Individual developed carpal tunnel and had surgery on 04/15/2026

While trying to knock drill steels loose employee was using a bolter wrench to hit them when they missed and hit themselves on the eyebrow causing a laceration that needed sutures.
the section mechanic was greasing the ripper roller on the tail of the miner and employee was on the opposite side of the roller when the relief breather blew out and struck them in the left side of the nose. resulting in a cut and grease in their tear duct.

A roof fall above anchorage was discovered on the preshift examination in the A Mains LK to BK slope's belt entry in the intersection at spad 4795. The fall measures approximately 20' wide 38' long and 28' high.
While cleaning rock off of the miner employee pulled a rock off and the end EE was holding went to the roof pinching the tip of EE's left pointer finger resulting in one suture.

When the miner operator activated the cutter heads on the miner, a foreign object struck the individual in the left side of face resulting in a laceration and fracture.
Employee had a rock stuck in the arms of the bolter when EE lowered the head the rock sprung out hitting the employee in the lip and shoulder causing lacerations that required sutures.
While pulling belt employee's channel locks slipped off of the belt causing their elbow to hit the roof resulting in a laceration that required 4 sutures.
Employee was changing fuse on a 60V quick charger. EE unplugged the wrong charger and inserted the fuse without the fuse cap which resulted in EE feeling a shock. Employee claimed later that EE felt a tightness in EE's chest and had abnormal breathing.
using a pry bar to assist the mechanics in changing the coal chain on the miner, the bar slipped and they struck their hand on a piece of metal resulting in a fracture of the right index finger tip. NO WORK MISSED.
