Employee was unhooking 12volt battery charger from diesel scoop when battery blew up cutting right pinky finger. Required two stitches.

While setting the feeder, EE was putting a crib block under feeder when one of the blocks crushed out letting the feeder set down on EE's hand.

Mining in adverse geological conditions-low cover. A Roof fall occurred overnight and was discovered at 5:46 am on pre-shift exam. Fall was investigated by section foreman, DMS, and MSHA

Employee was loading structure on 001 section. Scoop pulled forward catching waterline, bowing waterline over which pushed structure over. Structure caught employee's leg causing injury to left knee.

Employee missed step on ladder on way down and injured right calf muscle.

Rock fall brk 5 on #5 travelway.

No one witnessed accident leg was hurting and had some scrapes on face

A rock from the rib hit the pinner operator while rib bolting.

Found roof fall in the no. 1 entry approx. 400 ft from the face. Fall occurred in the intersection. 20 ft wide, 20 ft long, 6 ft thick. This is active non-producing status. No coal being produced at this mine. No one was in the mine when the fall occurred.

Somewhat of a fault area, sandstone roof inby fall and sandstone roof about 1 break outby fall.

Found roof fall in the 5th entry approx. 4000ft from the face. Fall occurred in the intersection 20 wide 20 long 5 ft thick. This is active nonproducing status. No coal is being produce at this mine. No one was in the mine when the fall occurred.

Acting as miner helper, watching cable when section of rib fell off striking victim

Repairman has hand in access hole checking to see if linkage needed repair. While troubleshooting, he had operator steer car. When steered, lever caught arm and injured left wrist. Returned to work next day with minor scrape on left wrist.

Small piece of rock fell striking victim on right cheek causing a puncture wound.

While tramming shuttle car rib rolled off hitting his right arm.

Multiple hill seams in #5 entry along #5 belt in route to 003 inactive section which is 1 of 3 intake air courses.

Riding 4-wheel buggy to surface, a small piece of draw rock fell from roof, 1/4" to 1/2" thick , 6" by 6" wide. The sharp edge hit him above left eye, making a cut that required 4 stitches.

Employee was going thru cross cut 4 Right when a piece of rock fell from the roof striking his leg (left).

Operator was standing beside bolter machine when a piece of draw rock fell. Hit him and knocked him into machine hitting his head on machine.

Bolter operator was drilling a hole to put up bolt and a rock fell hitting him on the right arm.

Was getting a rock out of way when he said he went to throw it he twisted knee.

Low Trac 6540 butted a hose. EE got off to look under to see what. Laid hand on top of machine when a piece of draw rock hit his hand. Broke fingers.

EE was changing places when the hook on the boom of the miner came loose striking him on the left arm. There forth causing a knot to come up.

EE was attempting to get drill steel out of the top when steel fell approx 3 1/2 foot striking the head of the bolter, causing the steel to bounce back and hitting him in the head.

We was pulling cable with a premissible ride up #3 entry when the right tire hit a rock 1 ft thick, 1 ft wide causing ee to hurt his back. Didn't need medical attention right away.

EE was assisting the bolter crew when a piece of draw rock fell and hit him in the back and ankle. The rock 5 foot 3 inches 6 inches thick. EE didn't need medical attention a this time.

Injured was using hydraulic jack to raise component on Joy miner. When trying to release jack, it jumped out striking left hand & chest area, knocking victim to ground.

Three men were lifting cable in order to hang it from the top. After hanging the cable 1.5 hours later EE said that he thought he had hurt his leg. He worked the rest of the shift and went home, called that night and went to the doctor the next morning.

Men were in the process of hanging cable so shuttle car could haul under it. EE's where holding up cable while another ee twisted wire around it in order to hold it up. He then left section and told another ee that he had strained his groin & that he was going home.

Employee was putting oil in CX2 scoop when he stood up striking head on top.

Employee called 3/14/11 to say he wouldn't be in. Called back on 3/15/11 to say he hurt his knee at the mine on 3/12/11, but didn't know how or where. Brought doctor's excuse on 3/17/11 and said he could report back to work 3/21/11. Called back 3/21/11 and said he had a torn ligament.

Employee was going to return airway to check on set of air lock doors. He did not check back in and was found unresponsive. No contributing factors, suspected heart attack.

Hit finger loading belt rails on a scoop.

Was hooking up a dust suppression box at #3 belt head, stated that knife slipped, cut index finger on left hand.

Loading roof steps on bolt machine when a sharp piece of metal cut his right pinky

PRE-Shifting the mine a draw rock fell from the top.

EE was pulling cable from pinner to power box.

EE was lifting a tire out of a scoop bucket, his feet slipped causing the weight of the tire to shift straining his right arm.

Belt man was riding a four wheel buggy up #6 belt line when front wheel fell off. When wheel came off steering wheel kicked around and struck his hand.

EE was beginning to make a belt and power move, he was attaching hangers to side power center to hang the miner cable in once move begins and somehow he got shocked.

Operator was standing behind miner running coal and a piece of roof fell on his left foot.

Bolter sat up to bolt break and a piece of rock 2' long X 1' wide X1-3" thick fell between the canopy hitting operator on his right foot.

Due to loose piece of draw rock falling from mine roof. Rock size approx 30" long 2" thick 1 ft wide.

Section of the roof gave way in an intersection at the first break inside the mine. It was about 5' thick X 18' w X 20' L. This mine is just getting first line of breaks cut through.

EE cut finger while changing motor on shuttle car.

EE was operating mantrip when mantrip struck a set of air lock doors causing the door to protrude into the operator's compartment.
