Employee was cleaning up a large amount of spilled dust, sweeping and shoveling approximately 4000 lbs. Noticed a pain in her left hip. Not better on Tuesday, April 2 (her next reporting day). Went to Doctor, diagnosed as lumbar strain.

Mostrando los 12Employee was splicing telephone wire and employee cut his left hand and required 4 sutures. Employee was wearing gloves and was using the right knife for the job a splicing knife. Employee stated that the knife came loose of the splice and contacted his lift hand.
The employee was using a pipe and a hammer to break up clumps of material in an outside hopper. The employee put one end of the pipe on a clump and went to strike the other end of the pipe. The hammer head glanced off the end of the pipe and the employee's hand that was holding the hammer hit the end of the pipe. The employee broke the second digit on his right index finger.
Employee was installing the ductwork and was tightening down a nut when his ratchet slipped off of the nut. The employee lost his balance and fell backwards. The employee impacted a handrail support that tore through two layers of clothing and left two deep wounds in his upper right thigh.
Employee was assigned the task of removing the liner from a furnace. Castable frefractory had formed around the bottom of liner and employee was using a hammer and chisel to loosen this and also a cum-a-long. Employee got up to get a drink of water when he had a sharp pain in his chest. He was sent to the medical clinic and was given muscle relaxers.
Mechanic was repairing defective links in chain on bucket elevator. He was using a steel punch to unlodge the pins; the steel punch got stuck in the groove and when he jerked it out, the blunt end hit him in the lip. He received 4 stitches on the outside of his lip and 1 on the inside.
Heavy Equipment/Diesel Mechanic with over three years of experience was using a pry bar approximately 5 feet long to straighten a mirror bracket on an end loader. The bar slipped out of the bracket and struck the employee in the lip. Employee's lip was lacerated requiring stitches and lost time away from work.
EE WAS INSTALLING STAINLESS STEEL BANDING ON PIPE. EE WAS GOING TO CUT OFF EXCESS WITH SIDE CUTTERS WHEN THE SIDE CUTTERS SLIPPED, CAUSING EE TO STRIKE HIS RIGHT FOREARM ON THE STAINLESS STEEL BAND - 1 1/2" LACERATION TO THE RIGHT FOREARM.
Two employees were in the process of setting the tension. The other was holding tension on spring with spanner wrench. EE was preparing to put a locking pin which holds spring in place, began to place the pin; wrench broke allowing the spring to release tension. Bolt cut through his glove causing a laceration on right hand in the fleshy area below the thumb. 8 sutures.
While employee was performing a maintenance activity at a height of approx 8 ft, the ee used a retractable tape measure as a pointing device. While pointing at a damaged roller, the tape became entangled resulting in laceration of right index finger, requiring six sutures.
EE WAS TIGHTENING BOLT ON A SCREW. AS WEIGHT WAS APPLIED TO WRENCH IT SLIPPED CAUSING EE TO CUT UNDERSIDE OF RIGHT FOREARM ON BLADE OF SCREW.
EE STRAINED LEFT SHOULDER WHILE USING A "POKE POLE TO DISLODGE KAOLIN CLAY WHICH HAD BRIDGED OVER IN A SHREDDER HOPPER. THE "POKE POLE" IS A PIECE OF 3/4" IRON PIPE ABOUT 10' LONG. IN ORDER TO DISLODGE THE KAOLIN, THE POLE MUST BE LIFTED UP AND DOWN. EE INJURED HIS SHOULDER WHILE DOING THIS.
