EE was on a shuttle car and pulled into a crosscut not fully getting out the way and was hit by another shuttle car. This has now became a loss time claim

Plant Tech was removing ice from a conveyor head roll area. Employee used a bar to remove the ice the bar was pulled into the head roll and frame pinching the techs left hand between the bar and the frame work causing a little finger injury. The employee had surgery to remove part of the distal bone from the little finger. No scheduled shifts were missed.

Employee was operating shuttle car when employee ran over a rock, hurting back. Employee was placed on a back board and brought to surface then transported to ER by ambulance. CT was negative. Employee was taken off work 2/15/2019 pending further treatment.

While operating mantrip, employee view was blocked by case of water. When they reached up to push it out of their way, they struck their head on the overcast of #3 belt. Employee did not seek medical attention until 02/14/2019 when they were taken off work pending MRI of their neck.

Employee was operating a diesel mantrip, Damascus Mac 3D, in secondary travel-way when the ride hit a piece of rock in travel-way bouncing employee's head upward into the roof.

Driver was on a slight descending grade truck was in 4th gear and as EE approached a curve EE was faster that EE should have. Driver passed a truck runaway ramp then jumped from the truck driver was taken to hospital EE had two fractured ribs.

While tramming a diesel scoop, ran over a rock in the roadway, hitting head on the top.

After helping change a set of scoop batteries, employee laid a hand on the scoop bucket. The operator inadvertently bumped the ram lever, catching employee's hand.

EE was riding in mantrip & hit a bump on a section of muddy roadway jarring ee neck. EE started missing work on 5/15/18.

While operating rock truck employee hit a hole on return run & jostled them inside cab. Employee has pulled muscle.

EE was digging in a shot, hit a rock causing loader to jerk unexpectedly & twisted EE's lower back.

Battery end operator side tire was coming apart, tramming car to repairman & a flapper tore fender off & came in deck, hit left arm in bicep area.

EE pulled scoop up to load & turned scoop off & dropped bucket on ground. When exiting scoop began to roll back causing ee to twist and fall. EE complained of pain in left knee. EE has not returned to work since the day of the DOI, 11/6/17.

EE was hauling coal, hit a hole and bounced into the canopy. The mining car was not loaded. EE stove up their neck, they went to ER and CT was negative for anything broken. EE was referred to see family physician to schedule MRI. EE has an MRI scheduled and is to remain off work until seeing doctor for MRI results.

EE was driving a Mac 3D ride when EE felt something pull in EE's right elbow, EE finished EE's shift, then went to the ER & is to follow up with EE's doctor. EE's doctor has taken EE off work until EE see orthopedic specialist. Has loss time starting on 08/07/2017.

Employee was re-hanging cable on off-side(blind spot) of shuttle car in face(#4 entry cut), when the loaded shuttle car moved & bumped them. They did not seek medical attention until 07/10/2017, stating their back was sore. Employee has lost time, but returned to work on 07/12/2017 regular/full duty.

EE was hauling supplies in diesel scoop when hit a hole in bottom bouncing them into the top with their head, EE was wearing a hard hat. EE felt pain in neck/back region. EE went to doctor & diagnosed w/muscle sprain. EE continued to have pain, followed up with dr. and has loss time starting on 07/20/2017.

EE was backing ride up & EE's hard hat struck a roof bolt. EE had neck pain immediately. EE continued to work 03/21/2017 thru 03/24/2017. On the 24th EE worked a half day, EE left early due to pain & went to local ER. EE was seen & release to return to work on the 28th, EE was told to follow up with family doctor. EE saw family doctor on 28th & has EE off until further notice.

While working on a track on H9 HWM the fork loader set fork down on top of the track. There was a slate bar laying on the track, when the fork hit the slate bar. It came up and struck employee in the mouth/bottom lip area.

Employee was coming back from getting bolting supplies and hit head on the mine roof while on the lo trac.

EE pulled shuttle car underneath the miner conveyor boom to begin loading. During loading, apparently a rock stopped the conveyor, resulting in the failure of the boom jack and sending several parts flying through the air, most landing on the shuttle car near the operator's deck. When the jack failed, the pin flew through the air, striking EE on right Knee Cap.

Employee was tramming scoop from #3 to #2. EE went through curtain then held curtain up with right arm. A shuttle car was parked in #2 inby locc with boom sticking out into intersection. EE pinched arm between canopy of scoop and boom of shuttle car.

While digging solid rock with bucket, the back of loader was raised up, during that time bucket slipped, dropping cab back down suddenly, jarring operator.

Driver was attempting to drive his truck down a hill, and although it had just been graded the truck lost its traction and starting turning until it turned over.

Hauling rock to dump area, hit a large mud hole, jarring body and causing pain in neck and back area

Returning to office, heavy fog resulted in bad visibility, truck left the roadway & struck a rock.

A rock rolled over striking side of bed he was operating, jarring him and causing neck pain.

Single vehicle accident - driver injured left leg and back.

Received notification today that this has been deemed a work-related claim. Allegedly strained back while operating rock truck.

Operating loader and had large rock in bucket, bounced loader striking head on inside cab.

On a delivery, EE pulled up to a suspicious vehicle, got out of the vehicle, approached the two men regarding their suspicious behavior, got phone out of the truck, while on the phone writing the plate numbers down, the two men jumped in the vehicle & put the vehicle in reverse striking EE, he fell to the ground hurting his back, the two men drove off.

Attempting to operate loaded truck on an incline when it overturned. This occurred at night. EE had seatbelt fastened and received bruising from belt. Due to size of drive the past was removed from the truck so ee could exit from vehicle. When upright truck started and no damage was done & fan on motor. Drive line was damaged and windshield broke, driver side door also damaged.

992G loader operator mistakenly hit the quick drop on loader while reaching into floor board area to pick up a snuff can. The boom came down on the side of the truck jamming the truck driver; driver felt pain in back.

992 operator was helping move large rock out of pit; bucket slipped off the rock, operator stated that it bounced him up and the seatbelt caught him. Slung him back down and he hit his left elbow against joystick controller.

EE was hauling rock from #1 Point Pond Creek Pit to #2 Valley fill, EE said he hit bump in haul road causing seatbelt to jam his right hip. He said he has pins in hip.

The roads were wet & slippery, EE was trying to pass a coal truck, lost control of his S10 and went to into a slump hole around Marker #22. It skinned his cheek, put a laceration in his knee requiring 5 stitches, and a contusion.

He backed the coal loader into the Highwall. Said he hurt his back.

MVA - Driving unit # 88103 shot loader down a haul road to bins, made a turn, truck flipped making the turn, hurt shoulder.

The 988 coal loader was chopping coal to load and the 992 spread was working same pit. As 988 was backing up chopping coal, the bumper of the Loader hit the bumper on rock truck.

EE was moving beam augers from pit #12 to the parts container. He was dumping them off his forks, when an auger slid off the forks and came through the front windshield. Busted out the windshield and EE got glass in his eyes from the windshield.

Loader caught solid rock on right side jerking operator, resulting in sprained neck.

Loading 777D with 988H (High Lift), tire fell in a hole and EE said he hit his head, causing a neck sprain.

Employee was digging shot rock and hit a hard spot next to highwall with loader. He had been working in shot since 6:00 PM. He said it hurt his back when he hit the hard spot.

992G #L27 was loading in shot at Hollylick Pit #64. He was digging rock. While prying rock, loader bounced up and down. Employee said then that he hurt his back. This took place about 7:50 PM on 2/27/07.

Driving down a steep grade, the truck struck a boulder, hit chest against the steering wheel, body went numb from the neck down

Employee clocked out @ 11:29 AM & was on his way in his personal vehicle. He drove 3.2 miles down the mine when he was struck by another vehicle. Unknown to us the mine did not file this MVA. We just found out the employee wants to file this claim with us. Initially this claim was not filed since he was off the clock. Unknown information @ this time on the other driver.

Truck driver backed truck under loader bucket to be loaded loader operator dropped a rock into truck bed causing Neck problem this incident did not require first aid or rescue squad.
