Two employees were clearing material from the feeder box. While using hand tools to remove remaining buildup, an employee's right little finger was caught in a pinch point on the feed box cover, resulting in a fracture.

During the removal of a large piece of metal, the angle iron shifted and struck the employee on the left side of the head below the hard hat brim. Employee sustained a laceration requiring a staple and a tetanus shot. The next morning, the Coworker reported dizziness and nausea and was diagnosed with a concussion.
EE was cleaning rock using pry bar. Pry bar slipped, pinched left thumb between bar and framing, resulting in left thumb contusion.
IW was in the process of loosening a hydraulic coupler on a boom cylinder when EE was sprayed with hydraulic fluid. This resulted in a laceration on EE's nose, and hydraulic fluid in EE's eyes.

EE was installing U-bolt in a bin truck. The jack shifted and struck the ee in the forehead, causing a laceration that needed 11 sutures.
EE's were replacing blow bars inside of a crusher. When the EE cut the steel bands on the skid, the top blow bars slid off and landed on EE's left foot. Two other EE's helped lift the bar off. EE got an X-ray and had no broken bones, only bruising.
The employee was mixing Nitrate and Diesel fuel in the Bulk Truck to produce Ammonia Nitrate. As the Nitrate slid down the bin, the flow of Nitrate stopped because of a crust build up. The employee used a digging bar to break the crust to resume flow. When the bar broke through the crust, the employee fell forward striking the lip of the bin.
EE was attempting to remove a bolt and nut. The nut was welded to a bracket. While using an impact gun, the wrench slipped off the nut, causing the bracket to spin. This caused a cut to the left pinky finger. EE tried to get bracket off by using a wrench so the bracket wouldn't spin, but it did, when the wrench slipped the bracket spun. This required 5 stitches to pinky finger.
Working under a haul truck changing out the tie rod. The ee was using a sledgehammer to assist with completion of this task and felt immediate pain in EE's lower back after swinging the sledgehammer. The injury resulted with lost time.
EE was removing a yoke from a CAT 769C Haul Truck Transmissions. When the yoke popped loose, it slid down the pry bar hitting the floor. it then fell over hitting the EE on the left wrist and arm area.
Removing a PTO from a mobile piece of equipment using wrenches and screwdrivers. A piece of metal got behind the safety glasses and into EE's eyes.
Employee was tightening hydraulic fitting with a 20" adjustable wrench and was pushing down when employee heard/felt a pop in right wrist. (Accident was not reported until 6/3/2021)
Employee was cleaning a box under a conveyor that was shoulder high. EE was using a 6-foot digging bar when EE felt a pain in EE's right shoulder requiring medical examination and lost time. Employee returned to work 9/23/2020
EE was using a bar to dislodge blockage from an oversized chute. When the blockage released, rocks hit the bottom of the bar, pulling the top end into the chute with the EEs right hand causing a fractured pinkie.
Winch got bird nest from cable being frozen on boom overnight and was in process of rewinding cable back on drum when crowbar slipped and EE fell back 4.5 feet of counterweights onto ground on outrigger pad mat.

The crew was installing a bearing on the stacker conveyor. A maintenance worker used a 6 foot pry bar to lift up the shaft to access the bearing. The pry slipped off the shaft and the worker's right shin struck the toe board below. Note: The injured employee received initial treatment for the injury on 11/16/2017.

As the EE was helping to change screens the lifting hook came off of the anchorage point and the screen struck the EE on the head. The EE was wearing a hard hat minimizing the injury. This incident occurred on Wednesday June 18, but the EE did not seek medical attention until Friday June, 20 and was placed on modified duty at that time, hence making this a reportable incident.
Employee was stemming finished bore hole and struck a piece of frozen stemming. Employee felt a pop in right forearm.
As the EE was helping to install a pin to hold the grapple onto the equipment another EE was using a sledge hammer to pound the pin into place. The hammer glanced off of the pin striking EE in the face causing a contusion on the R side of face.
