EE stepped on platform of an isolated catwalk that collapsed beneath them causing them to fall approximately 7 feet to the next level. EE sustained superficial lacerations and bruising/soreness.

Employee was performing Pre-Shift inspection, checking fluid levels 0n 374F cat Excavator and mis-placed footing and fell. injured left shoulder catching themselves during the fall. Stepped off a narrow walkway That surrounds the inside of house into the hydraulic hose assemble area for the boom.
Injured was climbing down the ladder of a 980M, left foot was on the ground, the right foot got caught on the rung, causing the injured to fall backwards into a parked pickup truck. Suffered a shoulder sprain.
Employee was walking on the catwalk and tripped over their feet causing them to fall on the same level striking their right knee causing a contusion.
EE slipped and fell while climbing out of a tub of a sand screw. EE did not report any injury to supervisor until EE was in emergency room after EE had gone home after EE's shift.

The crew was installing mobile crusher. The crew was locking the machine out and to start unfolding catwalks. Half of the catwalk stuck open. When EE had made it up top of the machine, EE started changing screens without closing the catwalk first. When EE moved to the right, EE fell through the open part of the catwalk. Employee received closed fracture of two ribs on right side
Employee was cleaning out the inlet duct in the raw mill. Miner stepped on the incline side of the chute and lost footing on the loose material falling on their left elbow.
Employee slipped and fell on ice on steps at raw mill ID fan.
Employee parked their personal vehicle in the scale house parking lot and slipped on ice and fell backwards as they were exiting their vehicle. Icy conditions contributed to injury.

EE was using marking paint to create a new berm that runs along the high wall. As the employee was marking the area along the high wall, this is when the EE fell from the high wall approximately 14 feet from the top of the high wall to the point where the EE laid at rest at the bottom of the high wall.

Employee was standing inside the crusher shack on the threshold at the door with right foot halfway out of the the shack. There is approx. a 4 inch drop to the catwalk from the threshold. EE's right foot slipped down to the catwalk and caused EE to fall to the catwalk. This action caused EE to fracture the small toe on right foot.
While assembling a mobile crane, employee stumbled and fell forward catching full body weight with left arm causing shoulder injury.
Employee was walking to the maintenance truck to begin running welding leads to the work at the belt hopper. The employee tripped and fell on left hand, fracturing left wrist.
EE was working at the plant on a clog in a discharge pipe. An other EE open the grating to clear out the discharge line. Grating to manhole was not replaced correctly and the other EE step on grating which cause EE to fall and fracture left wrist.
After getting a bucket of stemming EE went to turn and EE's left knee popped.
Miner was walking 208 Belt line, felt a twinge in right knee.
Employee was using a pry bar to remove a screw conveyor, when the screw became loose it made the pry bar slip and caused the employee to fall back into screw housing.
The employee was stemming holes and while walking EE stepped off in a hole and strained left knee.
Employee fell while descending conveyor about 3', resulting in a 1" cut to right elbow and 2 stitches.
Employee slipped on the mud and fell while rolling out a 6in pump hose and strained their right shoulder 4/8/21. Reported it to manager 4/9/21 and refused medical evaluation until 4/28/21 at 1015am. EE was prescribed medication 4/28/21 making this a recordable injury.
Affected employee was assisting in the removal of scaffolding from the Pre-heater tower via pallet jack. Whilst loading the scaffolding on to the elevator, another employee slipped on snow/ice. This sudden movement resulted in the shift of material thus trapping the EE's left hand between the material and elevator wall, smashing the fifth digit (pinky finger) on the left hand.
Miner fell from 330C after checking the fluids when miner lost balance closing the engine hood
01/11/21 at 0730 miners foot slipped off bottom step of CAT 769 truck while entering and EE pulled left shoulder. Was taken to DR as precaution and released to duty with no restrictions pending MRI. MRI results were read 02/03/21 and EE received a cortisone shot and PT order which turned this in to a RECORDABLE.
IE was fueling up haul truck with DEF fluid, as IE went to place the nozzle back IE's foot got wrapped with the hose. Causing IE to stumble when IE stepped forward causing pain in right knee. Took IE to clinic, was fully released. IE was still in pain after a week of first visit so we took IE back to the clinic on 1/18/2021, that's when IE received restrictions.
Employee was fueling equipment. EE was on the last truck at the time of injury. EE was pulling the fuel hose up to the gas tank. EE was walking backwards & sideways. EE tripped on a rock when EE turned to pull more hose from the fuel truck. As EE fell, EE heard a pop & then landed on the ground. EE suffered a broken leg which required surgery.
Employee was in the process of dismounting a front end loader and shutting the cab door. EE lost footing and fell off the ladder to the ground. Employee was released to work the next day with work restrictions.
The initial injury happened in 2019, employee was going into a chute at a belt transfer and as EE was climbing in EE's foot slipped off of the step and EE caught self with EE's arms. EE's shoulder hurt but EE did not want to seek medical attention. EE ended up having surgery on 5-18-20 and returned to work 5-19-20
Employee slipped on a wet conveyor belt while inspecting a splice and their right knee came into contact with the metal chute. Employee received 7 stitches on lacerated area below knee.
OSHA Shop Employee had just completed an on site repair on the Komatsu PC 600 Excavator and was doing EE's final inspection of the area making sure EE had all of EE's parts, tools accounted for and picked up. While walking back to the ladder access point EE stepped off of the 18 inch wide factory installed walking surface and fell 65 inches, breaking EE's femoral head, left hip
Employee was assisting with setting a water pump. After unhooking the chain the employee stepped down from the pump on uneven ground causing an ankle twist. X-Ray showed a fracture.
Our Employee arrived at Boxley to inspect the fire extinguishers. Upon the inspection next to the crusher. EE's foot caught on the edge of the bucket of the machine. EE fell and EE's chin hit one of the steps causing a cut. EE reported to Boxley management and went to obtain medical care, EE obtained 3 stitches and then was released.
Employee was dismounting a skid steer and as EE placed foot on the bucket and went to stand up EE's ankle twisted and EE went to the ground. EE was dismounting correctly and the bucket was not slick. This was not an MSHA reportable injury when it occurred. Employee had surgery 11/20/19 because ankle felt unstable.
Employee was carrying an approx. 50-lb ladder guard on shoulders across an unpaved lot when employee slipped & hurt back. Employee has been placed on restrictions.
Employee slipped and fell on ice in parking lot and strained right shoulder while exiting ee personal vehicle.
The employee stated they stepped forward off a platform with their right foot onto a 4-in drop onto a lower platform, felt their weight shift, and reported pain in their back & hips on January 11. On January 14, OccMed placed the employee on limitations and prescribed 800mg of ibuprofen.

Employee was changing a sight glass on a drive. When on the east side of the platform on the VSK bearing pedestal, EE hooked their left foot on an RTD Cable going to the outboard bearing, catching the wire with a foot and falling forward, landing on the left knee. Employee was prescribed 3 weeks of physical therapy by Ortho Surgeon on 10/2/2018.
As the miner was stepping off of a 980 CAT Loader,ee stepped onto uneven, Snow covered ground causing ee to fall and break left Ankle.
Miner was walking down the rock ramp that had been dug out at the surge pile to reach the feeder. The miner was part of a crew working on the surge pile feeder. It had been raining all morning and the rock was wet. The employee slipped and cut a finger on the rock as they caught themselves.
An employee was walking a 5-ft ledge of loose gravel to check the delays on a shot line when the employee lost footing. The employee fell against right forearm, which jarred shoulder and back. The employee requested medical care and cannot complete regular duties. The employee will be reviewing MRI results with a doctor shortly.
