Employee was moving a miner cable when EE tripped over a rock measuring 6"x12 " landing on EE's right hand and shoulder, EE hasn't missed any work due to this incident but started physical therapy treatments on 4/14/26 which makes it a reportable incident, EE has continued to experience pain on EE's shoulder since EE's fall
Employee dismounted grader to check tire chains, slipped and fell on ice, injuring left elbow. Employee worked at full duty until 2/2/2026, when EE was taken off work by physician.
Icy conditions on deck of crane. Operator was leaving cab and slipped on deck of crane and fell to same level. The operator grabbed the cab door on way down causing minor strain to right shoulder. The operator's hard hat came off, EE's butt hit 1st causing bruising to right humerus, and back of head struck platform resulting in a contusion and hematoma.
The employee was mounting a D11 Dozer, when EE lost EE's balance reaching for the grab rail, falling off the track to the ground.
Employee was tightening lug nuts on EE's permissible 4 wheeler when the lug broke and caused EE to stumble and roll EE's ankle and jolted EE's right leg. EE hairline fractured EE's right fibula and was seen at hospital and was instructed to heal normal and is allowed weight bearing on that leg.
Employee stated EE had unhooked a chain from the MRS on a LoBoy. When EE attempted to step off the LoBoy, EE's foot hit the end of the ramp causing EE to twist EE's left knee. Employee had surgery on 1/21/26 and started PT on 1/22/26. No lost days.

Employee stated as they were exiting the operator's deck. They stepped down onto the mine floor that was slightly uneven and damp to muddy and their foot slipped a little twisting right knee. They continued to work until 11/20/2025.
EMPLOYEE WAS CLIMBING OFF A D10R DOZER AND MISSED THE BOTTOM STEP ON THE PUSH ARM TWISTING RIGHT KNEE
Drill operator had assisted another drill operator and was returning/walking to their drill when they twisted an ankle due to uneven ground.
Employee was working on installing a belt scraper on the 11C head, when the outby belt unexpectedly started. The belt transported the employee approximately 30 feet before it was stopped by a pull switch. Resulted in a broken ankle.
EMPLOYEE WAS WORKING ON INSTALLING A BELT SCRAPER ON THE 11c HEAD, WHEN THE OUTBY BELT UNEXPECTEDLY STARTED. THE BELT TRANSPORTED THE EMPLOYEE APPROXIMATELY 30 FEET BEFORE IT WAS STOPPED BY PULL SWITCH. RESULTED IN A FRACTURED ANKLE.

While walking from AC10 towards power center to put breaker in, stepped in a hole in the travel way twisting left ankle.
Employee was setting bits, turned to go to back of continuous miner and stepped in a hole. EE twisted EE's left knee and fell to the ground.
Employee was tramming roof bolter from the #2 heading to out by the LOCC. EE was walking backwards and holding drill cable slack pulling it back as EE walked and trammed the drill. EE stated EE stumbled over loose gob and catching EE's balance EE stepped on a ledge on corner of the rib at the intersection twisting EE's right knee.
EE was standing on right side fender doing EE's pre-trip inspection when EE went to exit fender EE missed a step and slipped. Then EE fell to the ground on EE's right side elbow/arm. Went to ER, had strain to right arm.
Fell from Maintenance landing while tightening bolts.
Worker was walking on 2nd floor of transfer. EE stepped on unsecured decking. The decking slipped and EE fell to the floor below. EE hurt EE's knee in the initial fall and then EE's chest when EE fell over after landing. EE was wearing fall protection but was not tied off.
While installing roof bolts the employee reached back to get a bolt out of EE's tray and tripped and fell catching self with EE's left hand and fractured EE's wrist.
Employee fell from D 10 dozer after losing balance
Employee was working with others to anchor the tailpiece, after drilling the inby hole on the offside EE was moving drill to the outby hole when EE tripped and fell catching themself with left hand causing a laceration on left pinky finger on left hand requiring 2 sutures
Employee was lapping up belt, when employee slipped and fell, catching self with left hand, resulting in a fractured wrist.
Employee was getting on forklift to check diesel fuel level when EE's foot slipped out from under EE causing EE to fall and twist right knee.
At 11:50 AM EE was assisting a Lube Tech whose airline was frozen. EE provided an air source to the lube tech to complete the job. As EE was unhooking the airline, EE fell on the ice and landed on left shoulder. EE was transported to the hospital by the Safety Representative at the Workman Creek Mine.
AT THE END OF THE SHIFT, THE EMPLOYEE WAS WALKING ACROSS THE PARKING LOT TO EE'S PERSONAL VEHICLE & SLIPPED & FELL TO THE GROUND.
Employee was descending an engine ladder on a 730 rock truck when EE slipped resulting in hurting the left knee. Employee was scheduled off on 12/31 and 1/1 making 1/2/2025 the first shift missed. Employee had a ACL and meniscus repair 3 years ago on the same knee.
Employee was moving curtain from one rib to another when employee tripped and fell back on right wrist. This resulted in a fracture.
While loading bolts in a scoop bucket, employee fell on left shoulder and felt a pop in the shoulder.

Employee was walking through thick mud when EE's left knee popped and started hurting.
Employee stated EE went to the mantrip and was getting miner bits off the front of the mantrip. EE stated EE reached and got two buckets of miner bits and twisted to go back to the stinger. EE tripped over the waterline and caught self and EE's lower back began to hurt on the left side. Lost time as of 9/3/24.
Employee was getting out of coal truck at south side stockpile at Workmans Creek in near creek. IW using 3-point contact, employee stepped down on the ground. The ground was unlevel and EE fell. When falling, EE caught self on EE's right wrist.
After setting up DDR Roof Bolter in the No. 6 Heading, employee stepped off machine and stumbled falling to the mine floor and landing on radio injuring lower back.
Employee attempted to carry two 50 pound bags of rock dust, slipped and fell landing flat on back.

EE stated that while walking the roadway to the Continuous Miner, EE slipped and fell in the # 6 entry.
Employee was getting out of the middle of the shuttle car and twisted their left knee when they stepped on a rock in the mud.
Exiting bulk truck and foot slipped causing employee to fall to the ground. In bracing for the fall the employee strained EE's right wrist resulting in restricted duty.
Employee was carrying 2 bundles of rope bolt plates from scoop to load on roof bolter. EE tripped on power supply cable for roof bolter causing EE to fall into the walkway of the roof bolter. The plates EE was carrying caught EE's middle finger on left hand between plates and floor of walkway. This resulted in a comminuted displaced fracture to EE's middle finger.
The injured employee was dismounting a CAT D10R Dozer and stepped in a hole, spraining left ankle.
EE slipped off the bottom step on the customers lube truck and fell to the ground hitting EE's Back and Wrist. The jobsite had about 12 inch's of snow and EE said EE was wearing all EE's PPE. (Steel toe Boots, Glasses, Reflective Cloths and Gloves)
Employee was walking along a sediment control ditch over uneven ground to check a point source discharge at the outlet for this structure. Employee's foot slipped and employee's body twisted as employee fell to the ground. Employee broke right lower leg.
Employee was walking down the entry and slipped on a ledge hurting their left knee. This was a file only accident and the employee continued to work until 6-22-2024. This is now a Lost Time.
