KUHNTOWN PORTAL ELEVATOR OUT OF SERVICE FOR MORE THAN 30 MINUTES (APPROX. 45 MINUTES) WITH 4 PERSONS IN THE ELEVATOR. THE MINE HAD A MAIN POWER FAILURE & ELECTRICK/DIESEL BACKUP GENERATOR FAIL ED TO OPERATE DUE TO DISCHARGE STARTER BATTERIES.
AN EE RAN A 4-WHEEL CART INTO THE ELEVATOR DOOR, KNOCKING IT OUT OF THE GUIDE. THE ELEVATOR WAS OUT OF SERVICE UNTI GUIDE REPLACED BY MILLAR ELEVATOR SERVICE. NO ONE WAS ABOARD, THERE WERE NO INJURIES.
AN UNINTENTIONAL FALL OF ROOF WAS DISCOVERED ON THE 9-LEFT SECTION AT APPROX. 0630 ON 7-19. THE FALL WAS DISCOVERED IN THE 2 ENTRY AT THE INTERSECTION OF SPAD 18425. FALL MEASURES APPROX. 20' LONG X 20' WIDE X 8-10' IN HEIGHT. AREA WAS SUPPORTED WITH 5' TENSION REBAR BOLTS. NO EQUIPMENT CAUGHT--NO INJURIES. FALL WILL BE CLEANED UP.

THE OAKDALE ELEVATOR STOPPED MID-SHAFT. THE MALFUNCTION OCCURRED DUE TO A FAULTY EMERGENCY STOP SWITCH/WIRING. ELEVATOR SERVICE REPLACED SWITCH AND WIRING. THE ELEVATOR WAS RESTORED TO SERVICE AT 9:15 PM 2/23/00. ONE PERSON ABOARD FOR APPROXIMATELY ONE HOUR, 5:03 PM TO 6:03 PM. NO INJURIES.

TECHNICIAN HAD ASCENDED A THERMAL DRYER EXHAUST STACK PLATFORM IN CONJUCTION WITH SAMPLING EMISSION ACTIVITY. WHILE ATTEMPTING TO REMOVE ICE FROM THE PLATFORM WITH HIS FEET, A SECTION OF GRATI NG BROKE LOOSE CAUSING HIM TO FALL TO THE GROUNDRESULTING IN FATAL INJURIES.

EE DIAGNOSED SOME TIME BACK WITH DISC DISEASE & HAS SUFFERED BACK PAINS ON & OFF FOR SVERAL YEARS. EE WAS REMOVING A JERRY TOWN MOTOR & FELT PAIN
AS EMPLOYEE WAS WALKING DOWN A HILL HE SLIPPED &FELL ON SNOW & ICE, LANDING ON HIS BACK, CAUSINGA SPRAIN. HE CONTINUED TO WORK THE REST OF THE WEEK. HOWEVER, ON SATURDAY, 12/09/00, HE SOUGHT MEDICAL TREATMENT & WAS AUTHORIZED OFF WORK FOR ONE WEEK.
AS EE STEPPED ONTO SHIELD #145, THE EE SAID SOMETHING HIT HIM IN THE GROIN AREA. INVESTIGATION IT IS UNKNOWN WHAT STRUCK THE EE. HE WAS TREATED AND RELEASED SAME DAY FOR A LACERATION REQUIRING 4 SUTURES LOWER ABDOMEN.

EE WAS LOADING BELT STRUCTURE ONTO A SUPPLY CAR IN THE SUPPLY YARD. HE CAUGHT HIS FINGER BETWEEN STRUCTURE AND A STANDARD ON THE CAR.
WHILE OPENING A GATE, SHE EXPERIENCED BACK PAIN.
RIDING IN A GATOR HE BOUNCED OUT OF SEAT HITTING HIS HEAD ON THE CANOPY, CAUSING NECK STRAIN.

OPERATING 50 TON LOCOMOTIVE, PULLING THE TRACK CLEANER. HE LOOKED BACK TO CHECK ON THE CLEANER AND STRUCK HIS HEAD ON A PLANK INSTALLED IN THE ROOF.

EE WAS OPERATING THE TAIL SIDE SHEARER DRUM. SOME ROCK FELL INTO THE PANLINE INBY THE SHEARER AND AS IT WENT UNDER THE SHEARER A PIECE WAS KICKED OUT INTO THE JACK LINE STRIKING HIM ON THE LEF T LOWER LEG CAUSING A HAIRLINE FRACTURE TO THE SMALL BONE IN LOWER LEG.

5-NORTH-100 BLOCK POWER CAR. EE WAS RETRIEVING AN EXTRA FUSE FROM THE 100-BLK. POWER CAR TO BE USED IN ANOTHER CAR. HE HAD DE-ENERGIZED THE BOX BUT FAILED TO GROUND THE CABLE. HE INADVERTANTLY CONTACTED AN ENERGIZED COMPONENT. HE WAS SHOCKEDAND KNOCKED TO THE GROUND. INCIDENT RESULTED IN ELECTRICAL BURNS TO BOTH HANDS AND LACERATION OVER RIGHT EYE DUE TO FALL.

WHILE CHANGING THE SLOPE BELT TAIL ROLLER A STEEL BAR WAS BEING USED TO PULL THE SLACK OUT OF THE SLOPE BELT AND ALLOW EXTRA ROOM FOR CHANGING THE TAIL PULLEY. THE LIFTING EYE ATTACHED TO ONE END OF THE STEEL BAR BROKE LOOSE AND ALL ALLOWEDTHE BAR TO SWING AROUND AND STRIKE THE INJURED EMPLOYEE IN THE MOUTH AREA, CAUSING A LIP LACERATION AND KNOCKING OUT TWO TEETH.
EE EXITED A DIESEL GATOR, WHILE STANDING LOST HIS BALANCE AND FELL AGAINST THE RIB. HIS RIGHT HAND CONTACTED THE COAL CAUSING A CUT ON THE HEEL OF HAND REQUIRING 3 SUTURES.
EE WAS WEDGING POST FOR LINE CURTAIN. ROCK FELL FROM TOP & STRUCK LEFT SIDE OF HIS CHIN RESULTING IN A LACERATION REQUIREING SUTURES
HELPING ERECT KENNEDY STOPPING. WHILE JACKING A PANEL TIGHT AGAINST THE ROOF A PIECE OF SLATE FELL FROM THE ROOF AND STRUCK HIS RIGHT WRIST.

EE WAS HELPING ANOTHER EE ROLL OVER A TRACK RAIL BY USING TWO SLATE BARS. WHEN THE OTHER EE'S SLATE BAR SLIPPED CAUSING ALL OF THE WEIGHT TO SHIFT ONTO EE'S BAR WHICH IN TURN STRUCK HIM ON THE LEFT SIDE OF HIS NOSE.
WHILE HANGING A ROOF BOLTER CABLE TO THE ROOF WITH TIE WIRE A SMALL PIECE OF ROCK FELL FROM THE ROOF HITTING THE EMPLOYEE IN THE MOUTH. TWO WEEKS LATER THE FRONT TOOTH WAS LOOSE REQUIRING A DENTAL PROCEDURE.
EE WAS HELPING MOVE 8"" STEEL WATER LINE THROUGH AIR LOCK DOORS. THE 8"" PIPE SLIPPED OFF THE DOOR, CATCHING EMPLOYEE LIFT BAR. THE BAR CAME UP AND STRUCK EE'S LOWER LIP AREA, RESULTING IN A LAC ERATION REQUIRING 6 SUTURES.
ACCIDENT OCCURED AT 88 BLK M.N. HAULAGE. EE WAS STRUCK IN THE LIP & RIGHT SIDE OF CHIN BY GATE HANDLE WHILE ATTEMPTING TO OPEN GATE ON BALLAST CAR TO DUMP GRAVEL ON TRACK.
EE WAS HELPING TO JACK A LID UP WHILE REPLACING A MOTOR ON A CONTINUOUS MINER. THE JACK BAR SLIPPED STRIKING HIM ON THE FOREHEAD REQUIRING 5 STITCHES.
EMPLOYEE WAS ROOF BOLTING A FALL, AS HE REMOVED THE DRILL STEEL FROM THE HOLE A PIECE OF ROCK FELL STRIKING HIM ON THE RIGHT WRIST, CAUSING A LACERATION REQUIRING SUTURES.
EE WAS USING A HAMMER TO BEAT RUST OFF DECKING 101 BELT. AS HE MOVED HIS ARM BACK, HE CUT HIS RT. FOREARM ON THE DECKING. THE LACERATION REQUIRED 8 SUTURES.
EMPLOYEE WAS RETRIEVING A BOLT OUT OF H.G. DRIVE SPROCKET COMPARTMENT; THE EMPLOYEE'S RIGHT LITTLE FINGER WAS PINCHED BY THE COMPARTMENT LID/COVER.

WHILE TURNING TO REACH A NYLON SLING, HIS METATARSAL SAFETY SHOE CAUGHT ON CRIBBING BLOCK AND HE FELL DOWN ON HIS BUTTOCKS. DID NOT COMPLAIN OF ANY PAIN ON 6-16-00. INVESTIGATION COMPLETED 7-2 8-00 BY SAFETY DIRECTOR.
4TH FLOOR OF PREPARATION PLANT. REMOVING CHAIN HOIST FROM BEAM. THE HOIST SLIPPED IN HIS HAND SMASHING FINGER BETWEEN HOIST AND HAND RAIL ON STAIRS.
17-D TRANSFER EMPLOYEE WAS THREADING NUT ONTO 2"" ANCHOR BOLT. HE CUT HIS FINGER ON A BROKEN THREAD WHICH WAS PROTRUDING FROM THE BOLT.
EE WAS IN PLANT SHOP POSITIONING A 4' X 4' PIECE OF STAINLESS STEEL METAL TO BE WELDED TO ANOTHER 4' X 8' SHEET OF METAL. WHILE STEADYING THE 4X4 SHEET WITH HIS FOREARM, HE SLID THE METAL INTO PLACE TO WELD. THE 4X4 WHEET SLIPPED LACERATING HIS RIGHT FOREARM AND REQUIRING 12 STITCHES.
EE HAD JUST LEANED A SCREW JACK AGAINST THE RIB AND AS HE STEPPED AWAY THE JACK FELL TO THE MINE FLOOR AND LANDED ON A STACK OF ROOF STRAPS AND THEN REBOUNDED UP AND STRUCK HIM IN THE UPPER LI P.
EMPLOYEE WAS SERVICING 610 SCOOP AT 13D BATTERY CHARGING STATION. REMOVED LID TO GAIN ACCESS TO FLUID LEVELS, LID SLIPPED PINCHING HIS LEFT LITTLE FINGER BETWEEN LID AND FRAME.
EE WAS ROOF BOLTING - A DRILL STEEL STUCK IN THE DRILL HOLE. AS HE PULLED IT OUT WITH HIS HANDS. HE PINCHED HIS LITTLE FINGER BETWEEN THE DRILL STEEL END AND THE BOLT MACHINE DRILL HEAD RESULT ING IN A LACERATION.

HE DID HAVE AN INCIDENT ON 1-22-00 OF FALLING ONTO HIS KNEES, HOWEVER, THIS INCIDENT DID NOT CAUSE HIS CONDITION, AS HE INDICATED HE HAD THIS PAIN FOR SOME TIME, JUST WORKING IN MINE ENVIRONMENT.

ON 1/15/00 EE CLAIMS THAT HE WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME OF BOTH WRISTS.
