roll of Petra Matt onto rippers of a dozer, employee felt strain in back and left arm

Pain started in left shoulder. Moved down arm with pain and numbing. Later progressed around chest and then neck pain. Had X-rays taken, found arthritis and became more painful as days went on. Had MRI and found ruptured disk. No knowledge of exact time of injury to disk.

While unloading a punch plate onto a cat walk the employee's middle finger on his left hand was pinched between one of the punch plate openings and the cat walk hand rail. This resulted in a fracture to the finger.

Employee was bent over dragging hose under conveyor. Hose became stuck. Employee pulled hard to dislodge it and felt a pulling sensation in his lower back that gradually became painful during remainder of shift.

While trying to remove a piece of metal from the dredge cutter head, the piece of metal popped out suddenly, striking employee on the cheek.

While trying to move a metal feed disc into place, the prybar slipped out while his right hand was under it. Resulted in 3 stitches.

EE was standing still and then bent over and twisted to take a measurement off of a piece of equipment, heard a loud pop and was then in severe pain in his lower back.

EE SCRATCHED HIS LEG WHEN HE BUMPED INTO A BENCH AT HIS WORK STATION. HE GOT HOME AND NOTICED IT IN THE SHOWER, IT WAS INFLAMED AND HE TREATED IT. NEXT DAY AT WORK EE DEVELOPED A FEVER AND IN FLAMATION CONTINUED TO GROW. HE WENT TO DR AND WAS DIAGNOSED WITH CELLULITOUS BACTERIA. WAS ADMITTED TO HOSPITAL OUTPATIENT AND TREATED WITH ANTIBIOTICS ADMINISTERED BY IV.

EE WAS TAKING A SAMPLE OF 10-0 WITH SAMPLE TRAY SHE SET TRAY ON OTHER SIDE OF BELT & PULLED THROUGH-STREAM OF MATERIAL WHILE PULLING IT THROUGH,SOMETHING GOT CAUGHT ON THE TRAY, FLIPPING IT UP AND PINCHING HER FINGER BETWEEN THE SAMPLER AND BELT CAUSING THE CUT FINGER.

LOADING 4' X 6' WIRE SCREEN, APPROXIMATELY 40 POUNDS, ONTO SERVICE TRUCK, MISSED, RACK DROPPED ONTO LEFT FOOT, MISSING STEEL TOE OF BOOT. PUNCTURED HOLE THROUGH BOOT AND INTO FOOT.

WORKER WAS CLIMBING INTO A HOPPER OFF THE END OF A SCALP SCREEN. HE LOWERED HIMSELF DOWN WITH HIS ARMS AND TWISTED TO GET A FOOTING. HE FELT A PULL IN LOWER BACK.

EE WAS REMOVING ROCK OFF DECK WITH FULL WEIGHT ON LEFT LEG.

TRUCK DRIVER SHUT FINGER IN DOOR

EMPLOYEE HAD BEEN ASSEMBLING A LOG WASHER.

WHILE REPLACING A PIECE OF PVC LINE, EE TWISTED HIS BACK.
