The Kuhntown elevator went down at 9:22a.m. No EE's on board. The elevator stopped 150' off the shaft bottom. Schindler came to repair it. A blown fuse and control breaker tripped. Elevator was back into service at 11:00a.m.

Kuhntown Elevator went down at 11:50 AM, no persons on board. Doors would not open at the top of the shaft. Door switch and slow down switch were replaced by Shindler. Elevator was back in service at 2:00 PM.

The 15 W. section entry cut through a planned horizontal gas well at 6:17 a.m. The entry #1 at the face well gassed off following the planned cut. The power was removed from the section and all agencies were notified. Note that this form is being filled out for reporting purposes only. We do not agree an inundation occurred.

A Roof fall occurred in the 4 West cut thru entry to 6 - M between A8028 & 8027 cross cut. The fall did not obstruct ventilation - this is an old outby area. The fall area was dangered off. The fall will be posted to establish a walkway.

During a preshift examination by a fireboss he found a roof fall in the 1st North section at #3 entry x-cut #9. The roof fall was approximately 15' x 15' x 6' high and the area had been bolted with 6' resin bolts. The fall was not in an escapeway and did not impede ventilation. Travel was rerouted around the cave area.

Kuhntown elevator broke down at 10:40am. Surface doors would not open. Shindler came to repair elevator. Problem found was contactors and auxilery switch replaced for contactors. Elevator back in service at 6:10 pm.

An unintentional roof fall occurred on the 3 East Mains in the No. 4 heading at corsscut No. 37. The fall measured 20' long X 20' wide X 8' high. There were no injuries, no equipment was caught and the ventilation was not disturbed. This fall will not be cleaned up.

On August 8, 2009 an unintentional roof fall occurred at the 48+50 plus marker of the main east entries track entry. The roof fall measured approximately 50' long x 14' high & 17' wide. The fall did not cause any injuries, no entrapments and ventilation was not disturbed. The area had been supported with conventional roof bolts, crossbeams with lagging and 12' cable bolts.

Kuhn Town Elevator was down, door would not unlock at the top of the shaft. Shindler was called and found the problem. They cleaned contactors to correct problem. Elevator back in service at 2:30 PM. No persons were on the elevator at this time.

15 W #1 Entry 2863 inby 208. A planned horizontal de-gas hole was intersected at 12:30. The horizontal hole had water coming out as per plan. After water ceased we had excess of 5% which we followed the plan to clear the area. (Please note that this 7000-1 form is being filed for reporting compliance purposes only. We do not agree that an inundation occurred.)

Firebosses were pre-shifting the mine for the midnite shift on Sunday night in 5th West entry #2 at x-cut 47-48 when they found a roof fall. It was dangered off and reported to shift foreman and MSHA. The fall was 20' wide by 75' long and approx. 8' high. A double row of timber was set at all accesses to danger the area off.

Roof fall in 23-m section #4 block. spad location 9658. This is a weekly air course and ventilation not change. Fall was dangered off on the outby approach. This is an outby area that does not effect a working section.

The EE's were removing a joint of pipe from a dewatering hole and they were using torches to cut the pipe in two. While cutting the pipe, there were several small pops heard. The EE's did not see any flames.

On 07/08/2009 the firebosses found an unintentional roof fall in the 5th West section at x-cut 46-47 in the #5 entry (return). The roof fall measured 30-40 feet long by 12 feet wide and was 5-6 feet high and 5 foot resin bolts had been installed in this area. Not an escapeway-no ventilation impeded-no injuries.

Kuhntown elevator went down at 11:20 am until 1:24 pm due to a faulty door switch. No persons were on board at this time. Elevator back in service at 1:24 pm.

Kuhntown elevator door would not unlatch to open. The elevator was on the surface with no persons on board shindler came and repaired the door latch and the elevator was released at 11:20 am door switch detective.

Roof fall in main north. 35 block in the #1 entry 5' to 6' high, 14' wide and 20' long. This is an old area to the mine and a walk way between scals approachs have been succrred and dangered off.

A roof fall in old 4 west right return at 18 block, x-cut 8 to 9, above the anchorage bolts. Fall was between spad numbers 8355 & 8256. This fall will not affect ventilation or walkway. Approaches were posted and dangered off.

Kuhntown elevator went down at 9:22 AM with 20 EE's on board. EE's were on the elevator for 68 minutes. Shindler came to repair the elevator. The problem was that a screw came out of #2 blow out coil. Elevator was checked by Schindler and MSHA then released for full use at 1:38 PM.

5-North escape way hoist was out of service at 8:30 AM due to a broken control handle. The hoist truck was brought to site and set-up and tested. At 12:10 the EE returned to work.

Management elected to temporarily discontinue the normal use of the Camp Run elevator to investigate a fault condition with the elevator. Schindler repair service was called to investigate. The fault was eliminated with the replacement of a door switch. The elevator was returned to service at 3:34 AM. No persons were entrapped nor was their any damage to the elevator.

Kuhntown elevator stopped with 18 persons on board at 12:25 am. Elevators brought to the surface at 12:45 am and personnel unloaded. Shindler came to repair elevator. 2 contactors were overheated. Elevator back in service at 2:57am

16-W #3 Entry at 60 block longwall face/stage loader. Roof fall 30' length, 16' width and 8'-10' height. Fall on longwall crusher to the shield tips. Area outby was supported with cribs and posts. Longwall mined through the fall.

Miracle Run Elevator went down 100' from top landing with 25 people on elevator. Governor over speed switch tripped. Employees off elevator 5:05 PM.

Kuhntown elevator stopped when coming to the surface with 8 employee on board. The elevator malfunctioned at 10:02 and was manually brought to the surface and employees were off the elevator at 10;22 am. Door switch was loose. Elevator was checked and released by Schindler.

A non-injury accident, the unintentional stopping of the elevator at Camp Run Portal occurred. The source of the malfunction was found to be a timing relay. The elevator was returned to service at approximately 2:15 PM.

An uncharted gas well was intersected on the longwall face at 152 shield. Mining was haulted and all power removed. 1.3-1.7 methane was detected.

An unintentional roof fall was discovered on 4 North Mains in the #3 Entry at crosscut # 86, survey station # 18231. The fall measured 20 ft long x 20 ft wide x 10 ft high. No one was injured, ventilation was not disturbed, and no equipment was caught. This fall will not be cleaned up.

Was determined by Schindler Elevator Company that the tail cable on the Sugar Run elevator needed to be replaced. The elevator stopped running near the surface landing.

Roof fall discovered on 12-Rt. section #1 entry spad #25230, approximately 20 feet wide by 30 feet long by 10 feet high. Found by MSHA inspector on a regular inspection and a company person.

Fall at 216 block on the # 4 beltline. Fall was above the 5' resin bolts. 962' of overburden in this area. This area was mined in November of 1995.

5 North shaft escape hoist would not operate properly. Production ceased in that area of the mine until the hoist capsule truck was installed on the surface & operational. Mining resumed at 12:12 PM.

WANA supply elevator would not operate due to a bad emergency switch which was replaced. The elevator was back in operation 8:20am 2-2-09. No person was on the elevator at this time.

EE (fan attendant) was entrapped on the Sugar Run elevator for 80 minutes. No damage to elevator, govenor breaker was re-set.

At approximately 11:35 am the belt went off in 12th West. A roof fall was found at 21 x-cut in the belt entry that measured 10'L x 20'W x 4'-4.5'H and had cut the remote line shutting the belt off. The area was dangered off and federal inspector Donnie Durrant was notified on the property.

A roof fall in the 6th West bleeder 100' inby MPL #1 at x-cut 3-4 was found by a weekly examiner and reported to the Safety Supervisor. Shift foreman went to determine if it was reportable and it's size. Reported to be 20'W x 60'L x 6'H. Five foot resin grouted bolts were used in this area. No injuries, not impeding travel, or blocking escapeway.

While back flushing the emulsion system on the 14A stage loader, the employee was struck in the face by a hose.

Over-exertion in lifting objects

On 12/31/09, EE dropped a 2"" pivot line off the end of a barge, the weight of the line pulled his left shoulder, causing pain. EE returned to work the next scheduled shift, but began missing work on 1/7/10, McElroy was informed EE was off work due to a work related injury, via a WC-1 on 1/14/10. Now determined to be a reportable incident.

Employee fell while watering roadways at the 9 South Mains section. He didn't miss any work days until 1-6-10. No diagnosis was reported until 1-20-10.

Felt pain in left shoulder while building a crib. Continued to work until 11/23/09.

Contact with hot objects or substances

EE was observing mechanics trouble shoot for hydraulic leaks on the track mounted cutting machine in the motor barn shop. EE was leaning over the machine when an electric plug arced beside him and caught his coat on fire burning his right arm and both hands.

In lamp waiting area, stood up to say something to another employee, stepped backward, left heel caught bench brace. Right foot in front of left foot, fell sideways landing on right leg, hip and arm.

Struck against stationary object

EE struck his left knee attempting to put a trolley pole on the wire from outside the mantrip.

EE stated he was tramming the center bolter out of the way. While doing this the wheel of the bolter hit a hole causing the bolter to compress him against the rib. EE was taken to a med.fac.and found to have contusions to his left hip and leg. Further evaluation on 10/2/09 it was found he has a left hip fracture and is now being reported.

EE was using the retractable hose assembly to add engine oil to a pan. He pulled on the hose and the hose retracted or tried to retract. The EE reacted & tried to gain a better grip on the hose when he felt a sharp pain in the stomach area/upper abdominal area. EE continued to work his regular job until 10/30/09 when the doctor removed him from work activities.

The employee's left foot was caught by the left side digging arm of the loader as he started to cross the coal pile.

EE's left foot was struck by a shield pontoon resulting in 4 toes being fractured.

EE was directing a scoop into the x-cut to move cement blocks out of the walkway. A longwall roof support can fell striking the EE and landed on his left leg. EE was treated and received multiple bone fractures to the left leg and a broken ankle.

Employee stated that while he was unloading posts off of a Jeep, he had hold of one post and was pulling it off of the Jeep when the end of the post hit the end of another post and his right ring finger was caught between the posts.

Struck by falling object

The injured was operating the offside of the roof bolter on 15rt. when a piece of rock fell from the top and struck him above the ankle of his right foot. Causing a contusion and puncture.

A piece of draw rock fell between roof straps striking EE on the head and shoulder. The EE was treated and released back to work the next shift. EE continued to work until 12/16/09. EE went off work with fluid on the spine, ligament torn from spine at L-1, compression received from the rock fall.

The injured was riding in a man bus that was being used to pull a 400 ft. long steel cable at the mouth of 9 right. the cable got caught on the guide rail of the switch, and the jerking of the man bus caused the injured to come forward and he struck his head and chin against the man bus. The injured has 26 sutures.

Bent down to pick up a small oxygen bottle and he strained his back.

EE was working on 12 right longwall. EE was setting shield #116 half the way in and was walking by it when a rock fell in front of shield #117. The EE had his hand upon the hand rail when the rock struck his hand. The EE broke his right little, ring and middle fingers and also lacerated the palm of his right hand.

EE was bolting when the drill steel became stuck in the roof. The EE was cleaning out the dust from the drill head when the steel that was stuck in the roof fell striking the EE on the left hand. EE had a fractured ring finger knuckle on the top of his hand.

While crossing a locked out belt the employee stated that he over-extended and twisted his left knee.

On 9/15/09, EE was putting cable over a timber head, when he felt a pain in his right shoulder. He returned to work on his next scheduled shift and began missing work on 1/14/10. Now determined to be a reportable incident.

Struck by flying object

EE was operating a petitto mule and was pulling shield for removal from the face line on 11 right. The chain used to pull the shield broke and a piece of the chain flew back and hit the EE on the right cheek. Fracturing three bones in his cheek.

Struck against a moving object

D-6R dozer backing in fill area backed into bed on 40tn Volvo truck waiting to dump. Damage to cab and windows on D-6R Dozer. Collision caused whiplash/concussion to operator.

Fall onto or against objects

EE stated that when walking around the shuttle car in main west section, he stepped on the cable to cross it when the cable rolled causing him to fall. This caused him to fall on his light battery causing pain in his rt. hip. He was diagnosed at the hospital with a fractured hip.

Over-exertion in lifting objects

The injured was carrying a can of fuel with his left hand when he felt pain in his lower back and left leg. He had carried the fuel can approx. 30 feet.

Struck by flying object

EE was beside miner when the miner oper started the bits a roof bolt flew back striking the EE in the safety glasses. The glasses caused a cut on the side of his cheek below his left eye. Injury did not require sutures, after further invest.it was found he missed work due to the incident and it is being reported as such at this time.

EE was drilling with a rib drill on the miner when his right thumb was struck by a bolt in the drill chuck. EE was wearing gloves and protective equipment. EE had a (hairline) fracture and skin tear to the right thumb.

EE was walking to underground work area a rock 56"" long, 9"" wide 2"" thick fell from mine roof striking him on the left elbow.

Struck by rolling or sliding object

Tearing out a mine fan, the top half of the shroud had been unbolted from the lower half with several bolts left in the conndu without nuts. As one piece was being lifted by a 15ton crane ee stepped onto the top of the shroud closest to the blade. The shroud moved ee jumped off and the shroud rolled onto him pinning him under it.

Cutting off bolts at base of roof fall at 48+50 on main line track when a piece of stone rolled off the pile & hit him in lower back causing abrasions & contusion at tailbone area. Injury occurred 8/12/08 & he went to the hospital 8/13/09. He worked 8/14/09 & 8/15/09. After still having pain he went to ER 8/17/08 & dr put him off till 8/24/09. He started missing 8/17/09.

EE was loading bolts on a pillar block, when the wrench slipped off, catching the right ring finger between the wrench and the rib. He received 4 sutures to the right ring finger.

#3 E. #4 belt had just been completed & power was being restored at the belt power ctr when incident occurred. 1 ee was pushing the reset button, & injured attempted to set/latch the breaker. Breaker did not set/latch on this attempt & process was repeated 2 add. times. 3rd attempt there was electrical arc within power ctr w/flames, causing burns forearm.

Contact with hot objects or substances

A large piece of rock fell from the rib striking a joint 8"" x 20' waterline lying on the pavement. He was crossing the waterline at this time to clear himself from the area. The rock caused the pipe to bounce and tripped him causing him to fall. He suffered a broken tibia and fibula to the lower right leg.

EE was helping install a drive chain on #2 jeep. He was pulling on the chain when it broke loose, he caught the left pinky finger between chain and jeep frame. He went to hospital where received sutures to the finger and referred to umpl hospital for finger surgery on the small fracture.

Employee was walking by a ballast car when he caught his right jacket sleeve on the door release wheel, pulling his right arm backward. The event became reportable when he began missing work on 7-27-2009.

Caught in, under or between a moving and a stationary object

Employee was unloading a rail car in the 3B section. When lowering the ramp on the car it slipped out of his hands and pinched his left index finger between to ramp and the stop block. Employee was wearing gloves. The injury resulted in cummutative fracture to his finger.

Felt pain in low back after hanging bolt machine cable. First day missed was 6/24/09.

Employee was trying to take some bend out of 3/4"" conduit and was pulling on it with his right arm. The pipe twisted and pulled his arm causing pain in his right elbow.

Over-exertion in wielding or throwing objects

While throwing slack off of pivot line, employee felt a pain in his lower back.

Caught in, under or between a moving and a stationary object

Employee stated while performing center bolter duties, he was putting the head of a cable bolt into the bolt wrench when the bolter operator lowered the drill boom catching the victim's rt. foot between the drill pot and the mine bottom. This resulted in a fracture of the metatarsal bone just behind the metatarsal protector of his boot.

While positioning a section of roof screen above the miner, the employee stated that the vent tube was dislodged, which fell striking the employee on the top of the hard hat. The employee suffered a neck strain.

Over-exertion in lifting objects

EE was removing cable bolts from on top of the stage loader, when he felt pain in the upper right arm. EE continued to work at regular duties until 7/24/09. EE has a bicep and tear to the upper right arm.

When attempting to throw 2 South track switch, EE was reaching across his body and felt a pain above his right groin. He continued to work & due to it continuing to feel uncomfortable, he was seen by his family doctor on 5/26/09 and was found to have a hernia.

EE was walking along a supply trip, tripped and fell on right shoulder. EE worked until 8/5/09. Was aware of surgery on 8/27/09.

The employee states that while walking along the track, he rolled his left ankle when he stepped on the edge of a track tie.

Struck by falling object

EE stated he was at the river portal putting old rail in pile with other debris to be disposed of later. He was moving a 6' piece of 85# rail out of the track clearance space. He had moved 1 end of the piece out of the way & moved to the other end. As he picked it up he got it about waist high & the wet rail slipped out of his hands & landed on left foot resulting in a fracture.

The area was being rock dusted with the EE being inby. When the EE tried to walk outby he tripped and fell on his left shoulder. EE worked until 7-20-09, surgery will be done for a torn tendon.

The employee states that while reaching to hang the miner cable overhead, he felt pain in the left side of his neck. The accident became reportable when the employee began missing work on 5-6-2009.

While reviewing the blocking process of a de-railed shield carrier, the employee was struck in the right elbow by what is believed to have been a dislodged block of wood. The resultant injury was a closed fracture of the right elbow.

The employee stated that he was placing blocking material under the tip of a shield loaded on a shield carrier, the tip of the shield suddenly and unexpectantly came down on top of his right gloved hand. The resultant injury was a fracture to the tip of the right thumb.

While operating a shuttle car in main west rehab project, the wheel hit a bump forcing the sc into the rib & when this occurred, the canopy standard caught a rib strap causing the strap to swing back hitting her on the hard hat. This caused a contusion to the right side of her head and neck strain, went to a medical facility and was diagnosed with a slight concussion.

The mine safety department became aware of this incident on February 26, 2010. The employee stated that while walking along the slope belt, the employee turned her right foot causing her to slip and fall. The employee complained of pain in her right shoulder and groin area.

EE was changing a bottom roller, he could not get the roller high enough to place it in the cradle. He felt pain in the right shoulder and back of the neck. EE continued to work until 5/22/09. He went off work due to the injury on 4/7/09.

Pulled right bicep while decending the end loader. Had surgery on 4/16/09. First work day missed, 4/15/09.

Employee was exposed to coal dust in his eyes. The employee had a cold in his eyes prior to working.

Employee stated that he was lifting a box of resin off of the bottom in main west section and felt pain in his back. After the results of an MRI that was conducted on March 14, 2009 and a diagnosis was determined on March 18, 2009 this accident is being reported as a lost time injury.

EE was standing at the back of the miner on the miner cable with the right hand resting on the roof straps. The operator swung the boom catching the right pinky and ring fingers. He received sutures to the pinky and fracture to the 1st digit of the ring finger.

EE felt pain in his back after falling while carrying 3"" lay-flat hose.

EE landed on right elbow from tripping on loader cable. Began missing work on 10/29/09, will have surgery on 11/2/09.

EE was riding in mobile facing outby when the mobile's brakes locked up and slid 70' into a battery motor. EE had the air knocked out of him and back of the head was burning sensation. His shoulders were in pain. EE was treated and returned to work on 2-16-09. EE went off work for injury on 2-24-09 with shoulder pain.

Roof rock fell and hit employee on the head and shoulder while operating a motor. First day of work missed was 2/12/09.

EE was bolter helper with the track bolter. Several pieces of rock fell, striking ee on the right side of head, shoulder and back area.

EE was setting a 6"" beam on a new overcast wall, the beam caught a high spot in the wall causing the beam to roll over striking the EE's right hand and fingers. The EE's right hand(ring finger) received a severe cut from the palm to 2nd joint of finger. Sutures were required to repair cut.

Employee stated that on 1/15/09 he picked up a bottom belt roller & carried it approx. 20' & threw it against the rib. At this time he felt a ""pop"" in his left shoulder. This incedent was under investigation & upon the diagnosis from an MRI on 1/27/09 it was determined that this employee has a torn rotator cuff & is being reported at this time as a result of the diagnosis

The employee stated that he felt a pain in his back while attempting to set a dozer blade wing into position to weld.

EE was pulling monorail off the belt line across belt structure when he caught the left pinky finger between monorail and structure (bedrail). EE received 7 sutures & a fracture to the 1st joint of finger.

EE reported a strain to the groin area while loading conveyor belt. He did not miss any work until 3/9/09 when he had hernia surgery.

EE was working on the 15-W Longwall set-up face on 12/29/10. While removing a roll of high pressure hose off the duck bill of the scoop, another roll of hose rolled off the scoop duck bill, striking EE on the left knee. EE continued to work with no restrictions until 3/12/10, when he went off work for surgery to the left knee.

This is being submitted to abate citation # 8048325 - during a part 50 audit. The employee suspects that while hanging mine cables, he may have over extended himself. He indicated that he felt pain in his right hip radiating down his leg. He sustained a lumbar strain and was treated with chiropractic care.

The ee was roof bolting. He was installing bolts in 5th row of a 23'cut. While drilling hole for a bolt, drill steel became stopped up with dust, depth of 8 to 10"". When the pressure was released rock measuring 9"" long x 5""wide x 3/4"" thick slid down the steel which spun the rock to the side striking the ee in lower lip requiring 14 stitches.

The injured was roof bolting on the 13 right longwall developement section. He was bolting in the 2nd cut out of the left crosscut. As he let the drill head down he grasped the wrench with his right hand. A rock measuring 23"" x 24"" x 0-2"" thick with sharp edges fell approx. 3' striking his right thumb between the web & 1st knuckle causing a laceration requiring 5 stitches.

EE was repairing sump pump at #8 drive of the machine belts. EE had loosened the hose clamps that were holding the discharge line onto the pump,ee turned to grab another tool when the discharge line blew off hitting him in the left side of his face. This resulted in a laceration to the inside of his left cheek and a small cut to the left corner of his lip resulting in sutures.

EE was crawling across a conveyor belt and cut his left forearm. He was transported to Fairmont General Hospital in Fairmont, WV where he received 8 stitches. He was released back to duty with no restrictions.
