loading plates on the roof bolter they dropped them on their left hand resulting in a laceration on their ring finger that required 4 stitches and a fracture. Employee is here working outside on light duty.

When getting into the operator's compartment (kitchen) of the roof bolter they twisted and felt a pinch in their back that radiated down their leg. Employee is off and awaiting results from the MRI and doctor visit.

Loading 16ft cable bolts on bolter. The helper threw a cable bolt across and hit the operator in the eye. In result fracture eye socket.

A ROOF FALL ABOVE THE ANCHORAGE OCCURRED IN CROSSCUT 12 BETWEEN ROOM 78 AND 79 THE FALL MEASURE APPROXIMATELY 20 FEET LONG BY 17 FEET WIDE THE MINE WAS MINING TOWARD A RIVER VALLEY WHICH MAY HAVE INFLUENCED THE FALL

While hooking up to the low tail employee was crawling over low tail and got EE's foot caught between the tail and scoop bucket. Resulting in a broken metatarsal bone.

EMPLOYEE WAS ON EE'S KNEES, TURNED AT EE'S WAIST TO PICK UP A RAIL, WHEN EE TURNED BACK AROUND EE FELT A STRAIN IN EE'S KNEE

individual was loading t-3 straps when EE felt a pop in EE's lower back

when getting a rock off the dolly it slipped out of EE's hand and came down on EE's right little finger. Finger was between the rock and the dolly frame resulting in a broken finger.

Was bolting the roof with a 42in roof bolt and plate, while the bolt was in the roof with plate, the bolter went to bend the bolt to push into the roof, when a piece of rock fell and struck the bolters left hand. The bolter removed EE's left glove and found blood with a laceration between thumb and EE's pointer finger, resulting in 4 stitches.

Employee was holding next drill steel while lowering the drill head. Drill head came in contact with next steel forcing the steel into the employee's upper leg causing a puncture type wound.

The haulage system was backing up when the back MBC operator notice there was waterline on the low structure and instructed the operators to pull in by. The injured employee had EE's foot outside of the operator's compartment when EE moved the MBC and crushed part of EE's foot between the MBC and the rib/bottom. This resulted in broken bones in EE's foot and a lost time injury.

ind was stacking cable when a johnson super car was backing up struck the individual on EE's right side which resulted in a right hip fracture and bruised knee

EE was crawling through the section. A piece of rock fell out in between the roof bolts falling onto EE's left arm. EE ended up with a laceration. Which resulted EE got eight stitches.

EMPLOYEE STATES EE WAS PUTTING EE'S SECOND STEEL AND A ROCK FELL HITTING EE'S HAND CAUSING A LACERATION TO EE'S RIGHT MIDDLE FINGER

CRUSHED TIP OF RIGHT HAND RING FINGER WHILE UNLOADING CONCRETE BLOCK FROM THE SCOOP BUCKET, THE INDIVDUAL'S FINGER WAS CRUSHED BETWEEN A BLOCK AND THE SIDE OF THE SCOOP BUCKET AS EE WAS THROWING THE BLOCK OUT

A roof fall above anchorage was discovered on the weekly return examination in the number 2 entry near station spad 304. The fall measured approximately 90' long and 20' wide.

glove got stuck between 2 drill steels and EE accidentally hit the rotation when trying to get it out resulting in 5 stitches and a fracture all on EE's left little finger.

Individual stepped off a main tail onto a crib block, the crib block rolled on its side causing EE's ankle to roll and breaking EE's tibia.

EMPLOYEE WAS SHOVELING AND LIFTING STRUCTURE WHEN THEY FELT A PULL/STRAIN IN LOWER ABDOMINAL AREA. THIS RESULTED IN A HERNIA. SURGURY WAS PREFORMED ON 4/16 RESULTING IN LOSS TIME INCENDENT AT THAT TIME.

Individual developed carpal tunnel and had surgery on 04/15/2026

reaching over the bolter they struck their right forearm on a T3 channel resulting in a laceration. They received 4 stitches and came back to work. no time missed.

The individual was smashing ice in the travelway with an ax, when they felt a strain. Later individual was diagnosed with a hernia and went off work on 4-15-26.

Individual was removing the canopy from the twin boom while performing maintenance when it slid off striking EE's wrist resulting in a fracture.

Individual had a sore knee from repetitive crawling, later was diagnosed with a torn meniscus, became lost time on 03/03/2026.

individual was pick up a piece of low belt structure when EE felt a pop in EE's low back. the individual went off work on 04/06/2026 as a lost time accident

While crawling employee turned and felt a pop in their knee. Employee worked until surgery on 2/17/2026

Employee was walking to plant to start EE's shift and slipped on a patch of ice resulting in a fractured ankle.

the section mechanic was greasing the ripper roller on the tail of the miner and employee was on the opposite side of the roller when the relief breather blew out and struck them in the left side of the nose. resulting in a cut and grease in their tear duct.

While trying to knock drill steels loose employee was using a bolter wrench to hit them when they missed and hit themselves on the eyebrow causing a laceration that needed sutures.

Employee was getting a T3 Channel off of the center of the bolter when the stack of T3s shifted smashing a finger and causing a laceration which required sutures.

While unbolting a ram cylinder, the wrench EE was using to hold the nut pinched EE's right pinky finger causing a laceration resulting in stitches.

Individual was experiencing pain in EE's right shoulder from repetitive use which resulted in surgery on 01/30/2026, making this a lost time accident.

loading resin onto the fletcher twin boom bolter in the #2 entry of SW section the operator of the bolter thought that they were done and started to tram the bolter and ran over the employee's left foot resulting in 2 broken toes.

EMPLOYEE WAS STEPPING OFF OF THE FORKTRUCK. THEY STEPPED OF OF THE LAST STEP ONTO THE GRAVEL AND FELT A SHARP PAIN IN THE BOTTOM OF THEIR HEEL. EMPLOYEE WENT TO THE DOCTOR ON 11/06/2025 AND WAS TOLD TO STAY HOME WITH A BOOT ON.

A roof fall above anchorage was discovered on the preshift examination in the A Mains LK to BK slope's belt entry in the intersection at spad 4795. The fall measures approximately 20' wide 38' long and 28' high.

While cleaning rock off of the miner employee pulled a rock off and the end EE was holding went to the roof pinching the tip of EE's left pointer finger resulting in one suture.

torn meniscus in the right knee from crawling. EE Never missed any work until today 12/4/25 because EE's surgery is today. EE worked on light duty until today. Again NO work missed until now.

Bolting at the face and a piece of draw rock fell hitting EE's right finger.

bolting a cut a small piece of rock was falling and they deflected it with their hand. the rock cut the palm of right hand resulting in 5 stitches.

The scoop guy was pushing a skid in to the belt line. The skid caught a roll of belt pushing it sideways into the employee who was in a crosscut, bruising EE's hip.

A roof fall was discovered on the preshift examination on 9/16/2025 near spad 1740 in the beltline. The area was mine in July 1980 and bolted with 5 fully grouted bolts with straps. The fall was about 5 high and 98 long.

Employee was putting belt structure together when the top roller fell off the stand pinching left ring finger between roller and rail.

While standing up, stooped over, getting off mantrip when EE stepped forward felt a pop in EE's foot. This resulted in a fractured 5th metatarsal bone.

Patient was hanging line curtain when EE's feet got tangled in it causing EE to fall. EE fell into the side of a scoop battery causing a laceration and broken nose

Rolling belt in the old works. Two EE's working together both of them had ahold of the square tubing the other EE pulled on the 4" tubing pulling the other EE's hand into the pack man plate pinching EE's finger between the plate and the tubing. Resulting in two stitches and a broken finger.

Individual was installing 48" roof bolt and felt a pinching type of pain in EE's back. Worked up till 09/10/2025.

A roof fall was discovered on a wire run on Sunday 9/31 near spad 2350 extending to spad 2355 #3-4 crosscut. This area was mined approximately 14 years ago and was in an area where sandstone in the intermediate roof created a local "dome" of stacky shale below it in the #3-4 crosscut.

Building a wall. Employee was pulling a block off the top layer of the cube with right hand. when the block fell smashing left pointer finger. that they had resting on the edge of the scoop bucket.
