While operating the company No. 405 roof bolter the employee was drilling a 12' hole for a cable bolt in the No. 5 Entry. During the drilling process the operator's Kevlar sleeve was caught between two sections of stack steel where it was bunched up around his elbow and when he engaged the rotation his arm became entangled fracturing his foreman.

Employee was pulling a shield in on the longwall face. In the process of pulling in the shield his foot caught got under the pontoon of the shield and it caused him to smash his right foot. It broke the tips of three of his toes and the employee had to receive four stitches in one of them.

Employee was struck in the mouth by a socket from an air impact gun resulting in a laceration to his upper lip and damage to his teeth.

Employee stated he was operating the roof bolt machine and as he placed the drill steel in the chuck his left hand was pinched between the steel and chuck, resulting in a laceration. Sutures were required to close the wound.

Employee was making welding repairs to the Dragline bucket and a hot piece of metal burned a hole in his pants and went down inside his boot and burned the top of his left foot. Employee went to CAMC Urgent Care on 06/19/2015 and taken off work for a Partial Thickness Burn on his left foot.

Injured employee sustained a laceration to his left thumb when it was struck by a piece of drill steel that fell from the mine roof while he was roof bolting. The wound required 11 sutures to close.

Employee was bolting 3 left and attempted to install a t-3 strap. He installed one side of the strap and attempted to push up the other end with the drill pot. The strap slipped off of the drill pot and sprung back striking him about 1" above his left eye. The employee received 8 stitches.

The employee stated that on 4/20 he was operating the D6 Dozer in the right grain ditch when he ran over a rock and strained his back. The incident became reportable on 5/11/2015 after the employee sought medical attention and removed from work for a back strain.

Williamstown contractor employee sustained a laceration to his right upper thigh while using a hand held grinder to cut a section of the impact bed of the 1 North #2 belt when the grinder kicked back and struck his leg. Sutures were required to close the wound.

Employee was standing beside the C/M operator while he was cutting out a belt channel on 16 Headgate. When the C/M operator was cutting some top, a piece of a roof bolt came back and struck the employee in the mouth.

The EE was mining in #5 heading. He pulled the cable away from the miner and walked towards the rear of the miner. Through the investigation, it was determined that he walked past the standoff while the pump motor was still energized & accidentally pressed the tram lever resulting in the miner pinning his midsection against the rib.

While rib bolting on the miner on the 10 right section, the employee pinched his left hand finger between two drill steels while adding an additional steel to finish drilling his rib bolt hole.

Employee stated he was installing a roof bolt in the face area. He did not know that the rotation hose on the bolter boom had hung on the canopy post. When he pushed the bolt up the hose dislodged and struck him in the chin.

Employee was operating a Fletcher roofbolter and back into an idle miner sitting in the belt entry, pinching his right forearm. He received stitches to close the wound.

Employee was installing roof bolts down the face of the long wall in preparation of moving the long wall. Employee was trying to position the roof bolt plate between the shield and ATRS. The bolt was pushed up, smashing the tip of his left point finger between the roof bolt plated and mine roof.

Employee stated that he was operating the right side of a Fletcher DDR-13 and had just finished installing a rib bolt. He stated that he was preparing to swing the boom inline to drill the next hole in the pattern, when he mistakenly engaged the boom swing out function of the bolt machine.

Employee was trying to free a drill steel when he caught his left hand and suffered an injury. Stitches were required to close the laceration.

Employee was reclaiming a high wall in area N. Employee states that he had pushed to a bench and was in the process of turning to go back up the slope when a rock dislodged causing the tractor to slide over the bank onto the machines right side. This resulted in the tractor overturning approximately three times before coming to rest in an upright position. Received 9 sutures.

Employee stated that he was attempting to remove a bent piece of drill steel from the top when he accidentally engaged the rotation and the bent steel struck him on the right wrist.

Using a torch to cut out bearing and hot metal flew off into Left ear.

Employee stated that he was operating a roof bolter on his right knee and when went to stand up his left foot slipped on a piece of draw slate. This caused him to fall downward striking his forehead on the roof bolter. EE received 9 sutures.

Employee was welding and raised his hood when another employee began grinding in the area. Employee eye became irritated over the course of the week, but kept on working till 6/16. Went to eye doctor on 6/16 and there was a small piece metal in his eye. Safety glasses were being worn under the hood.

Employee stated that while roof bolting on the left side of the Sandvik continuous miner on the 9 Right section the roof bolt that he was inserting into the hole started to bend. Employee then attempted to grab the roof bolt and caught his left hand between the roof bolt and the mast of the bolter causing a laceration and a fracture to his left forth finger.

Employee was grinding on #21302 992G loader. Grinding dust settled in his hood and when welding helmet lenses came down material went into employee's right eye. Employee went to Mountain State Eye Associates and was seen by Dr. A piece of metal was removed that was free floating but employee received a prescription.

Injured employee was in the process of connecting a finishing steel to a starter steel while roof bolting when a fellow employee at the center of the roof bolting machine attempted to reposition the head. This caused the drill steels to move suddenly and strike the injured emplyee's right hand resulting in a bruise.

Employee stated that he was installing a rib bolt and as the bolt entered the hole it bent and then the rotation was engaged, which caused the bolt to strike/twist his hand and wrist. This resulted in a contusion and restricted work duties.

Employee had his left knee resting on a relay bar at shield #17 when someone pushed the panline causing the relay bar to jump up and the employee sprained his left knee. *Employee was not removed from work due to injury until June 3, 2014.*

Employee stated he and another employee were using air chisels to remove a bearing race from a bearing assembly for the hoist drum on the 495 shovel. In doing so, the chisel tool had a piece of metal chip off and struck the employee in the left leg just below the knee.

Injured employee sustained a laceration to the left side of his face while using a hand-held grinder to cut a roof strap along the 8 Right belt when the grinder kicked back and struck his face. The laceration required sutures to close.

Employee was bolting in No 6 entry. As he was drilling a hole, his pusher steel got hung and flew out striking employee above his left eye.

Employee was hanging curtain in #9 entry. The continuous miner was loading gob out of the intersection. A plate picked up by the continuous miner head struck the employee in the left thigh causing a incision that required 3 sutures to close.

Employee states he backed over a rock operating #20529 D10 dozer and was jarred straining his left leg and lower back. Employee was examined by Dr. at Lincoln Primary Care on 2/11/2014 and taken off work for a strain to his lower back.

EE was operating operator side of #3 bolter on #1 MMU. He had left hand on handle under canopy and right hand operating swing lever. There was an overhang on rib . As canopy swung, it went over the top of the overhang and mashed his left ring finger between the overhang and canopy handle. EE received 3 stitches to finger.

EE was installing last bolt in the face of #3 entry of #3 MMU. He put the top section of his finish steel in the roof and the bottom section in the chuck of the bolter. He coupled the two sections together. As he removed his hand , he hit the rotation and feed lever. The pressure from the boom caused the steel to bend and as it rotated, it struck EE in the arm.

Bolter opp. was tramming the bolter from the deck and was trying to back around the corner of 5 L.T. He backed the bolter into the rib and brock the canopy jack that held the canopy up. When the jack broke the canopy fell down striking the employee in the shoulders and back.

A contract mechanic received a laceration to the side of his head while attempting to run the boom on the service truck out. Mechanic toggled the boom swing button and he meant to press the boom extension button. Mechanic was taken to CAMC General ER where he received 4 sutures to close the wound. He returned to work on 12/10/2013.

Employee was bolting on 8A section. He was backing his drill steel out of the hole when the pusher came apart from the finisher. The pusher steel came around and struck the employee on the right side of his face.

Employee was operating the continuous miner and stated that he took the 1st cut on the curtain side and then backed the C.M. up to trim top. After repositioning the C.M. employee began trimming top and walked up beside the C.M. The process of cutting top caused the rear of the C.M. to bounce and bump against the employee's right knee. This resulted in right knee effusion/strain.

The employee stated that he felt something hit the corner of his eye as he was installing a roof bolt. He removed his glasses to wipe his eye and felt a burning sensation in his eye and then flushed his eye but it got worse and he decided to go to the Dr. to have it examined the Dr. prescribed him antibotics to prevent infection and placed the employee off work for two days.

Was drilling hole in preparation to install rib bolt. Drop the boom on the bolt he had prepared for installation and caught his left foot between the roof bolt and the mine floor, causing a contusion to the left foot.

The injured was lowering the man-up on a roof bolter and smashed his finger on the frame of the machine.

EE was cutting, welding and grinding on a scoop frame and some time during the process a sliver of metal became embedded in his left eye.

Shuttle car operator pulled under boom of the continuous mining machine. While loading the s/c, a rock became fowled in the conveyor chain of the continuous miner. The rock dislodged out of the boom & struck the s/c operator in the left arm. Due to new medical information received on 11/15/13, this incident became reportable on 11/15/2013.

Employee was installing a roof bolt in the rib on the 1 West section and as he was inserting the bolt into the rib he hit the top of his left hand on the drill guide. Employee felt pain and a knot raised up on his left hand. Employee worked fifteen days following the incident was off three days came back and worked seven days before being taken off work on Nov. 21,2013.

Employee stated he was rib bolting in the belt entry and felt a pain in his right arm.

The employee stated he was standing in front of the RBM cable when the reel engaged which caused him to fall backwards onto his back. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.

Employee was grinding brackets on a ladder platform. Employees left eye started itching and he removed his hood, safety glasses, and gloves and rubbed his eye. A small metal particle got into his eye and had to be removed by an Ophthalmologist and he was given prescription eye drops to prevent infection.

Employee moved the RBM into the 3 left crosscut and dismounted the operator's deck. As he was exiting the deck of the RBM, the continuous miner trammed into the flypads directly behind the roof bolter, striking the employee and knocking him to the mine floor. This resulted in multiple sprains/strains of the upper body.

Employee stated that he was pulling back on a roof bolt to line it up with the drill pot. As he applied pressure on the bolt he felt something pull in his right shoulder. This resulted in a right shoulder strain.

Injured employee was using an air powered hand drill to install 12' cable bolts in the crosscut #1-2 at 11 block of the 7 Right section. While drilling the roof bolt hole a jack being used to support the roof strap kicked out and struck the employee on the left side of his head.
