Employees were using a winch to pick up an impeller when the chain got hung up on a bolt stud. The chain popped off of where it was hung at the same time as the employee put his hand onto the pipe to brace himself. At that moment the chain hit the pipe where the employee hand was resulting in lacerations to the left index finger.

During the process of replacing a guard on a conveyor, due to the weight and awkwardness of the guard, it slipped through the employee's hands causing lacerations to 2 fingers and a bone break to one finger.

Material was located on the step of the Cat 725 haul truck resulting in the employee slipping and spraining his right ankle.

Emp. was operating a Skid Steer moving material near the belts when the skid steer went forward into a hole. The equip went forward into the hole & then righted itself up with jerks until it stopped & rested correctly on the ground. Everything was fine until the employee got off the piece of equip 20 minutes later when he began to experience pain in his back.

Employee was tugging on a pry bar when he felt a burning in his lower back.

Employee was on the work boat with 2 other employees working on repairing the left swing dredge cable. During the course of the repair, the boat turned and the cable struck the employee in the face and knocked him to the deck of the work boat.

A Contracted electrician was installing a junction box to the wall and in his attempt to remove a cardboard box from underneath the existing junction box he was installing, he hit his right middle finger against the box cover causing a laceration requiring 10 stitches. He was wearing gloves at the time of the incident.

An Orkin Pest Control Technician had sprayed around the office trailer for bugs & was in the parking lot to leave the mine site. He slipped on a 1' x 5" mud section disguised by grass. When he fell he landed and made some noise. He was able to stand & walk lightly. He declined medical care & left. He ended up going to the Dr. X-rays showed a broken left leg. Surgery is scheduled.

While pulling on water hose underneath the wash plant and the hose got caught on support beam; as the employee attempted to pull on the hose to free it he felt a sharp pain and burning sensation to the left elbow.

Employee reported on July 14 that he had sustained an injury at work on July 12, 2011 while using a come a long. He was sent for evaluation and the physician recommended further testing to confirm if his condition was directly related to his work activity. Received confirmation from physician on 07/20/2011 that is directly related to his work activity.

Driver drove to the dump off pile, got out of his truck wearing PPE but no gloves, walked to the rear of the truck, opened the tailgate of the trailer with his hands & tried to get 200 lbs. of sand out of the trailer bed. When driver was attempting to ensure sand was not lodged between the tailgate & the bed, the tailgate latches closed on his right thumb crushing it.

While performing a pre-shift inspection on a 992G Loader, the employee fell from the rear bumper when he attempted to cross to the other side via the rear bumper. He sustained a fracture of the right heel.

Employee was operating a Cat 988G loader when a hydraulic line failed causing hydraulic fluid to go through the front window and out the back window - Striking the Employee in the chest and ribs.

EE called his plant manager and complained of chest pains. The manager called for an ambulance and EE was transported to the emergency room. EE has been diagnosed with arterial blockage. This event is not work related. As of 8/18/2010 the employee has been release by his doctor to return to work.

An employee with 35 years of experience, seven years at this company, was found to have a hearing loss through the annual audiometric testing. The employee was sent to the doctor whom confirmed that the employee was suffering from sensorineural hearing loss caused by a combination of noise exposure and Presbycusis (aging process).

After cleaning out a rail car, employee went to close the latch. In attempting to secure the latch he pushed on it with his hand, the latch came loose and pinned his left ring finger. He received several lacerations which required 12 stitches.

While on a manlift, the employee shifted his weight and he had his middle finger right hand on the outside of the lift. As the lift moved slightly he smashed his finger between a beam and the manlift rail.

Empolyee was performing cleanup in the plant near the secondary crusher when the crusher backed up causing material to overflow. The overflowing material struck the employee on the left side of his back above the belt line.

Employee complained that his lower back was hurting after operating the 980G and the pug mill. He was seen by a physician and medications prescribed and released to return to work.

While working with a torch to cut a steel pipe that was apaproximately 12" from the highwall and anchored at the top. The pipe moved away from the highwall approximately 20" and dislodged rock. Several rocks fell, one striking injury on the head knocking his hard hat off. A second rock fell striking him on the head, causing a laceration.

While walking up the catwalk alongside the conveyor to remove spillage, employee slipped and fell. He sustained injuries to his neck and back.

EE was changing a Wear Plate on the 988H. EE was using Impact Wrench to take nuts off of the bolts. EE stopped to check if the bolt had dropped down and at the same time, he was holding the impact tool with his right hand accidentally pulling the trigger, causing the bolt to turn, cutting through the glove on his left hand and cutting his left index finger.

Employee was instructed to perform housekeeping task at the east plant. While picking up pieces of expanded metal, caught the end of his middle finger of his left hand on a sharp edge of the metal, resulting in a laceration. The employee was not wearing his gloves at the time of the injury. The leather gloves were in his back pocket.

The employee was getting out a prybar out of the service truck and hit his rt thunb on the side of the door causing a tiny fracture. He went to get an x-ray and was determined positive and drainage of blood under nail plus immobilization of the thumb.

The employee was walking the job site area, when he slipped on ice and twisted his ankle.

EE was cutting a wire tie from a bundle of computer wires. The knife blade broke causing EE's hand to strike a piece of metal lacerating the tip of his thumb & nail. The injury was treated with 1st aid & the bleeding stopped. EE returned to work requesting no further medical treatment. He self treated for 5 days but suffered periodic bleeding. On 2/6/09 he was sent to dr.

An employee was stripping a barge of the haul cables and the haul cable was caught on the outside of the timberhead causing tension. The employee went to release and the cable struck the employee in the nose/mouth causing a laceration and hair-line fracture to his nose. The employee was sent to the emergency room and conducted X-rays and an MRI.

Unbolting flange to flex coupling from Dredge to pipeline. Flex coupling when disconnected, swung to side, bruising hand of EE between workbarge "A" frame and flex coupling.

EE was enlarging a hole in metal beam to install stairway with a cutting torch. As EE was removing torch from between stair treads, the torch tip got caught in the cuff of his welding gloves, causing 1st and 2nd degree burns to the wrist.

The employee was clearing rocks jammed in between the grizzly bars on the grizzly bar screen with a sledge hammer and strained lower back. The employee cleared a few rocks and went to bend down to reach for the sledge hammer and felt a "pop" in his lower back. The employee didn't report the injury until 10/27/2008.

The EE working in direct sun light without water or taking frequent breaks adding a joint to the dredge line. EE failed to follow instructions to come off the pond. He went home without properly notifing Supervisor and later that evening went to ER and was diagnosed with heat exhaustion.

EE was assisting in rebuild of wet end of dredge pump. He was attempting to remove two bolts holding pump shell to base using an end wrench. The ee lifted on the wrench with his back, straining lower back. He failed to use proper tool for this procedure and while using the wrong tool the ee lifted with his back and should have lifted with his legs.

*Please see the attached document.* Customer truck left on property overnight was found completely burned the next morning. The truck had suffered mechanical problems.

At approximately 9:30 pm the EE was repairing laces in the #7 belt using a die grinder to cut the laces. The trigger on the die grinder stuck in the on position and wouldn't release. Upon reaching around to unplug the grinder, it dropped down, struck, and lacerated his left knee.

EE and a mechanic where using a hydraulic wrench to tighten the spider bolts on the gyratory crusher. While the EE was repositioning the wrench, a miscomunication accurred with the mechanic. The mechanic energized the wrench, pinching the EE's finger between wrench and crusher structure.

EE informed foreman he felt a twitch in his leg but it did not hurt when it happened. When asked if he wanted to see a dr. the EE said he would wait,stating? it could just be a pulled muscle?. On 6/12/2008 the EE went to see a surgeon and was diagnosed with a hernia. On 6/24/2008 the injury was determined to be work related. To date the EE has not missed any time from work.

At approximately 10:30 AM this morning, the employee was attempting to assist another employee roll a drop chute that was being painted for installation in the new Newton County Plant. A contractor was also assisting in this effort. The chute they were attempting to roll was setting on two 4 x 4 wood posts.

After the mechanic hooked the jumper cables to the batteries on the excavator he told the operator to start the machine. When the operator turned the ignition to start the machine the mechanic noticed a puff of smoke come from a filler cap. Upon noticing the smoke the mechanic turned his face away from the area. The battery then exploded causing loss of hearing in right ear.

Wedge kicked back from crusher and pushed employees arm into handrail. He noticed some swelling in his arm after the incident so an ice pack was used to reduce swelling. The employee said he felt like he was OK. On 10/25/2007 he became concerned about the extent of the bruising on his arm and requested a doctor's appointment. He was taken to the doctor, x-rays were negative.

While cutting a weld with a cutting disc on a 4" grinder the disc caught an edge breaking the disc and caused the grinder to kick back. EE lost grip and the grinder landed on his left forearm causing abrasions and lacerations.

Employee closed van door on right index finger.

Employee burned himself installing Skirt Rubber. First Aid was administered on site and the injury only appeared to be a red spot on the inner arm. The burn became infected and the employee was sent to the doctor on 09/24/2007. He was prescribed antibiotics and returned to work that day with no restrictions.

Employee was pulling on a wrench when he heard something pop in his side. Employee did not think he needed to see the doctor at the time but on Monday 8/10/2007 he said he was still hurting and wanted to see the doctor.

Employee was doing repair work on a Motor Control Center and wrench slipped off connection and struck employee's safety glasses which broke cutting him above his left eye. The cut required stitches.

While loading 50# bags of ANFO onto a shot, felt a sharp pain in lower back

While stringing out an extension cord the employee stepped from the chute deck to the motor deck where he stepped on a rock and twisted his left ankle.

Employee suffered from pre-existing back injuries, degenerative bone/joint condition. A chiropractor examined employee and told him pain was a result of bouncing in the seat while operating truck. Injury is still under investigation but is being reported as a result of doctor's statement received on 07/02/2007. Employee did not report injury until 06/04/2007.

Employee was assisting the Orica blasting crew in getting a shot prepared. He was cutting a blasting tube to hold the shock tube. He was cutting downward and it only cut a couple of inches. He turned the knife around and cut upward on the tube. As it came through the tube the knife stuck in his left wrist.

The maintenance employee burned his eyes due to welding / cutting. The employee states that he was wearing PPE.
