While exiting the read door on the engineer side of locomotive #1200 to set the hand brake the employee stumbled down the two steps to the cat walk. The employee was carrying a lunch bucket therefore had no way to catch self resulting in a broken right leg and fractured right ankle. The area was dry and had zero moisture accumulation. Area was well lit. Approximately 47F out.

EE backed up to coal pile at Quincy Dock to dump a load of coal. EE raised EE's bed about 2 stages and the tailgate did not release. At that time EE lowered the bed but the dump bed was leaning too much and the truck turned over - truck and trailer both damaged, EE sustained 2 broken ribs and a bruised leg.

An employee was cleaning out a chute on the #1 river belt. While doing so, the employee accidentally turned on the valve of a water hose and the water hose was inadvertently angled toward the employee's face. When the second valve was turned on, the hose sprayed water into the employee's eyes, causing an eye injury.

Employee was mounting locomotive 413 and as EE was ascending the locomotive stairs, the employee felt a pop in the right calf area. EE was evaluated at ER and given time off until follow up with Ortho. Ortho prescribed PT for a calf strain and degenerative conditions of the knee.

Employee finished shift, changed clothes, left shower house and stumped toe on parking lot steps and broke big toe. Employee must follow up with Ortho.

Overland Belt Fire. Belt caught fire, possibly due to a faulty roller, after being shut down for lunch. The belt structure, belt, rollers and electrical equipment will need repaired/replaced.

Employee was attempting to reposition a trough roller which was located inside a skid steer bucket. When the employee pulled on the roller they felt a strain in the left bicep. First Aid given, x-rays (negative). MRI ordered on 6/13/19 which revealed a slight tear in the bicep tendon. Employee referred to Ortho for further evaluation. No lost time, No restrictions currently.

Employee tripped and stumbled over a pallet in the maintenance shop yard and struck the front bucket of a parked backhoe causing the fracture of 3 ribs.

Coming down little grade. EE had truck in higher gear. Got brakes hot and couldn't make turn. Truck over turned.

Employee attempted to align a railroad switch. When employee rotated EE's torso and exerted the appropriate amount of force to move the switch handle to the opposite position, EE's right foot slipped off of the cross tie into the adjacent ballast gravel used to stabilize the switch. This caused the employee's lower body to do the splits, resulting in a strain to the lower back.

Working with employee from the mine removing dog bones from a 988G machine to take to shop for repair. Dog bone fell forward hitting and fracturing employees right arm

Employee identified a broken crusher door pin at the rail loadout. After employee and the electrician removed the broken door pin, the employee held the crusher door in place while the electrician left the crusher platform to get a new door pin for replacement. The door then fell on employee's foot.

Reported on August 5th that an employee had filed a compensable hearing loss claim.

The employee, with the help of co-workers, was attempting to remove a bad roller. After loosening the roller, it tilted and pinched the employee's finger between the roller and the belt structure. This caused a laceration to the tip of the employee's left index finger resulting in 7 stitches.

Employee pulled hard to open a track switch, switch opened easy causing him to pull a muscle in his left rib.

EE aggravated his chronic back issues while mounting hill engine. EE stated that incident did not contribute to his chronic back pain, which is managed by meds. EE went to personal Dr. where he was put off until 7/1/15. EE had personal Dr. complete short term disability papers on 5/27/15, but this was rejected by HR. Case was processed as WC on 5/27/15.

Driver fell off scales 5 Ft onto ground. Bruising to left side of body.

Employee was shoveling coal near the truck scales and twisted his upper body resulting in a back strain. The employee had returned from back surgery with no restrictions the month prior (1/14/15). Employee didn't inform management until 3/1/15. Employee was evaluated on 3/2/15 and missed first shift on 3/3/15. This is a lost time accident.

The injured ee was re-arranging miner cable on the mine floor when he fell backwards onto his back. The ee continued to work; a few minutes later he stated he felt back pain. He was treated on the section and transported to the surface where an ambulance transported him to the local hospital.

In the process of building a brattice; the ee was placing a block when another ee tossed him another block-catching the injured ee's finger between the blocks, resulting in a fracture to the tip of the finger. There was no medical attention administered, only diagnostic and first aid.

The ee was injured while spot bolting one-break outby the #6 heading. The bolt rotated when the ee attempted to place the bolt into the drilled hole. His little finger was caught between the bolt and drill chuck resulting in a laceration and fracture to the finger.

A ROOF FALL occurred on the 001 Section measuring 20' wide X 110' long X 6' high from the #3 intersection through 4L crosscut; one break inby SS 4364 in #3 intersection and one break inby SS 4363 in #4 intersection.

The ee was injured as a result of a man bus derailment. In an attempt to re-rail the man bus a ratchet type jack was placed under one corner of the man bus and two crib blocks were placed under the center to rest the frame. The jack handle suddenly released upward striking the ee on the left side of the face resulting in a temporal fracture.

Employee arose from a sitting position while on break and temporarily blacked out causing him to fall forward out of the conductor's shanty and onto the track ballast rock resulting in lacerations to the forehead and chin requiring 1 staple and 3 stitches.

The roof bolter operators left hand was caught in a pinch point when the offside operator engaged the control to lower the ATRS. This resulted in fractures to the ee's left hand.

The Human Resources Department received notification on 3/17/14 that the ee has been diagnosed with CWP.

The ee was installing a roof bolt and lifted his left hand to hold the plate in position. His left middle finger was caught between the plate and mine roof. The ee received a laceration to the finger requiring several sutures.

The ee was struck in the face with a 4" water line. The empty discharge line was struck by the shuttle car and in the turn hit the ee in the face. The ee was dazed but stated that he was ok and completed his shift. Later the ee went to the hospital and it was determined he had three fractures to his jaw.

The ee was in the process of tramming miner and the machine became fouled. The ee de-energized the miner and was traveling on the right side of the machine when a piece of rock measuring 12" X 11.5" X 5' rolled out and pinned his right leg between the rock and mine floor resulting in two breaks to the tibia.

The EE was unloading a leaf spring at the end of the track. The EE lost his footing and fell into the manbus-pinching his right hand between the spring and the manbus,resulting in injury to two of his fingers.

The #1 conveyor belt chute at the Ceredo Dock plugged off with coal. Employee was using a sledge hammer to hit the side of the chute on the upriver side of the middle level of the transfer structure to knock loose the frozen coal when a piece of metal about 1/4x10x34 inches in size hit his left hip and back area, causing a bruise/contusion.

A ROOF FALL occurred at Break No. 14 on #2 belt. The fall measured approximately 20' wide, X 70' long and 6' high.

A ROOF Fall occurred in the No. 3 to No. 4 crosscut of the 7-1 development section to the left of survey station No. 4091 in a return air course. The roof fall measured 50'L x 20'W x 5'H.

EE was cleaning the shop and was attempting to drag a piece of fabricated pipe across a steel work table. The pipe pivoted catching the ee's left index finger between the pipe and table.

On 6/26/13 Dominion Coal received notification from MSHA that the ee had exercised his option to work in a low dust area.

EE had finished installing roof bolts and was advancing the line curtain to provide ventilation to the face. He raised himself up off his knees and felt a tightness in his left knee. EE was administered first aid and transported to the surface where he was taken to the local hospital.

A utility man was injured when the rubber tired vehicle he was riding was struck by a second vehicle. The ee (passenger) was exiting the mine at the end of his shift when the second vehicle which had been parked in a crosscut suddenly and unexpectedly backed out and into the path of travel.

A sand stone slab fell between #4 and #5 entries approx. 18' x 20' - #4 entry was cribbed at the intersection and had 8 Heintzman jacks set along travel way.

A roof fall was discovered in the #7 crosscut between the #5 & #6 headings. The fall was a continuation of a previous 2006 fall. The fall measured 40 x 16 x 5 to 6 feet in thickness. The area was initially bolted with 4 foot conventional bolts. The area was timbered off to prevent access and six 80 ton jacks were installed around the edges of the fall.

The ee stepped into a muddy spot, while removing his foot from the mud he experienced back pain. The ee finished his shift & worked the remainder of the week before seeking medical attention. This ee has had surgery to his back following a reportable incident in 2008. The doctors office informs SunCoke that "he will likely continue to have problems at work or not" (3/18/2013).

EE had finished a cut and was backing the miner out of the face. EE began to install curtain and was reaching to put up the curtain and he felt a "pop" in his right knee. He was transported to the local hospital.

EE was pulling cable on a roof bolter when he felt a "pop" in his left knee. He continued to work but the knee "popped" again. The ee later reported pain in his knee & went to the local hospital for exam. He was referred to a panel dr. for a follow-up.

The ee was crossing the belt near #3 belt drive when he hurt his hand and arm. The ee received fractures to his left hand and left arm.

A scoop operator received lacerations to his middle and ring fingers on his left hand when he was moving a shuttle car and the shuttle car dropped off of a small ledge in the mine floor. He had positioned his left hand outside the shuttle car compartment and his fingers were smashed between the operators deck and the mine floor.

The Human Resource Department received notification from the U.S. Dept. of Labor that the ee had participated in a medical examination authorized by the Federal Mine & Safety Act and this examination indicated that the ee has a sufficient degree of pneumoconiosis.

He was dislodging ice from 2" discharge pump by hitting the hose on a concrete barrier when the ice was loosen the hose was uncontrollable, the of hose hit him in the face.

The superintendent was given letter of correspondence by employee from the dr. on 1/14/13. The employee has been diagnosed as having CWP.

A continuation of a previously reported roof fall occurred in the #1 entry just inby survey station 173 on the G-Section. The fall area measured 40' in length X 20' in width X 6' in height. The area had been bolted with 48" resin bolts. The fall area has been dangered off to prevent access.

The ee was hauling loose material from a settling pond and stepped from the cab onto the dump bed steps to check the placement of material. The ee was standing on the 3rd step when the step collapsed. The ee was holding on to the top of the bed and did not fall to the ground, but did slip down and made contact with the broken step-resulting in a laceration to the leg.
