EE was opening a bag house door. The employee was using a wrench to loosen the fastening nuts, and when the nut broke loose, the employee left hand struck the frame of the door frame. The impact resulted in a laceration on the left index finger. The wound required 5 stitches to close.

Employee was changing silos by using a bar/pole to switch. Upon using the bar to switch silos he pulled and shifted his weight on his left knee causing a strain. Employee originally reported this as a first aid on 4/2/15 but came back the next day requesting to go to a doctor. The doctor prescribed restricted duties on 4/3 making this a reportable to MSHA.

EE was injured when trolley style manual chain hoist fell during coal mill roll replacement and chains struck him in the lower back. EE was x-ray'd with no impairments found. EE was seen by doctor and pain RX's prescribed and removed him from work for one week due to soreness and inflammation. EE in PT.

Employee sustained fractured rib when come-along hooked to air compressor shifted making contact against his left rib cage.

Employee was cleaning out a plug with a poke bar when he bent his thumb back. He was given first aid at that time but later, on 1/2/2014, stated a desire to go to the doctor. The doctor prescribed Naproxen and gave the employee a splint to wear.

While employee was reaching through a handrail to tighten a u bolt, the ratchet slipped from the nut causing his right wrist to strike against a sharp edge on uni strut causing a laceration. Employee received four sutures.

Tumblers wedged between wear plates of finishing mill. As employee hit wear plate with sledge hammer, small piece of metal tumbler was ejected striking him in the shin area. Metal was imbedded in leg. Required removal and stitches. Released to full duty.

We were change out the bucket teeth on a 385 excavator. The hammer struck the bucket and a small piece of metal hit EE in the abdomen. The silver of metal punctured the skin causing bleeding.

working in coal silo on the preheat coal mill. Turned to grab a 20# wrench. Twisted back to start working and felt sharp pain in lower left back.

While changing electrical switch, screw driver slipped off screw on terminal block stabbing/puncturing left hand which was holding the block.

Employee was removing a nut from pump shaft, when wrench slipped causing employee to strain left shoulder.

Injured was using a pickaxe to remove hardened material from roller mill table when he felt a pain to his lower back.

While using a metal pry bar to attempt to loosen build up of material ee injured his left shoulder. Doctor diagnosed inflammation of tendons and placed on restricted duty.

Employee was lifting a come-along over his head when he felt pain in his right shoulder. Dr diagnosed as impingement syndrome and requested light duty for three weeks.

EE cleaning chute with a scraper bar when he felt a sharp pain in his right shoulder.

Employee was unchoking #3 stone feeder using a pry bar, when the chunk broke free employee struck finger against discharge chute.

The employee was loosening and removing bolts on a vessel door using a 12 inch pipe wrench and felt a kink in his back.

Employee was repairing an FK pump, cleaning air ports with a hammer and chisel, when the chisel struck the left ring finger, causing a laceration.

The injured employee was tightening bolts on nose castings with a custom socket and ratchet when the socket slipped off the bolt hitting the employee at the right eyebrow resulting in a 4mm laceration requiring sutures.

Injured was using a pry bar at the reclaimer to locate harrow chain into guides. The pry bar slipped, forcing the wrist to become twisted abnormally.

The employee was laying next to a large motor to obtain a bolt measurement to ensure he had the appropriate socket and was required to scrape off some build up cement dust. After scraping off the dust, he sat up and felt some dust in his left eye. He received some small eye scratches.

employee was attempting to free frozen material from a chute with a sledge hammer and a prybar. The employee mis-hit the prybar and struck the index finger of his left hand against a piece of angle iron on top of the hopper.

Employee was using prybar to knock down material buildup on pre heat tower. When material was freed up, a chunk struck the prybar forcing it downward, while driving the opposite end in an upward motion. Employee struck his finger against a nearby air cannon, causing a laceration to middle finger on right hand.

Employee was closing a butterfly valve with an 18" crescent wrench on the valve stem. When valve broke loose it caused the employee to strike himself under the left jaw. Neck and tooth sore, cut inside lip, possible chip behind front tooth. His neck remained sore; saw doctor on 7/23/08. Follow-up on 7/30, doctor ordered therapy.

The employee was using a knife to cut insulation from lugs on a motor. The blade slipped and the employee cut his small finger on his left hand.

Employees were sweeping cement dust, EE bumped a wall that had a lot of dust on it. The dust fell on him and he inhaled some of the dust.

Employee mashed left hand ring finger while opening a door on screw conveyor. Fractured tip of finger, is on light duty.

Employee was removing liners from #2 FM, cutting lines and breaking loose with hammer. Piece of metal broke away when liner was struck with hammer, no larger than 1/2" to 1" cut through pant leg which punctured right inner thigh. He received 2 stitches/sutures and returned to work same day.

Employee was working inside discharge cone on finish mill. EE was using wrenches to work on water spray pipe. EE reported injury at this time, but he was not sent for medical treatment until 12/4/07. After medical treatment, EE was diagnosed with sprain in lumbar region.

Employee was using (2) jacks to position kiln tire pads. One of the jacks slipped and EE's left index finger was pinched between tire pad and retaining block. EE failed to properly secure jack and failed to keep his fingers away from potential pinch points. Injury resulted in a pinched finger/fracture and required stitches to close wound.

11/13/2007 shift start. While using metal shear employee cut right index fingernail off.

Injured was using a poke bar to remove buildup from preheater duct under the tire chute. The tire feeder fed a tire into the chute and tire hit the poke bar causing the end to be forced down. The other end of the poke bar which was being held by injured was forced upward striking the employee in the jaw. Injured had four stitches inside mouth

While shoveling cement powder into a pile the shovel struck a raised piece of concrete causing a jolt in right wrist.

While cleaning plugged chute, pry bar hit employee in right shoulder causing pain. Diagnosed with mild shoulder separation.

Calibrating #3 kiln feeder, pulling on adjustment with wrench, he struck his right bicep on feeder framework. A week later he started to experience swelling in right elbow; went to see a physician. Physician diagnosed swelling of bursa sock; referred him to another doctor. The following week he had the elbow drained.

After cutting a piece of channel, co-worker struck channel with a hammer, the piece broke and struck employee in the shin, requiring 3 stitches to close the wound.

EE dropped measuring tape with weight down to measure cement silo. Tape got entangled in screw pump. Pully reeled out of hand, handle on tape caught middle finger & lacerated finger. EE rec'd 7 stitches.

While swinging sledge hammer EE overswung & on follow through held handle with both hands his rt index finger struck damper, ripped off fingernail & split tip of finger open. EE rec'd sutures on finger tip and there was also a fracture to the tip of index finger.

EE strained back while pulling a hoe across the bottom of a gear base removing excess lube oil. He was bending and twisting to his left side and felt discomfort in his lower left back.

Employee was hitting metal plate with hammer, when a piece of steel flew off and struck his leg.

Employee was pulling on a comealong when strap broke, allowing employee's arms to strike against division screens.

Employee was chipping with a hammer during a welding process and a piece of metal lodged in the right eye. He was taken for medical treatment where the metal was removed and he returned to work.

Employee was driving a wedge into a chute a with hammer, and the employee who got hurt reached for the wedge and was struck by the hammer, resulting in laceration right thumb (6 stitches).

While working on a dust feeder (528-20) employees wrench slipped off the bolt and his hand landed in a pile of dust on a weigh feeder. Dust splashed up under safety glasses and into right eye.

The employee mashed the end of his little finger of his left hand while punching out an obstruction on the #2 mill feed belt below #7 weighfeeder. The finger was caught between the punch bar and the chute housing.

Employee was moving a 500 lb reducer in place with a chain hoist. While positioning the reducer employee strain groin

The need arose to switch from synthetic gypsum to additives. The gate failed to open so the injured employee attempted to open the gate manually. He used a piece of angle iron approximately 3 feet long (weighing approximately 4 pounds) to pry the gate open. The angle iron slipped from the gate suddenly. The employee immediately began to feel pain in his back and right shoulder.

Employee was removing a pipe cap with a large pipe wrench when his hand slipped from the wrench. This caused him to stumble and fall, subsequently twising his ankle. The fracture to the ankle required surgery.

EE was attempting to cut plastic pipe with a utility knife. Utility knife slipped and cut ring finger. EE's ring finger was lacerated and re- quired 6 stitches.
