A ROOF FALL WAS DISCOVERED ON THE TRACK HAULAGE AT 143 BLOCK. THE FALL MEASURED 4 1/2 TO 6 FEET HIGH, 18 FEET WIDE AND 20 TO 25 FEET LONG.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY A MINE EXAMINER (MAKING MINE AFTER 2 DAY FAN CHANGE OUT PERIOD) ON 4 NORTH MAINS TRACKWAY BETWEEN BREAK 30 & 31. THE APPROXIMATE DIMENSIONS WERE 12 FT WIDE X 100 FT LONG X 4 - 6 FT HIGH. THE FALL HAS BEEN CLEANED UP. NO INJURIES OR EQUIPMENT DAMAGE INVOLVED.
EVIDENCE INDICATED THAT A FIRE OCCURRED IN THE AREA OF NO. 3 CONVEYOR BELT RESULTING IN THE BELT BURNING IN HALF. THE FIRE SUPPRESSION SYSTEM ACTIVATED TO EXTINGUISH THE FIRE. ONCE FOUND AT 7: 45 P.M., ONLY HOT EMBERS AND SMOLDERING REMAINEED THAT WERE EXTINGUISHED TOTALLY WITHIN 15 MINUTES.
A FALL OF ROOF WAS FOUND WHILE ON INSPECTION WITH MSHA. THE LOCATION IS BETWEEN THE MAIN EAST TRAVELWAY AND MAIN EAST #2 BELT AT 66XCUT ON THE BELTLINE SIDE OF THE STOPPING. H 6 1/2' X 18' W X 15' L. SUPPORTED BY 6' CONVENTIONAL BOLTS.

CONTRARY PORTAL SERVICE CASE-THIS OBSERVED IN ONE OF THE HAUL ROPES. THE SLACK WAS TAKEN OUT OF THE HAUL ROPE AND THE SERVICE CASE WAS PUT BACK IN OPERATION. THERE WERE NO INJURES AS A RESULT OF THIS INCIDENT.
FALL OCCURRED AT THE EDGE OF XC 130 ON THE MAIN EAST TRAVELWAY. IT IS NEAR A 4-WAY INTERSECTION INVOLVING THE NORTH SIDE XCUT OUT 6' INTO TWAY. FALL 33' LONG, 20' WIDE, 7' HIGH. 6' COMBINATION SHELL/RESIN BOLTS USED, 1-CRIB & 2-6X6 TIMBERS.
CONTRARY PORTAL SERVICE CAGE- THIS FACILITY WENT DOWN DUE TO AN ELECTRICAL MALFUNCTION (LEAD OFF MOTOR). THERE WERE NO INJURIED AS A RESULT OF THIS INCIDENT. REPAIRS WERE MADE AND THE SERVICE CAGE WAS PUT BACK INTO OPERATION.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ALONG GRASSY CREEK MAINS PRIMARY ESCAPEWAY IN THE NO.6 ENTRY AT SPAD 15684. THE APPROACHES TO THE FALL AREA WAS DANGERED AND BREAKERED OFF. APPROXIMAT E FALL DIMENSIONS WERE 8'HIGH X 25'LONG X 30'WIDE. OCCURRED IN AN INTERSECTION.
A METHANE IGNITION OCCURRED IN THE FACE OF #4 ENTRY OF THE 4 RIGHT SECTION OFF OF 3-EAST MAINS. THE SMALL FLAME WAS NEAR THE MINE FLOOR. IT WAS EXTINGUISHED BY THE MINER WATER SPRAYS AND BY DI SCHARGING A DRY CHEMICAL FIRE EXT ONTO FLAME. THE DURATION OF THE FLAMES WAS APPROX ONE TO ONE AND A HALF MINUTES.
A SEVERE ELECTRICAL STORM OCCURRED CAUSING LOSS OF ALL POWER TO THE MINE. THE STORM CAUSED AN ELECTRICAL MALFUNCTION IN THE HOIST CONTROL PANEL WHICH WOULD NOT ALLOW THE HOIST TO BE OPERATED ON GENERATOR POWER. 1 EE WAS STUCK ON THE HOIST CAR FOR MORE THAN 30 MINUTES.
AN ALLEGED FACE IGNITION OCCURRED IN THE #1 ENTRY OF THE 4-A SECTION. THE INCIDENT OCCURRED AS THE MINER HEAD PENETRATED THE BOTTOM OF THE COAL SEAM, CUTTING INTO ROCK AND SULFUR DEPOSITS. THE INCIDENT LASTED APPROXIMATELY 1-2 SECONDS AND WAS SELF EXTINGUISHING.
AN IGNITION OCCURRED ON THE 2-A LONGWALL FACE AT THE #120 SHIELD AS MINING WAS PROGRESSING TOWARDS THE HEAD. NO INJURY.
4-A SECTION #3 ENTRY-A FACE IGNITION OCCURRED IN THE #3 ENTRY AS MINER WAS TRIMMING THE BOTTOM.

WHILE TRAVELING TO THE BOTTOM THE ELEVATOR HAD SOME VIBRATION AND SLOWED DOWN. BOTTOM ROLLER GUIDES WERE FOUND OUT OF LINE.
MINE IS EXPERIENCING HOT SPOTS IN THE COAL SILO ADJACENT TO THE PREPARATION PLANT. PREP PLANT ISIDLE. LOAD OUT OF HOT MATERIAL BEGAN IMMEDIATELY& CONTINUED UNTIL ALL MATERIAL WAS REMOVED.

THE LONGWALL TAILGATE INTERSECTED A FALL THAT WAS REPORTED ON 6-13-01. THE LOCATION IS IN 2J 3AC @ 64 XC 77+20 TO 77+35 FOOTAGE MARKED. THIS FALL NOW IMPEDS THE PASSAGE OF MEN OUT THE TAIL GATE AND THE SECONDARY ESCAPEWAY HAS BEEN RE-ROUTED. 6'H X 18'W X 15'L.

EE WAS DIAGNOSED ON 7-19-02 WITH HERNIA THAT HE CLAIMS IS WORK RELATED. ON 7-2-02 HE EXPERIENCED BURNING IN RT SIDE OF GROIN WITH CON CONTRIBUTING CAUSE. EE HAS CONTINUED TO WORK.
EE WAS OPERATING THE DOZER ON THE REFUSE PILE & BACKED OVER A WIND ROW JARRING HIS NECK & BACK. (DID NOT START MISSING WORK UNTIL 1/2/03 DUE TO THIS INCIDENT).
EE WAS WALKING BY 6-NORTH TOOL CAR SLIPPED ON LOOSE COAL. AS HE WAS FALLING HE CAUGHT HIMSELF ONTHE VICE MOUNTED ON THE TOOL CAR.
MINER CAUGHT BETWEEN 2 PIECES OF EQUIPMENT. ONE PIECE INOPERABLE THE OTHER TRAMMING THROUGH CURTAIN INTO AN INTERSECTION.
EE WAS CLOSING THE ELEVATOR DOORS ON THE PORTAL BOTTOM AND INADVERTANTLY CAUGHT HIS RIGHT HAND BETWEEN THE DOORS.
EE WAS RIDING IN A TRACK PERSONNEL VEHICLE WHEN IT RAN INTO A PARKED VEHICLE. THIS CAUSED EE TO HAVE A WHIP-LASH OF HIS NECK, RESULTING IN NECKMUSCLE SPASMS AND STRAIN; AN AGGREVATION OF A PREVIOUS NECK CONDITION FROM JAN 01 INCIDENT.
EMPLOYEE STATED HE SLIPPED & TWISTED HIS KNEE. THIS INJURY HA BEEN UNDER INVESTIGATION AND WAS NOT DETERMINED TO BE REPORTABLE UNTIL 1/6/03 AFTER RECEIVING A REPORT FROM A SECOND MEDICAL OPINI ON.
EMPLOYEE WAS EXITING THE MANTRIP ELEVATOR ON THE SURFACE AT THE END OF HIS SHIFT WHEN HE TRIPPED AND FELL, STRIKING HIS LEFT SHOULDER ON THE CONCRETE FLOOR OF THE BUILDING, DISLOCATING HIS SHO ULDER.

EE AND CO-WORKER WERE CARRYING CRUSHER GUARD AND HIS LT FOOT SLIPPED, CAUSING HIM TO FALL AND GUARD STRUCK HIM ON LT KNEE, PAIN AND SWELLING IN LT KNEE WITH ABRASION.
1-A SUPPLY TRACK - EE WAS STANDING BESIDE A TRACK CARRIER WHILE CO-WORKER WAS LIFTING TAILPIECE OFF OF IT WITH A UNI-TRAC. AS THE TAILPIECE WAS BEING LIFTED A LOOSE BELT ROLLER WHICH WAS ALSO ON THE CARRIER FELL TO THE PAVEMENT AND STRUCK INJURED A GLANCING BLOW TO HIS RIGHT THIGH.
EE WALKING THROUGH LINE CURTAIN CAUGHT RIGHT FOOT IN LINE CURTAIN TWISTED RIGHT KNEE, DIAGNOSIS OF MENISCUS TEAR.
EE AND CO-WORKER WERE CARRYING 4""PIPE FROM THE TRACK ENTRY TO THE BELT ENTRY. INJURED FELL TO THE PAVEMENT, THE JOINT OF THE PIPE THEY WERE CARRYING STRUCK HIM ON THE LT. LEG. HE EXPERIENCED PAIN IN LOW BACK AND LT. LEG.
USING A SLATE BAR TO PRY ON LOADING MACHINE FOOT SHAFT. THE BAR SLIPPED CAUSING HIM TO SMASH HIS FINGER BETWEEN BAR AND OBJECT ON LOADER.
EMPLOYEE WAS ASSISTING IN THE REHABILITATION WORK 4 BLOCKS IN BY UNKNOWN INTAKE AIR SHAFT. EYES BECAME IRRITATED IN SULFUR DUST ASSOCIATED WITH LEACHING PROCESS OF OLDER WORKINGS.
EMPLOYEE REMOUNTED 4 INCH BELT ROLLER FROM MOUNTING BRACKETS, WHILE REMOVING ROLLER FROM UNDER BELT, ROLLER SLIPPED STRIKING EMPLOYEE ON INSIDE OF LEFT ANKLE.
THE INJURED EE WAS INSTALLING A ROOF BOLT ON THE MAIN EAST BELT ENTRY WHEN HE WAS STRUCK BY A PIECE OF FALLING ROCK.
EE WAS OPERATING A 20 TON LOCOMOTIVE IN THE SUPPLY YARD THAT DE-RAILED.
EE STRUCK A COUPLING ON AN AIR COMPRESSOR LINE TO TIGHTEN LUGS WITH HAMMER. THE COUPLING BLEW APART, STRIKING EE ON BACK OF HEAD AND THROWING HIM 6 TO 8' IN THE AIR. EE RECEIVED A LACERATION T O THE BACK OF HIS HEAD, REQUIRING STITCHES.
EE WAS OPERATING THE HEADGATE END OF THE SHEAR APIECE OF ROCK FELL FROM BETWEEN #73/#74 SHIELDS AND STRUCK HIM ON HIS LEFT HAND WHICH WAS RESTING ON THE PANLINE
EE STATED HE SLIPPED OFF OF D-8 DOZER STEP AND INJURED RT. ANKLE.
HE WAS OPERATING A HAND-HELD ""GOPHER"" ROOF DRILL TO INSTALL SUPPLEMENTAL ROOF BOLTS IN THE BELT ENTRY OF 8-R SECTION. AFTER DRILLING HIS HOLE THE DRILL STEEL BECAME STUCK IN THE ROOF THEN FELL OUT UNEXPECTEDLY CATCHING HIS LEFT THUMB BETWEEN THE END OF THE DRILL STEEL AND THE DRILL HEAD.
EE STATED HE WAS WALKING OUT OF PUMP BLDG. AT 31X RIVER MAINS AND STRUCK HIS HEAD AGAINST TOPOF DOOR FRAME.
EMPLOYEE WAS OPERATING TAIL DRUM ON SHEARER, CUTTING FROM TAIL TO HEAD, WHEN #164 SHIELD WAS INADVERTENTLY PULLED IN CATCHING HIS RIGHT FOOT BETWEEN THE SHIELD RELAY BAR AND THE PONTON.

14 BLOCK, D-HAULWAY SUPPLY TRACK-INJURED WAS SUPERVISING THE RE RAILING OF A CARRIER LOADED WITH A PETITO MUTE. HE WAS KNEELING TO SEE IF CARRIER TRUCKS WERE ALIGNED WITH THE TRACK RAILS. HE FELL DOWN ON TOP OF THE CUT DOWN ROCK MATERIAL AND RECEIVED A LACERATION BEHIND THE RIGHT EAR AND LACERATION TO THE LEFT KNEE.
4 NORTH MAINS- BREAK 48. EE HIT THE TOP BACK PORTION OF HIS HARD HAT AGAINST THE MINE ROOF WHILE RIDING IN A SLOW MOVING MANBUS. THIS CAUSED THE FRONT BRIM OF HIS HARD HAT TO BE PUSHED DOWN ON TO HIS SAFETY GLASSES AND THE BRIDGE OF HIS NOSE AND FRACTURED HIS NOSE IN THE BRIDGE AREA.
EE WAS PASSENGER IN A MANTRIP, THE TROLLY POLE WAS DISLODGED FROM THE TROLLY WIRE & STRUCK HIM ACROSS THE BRIDGE OF THE NOSE.

WHILE DRILLING HOLE FOR ROOF BOLT THE DRILL STEELS BECAME PLUGGED. HE RETRIEVED ONE DRILL STEEL OUT OF THE HOLE BUT ONE STEEL REMAINED STUCKING THE HOLE. HE PLACED HIS HAND OVER THE DRILL UNTI TO CHECK THE SUCTION WHEN THE STEEL FELL OUT OF THE HOLE AND STRUCK HIS LEFT RING FINGER.

HE GOT INTO TH E BACK OF THE PICK UP TRUCK BECAUSE OF THE FLYASH ON HIS SAFETY BOOTS AND CLOTHING. INROUTE HE ALLEGEDLY STRUCK HIS LOWER BACK ON THE WHGEELWELL INT HE BACK OF THE TRUCK.
EE WAS ATTEMPTING TO PIN DRAW BAR TO PARTS WAGON SO SCOOP COULD MOVE PARTS WAGON. THE PIN MISSED HOLE ON BOTTOM END, SCOOP MOVED CAUSING PIN TO KICK BACKWARDS. THE EE'S LEFT LITTLE FINGER WAS PINCHED.
EMPLOYEE WAS STRUCK BY PIECE OF WOOD THAT BROKE OFF WHILE ANOTHER EMPLOYEE STRUCK IT.
INJURED WAS INSTALLING A COIL ON THE FEEDER UNDER THE CLEAN COAL SILO. THE 3/4"" OPEN END WRENCH HE WAS USING SLIPPED, CAUSING HIM TO CATCH RT. INDEX FINGER BETWEEN THE WRENCH AND METAL HOUSING THE COIL IS MOUNTED ON.
THE EE WAS HOOKING UP A DIESEL TRACTOR TO A ROCKDUST. THE EE SMASHED HIS LEFT LITTLE FINGER BETWEEN THE DRAW BAR AND THE DRAW BAR HITCH ON THE TRACTOR. 7 STITCHES & FX OF TIP OF FINGER.
LIFTING A METAL PLATE THRU AN ACCESS DOOR UNDER PICKING TABLE SCALPING SCREEN WHEN THE PLATE SLIPPED AND CAUGHT HIS LEFT MIDDLE FINGER BETWEEN PALTE AND DOOR.
EE WAS USING A 4"" HAND GRINDER. THE GRINDER WAS BEING CRADLED IN HIS RT. ARM WHILE HE WAS PUTTING ON HIS GLOVES. DURING THIS PROCESS THE POWER SWITCH WAS ACCIDENTALLY ACTIVATED. REC'D 7 STITCH ES.
EE WAS CUTTING OFF THE BOLTS THAT HOLE THE TROLLEY POLE IN THE POLE BASE. WHEN THE BOLTS CAME OFF THE POLE BASE SPRUNG UP AND STRUCK HIM IN THE FACE.
EE WAS STRUCK ON HEAR HAT BY SLATE THAT FELL OUT FROM BETWEEN #11 & #12 SHIELD, KNOCKING THE HARDHAT OFF AND WAS STRUCK IMMEDIATELY AFTERWARD BY SLATE THAT CAUSED A LACERATION TO SCALP.
3 EAST MAINS--EE HIT LEFT THUMB WITH BALLPEN HAMMER WHILE ATTEMPTING TO REMOVE A SNAP RING OUT OF AN OLD PLANETARY INNER HUB.
UTILIZING A HAMMER & CHISEL--EE WAS REMOVING A TAIL SHAFT FROM CONTINUOUS MINER. SPLINTER OF STEEL WENT INTO LEFT FOREARM. HAD IT SURGICTLY REMOVED ON 3/5/02.
3 EAST MAIN EE WAS REPAIRING A SHUTTLE CAR CONVEYOR CHAIR. HE HIT HIS LEFT THUMB WITH A BALL PEN HAMMER WHILE DRIVING A PIN OUT OF A CONNECTOR LINK.
EE WAS CARRYING BASE LIFT ASSEMBLIES FOR LONGWALL SHIELDS ON SHOULDER AND BEGAN FEELING PAIN ON TOP OF RT. SHOULDER. DID NOT REPORT ACCIDENT UNTIL 2/18/02 BECAUSE HE THOUGHT PAIN WOULD GO AWAY
OPERATING CONTINUOUS MINER, CUTTING OVERCAST. SOMETHING FLEW OFF BITS AND STRUCK HIM ON THE FOREHEAD.
