A roof fall was found at x-cut 62, #4 entry, in 4th East. This is an outby area of the mine and is not an escapeway or travelway. The roof fall is not impeding ventilation. It has been dangered off and additional support set on both sides of the fall. The fall is approx. 15' wide x 40' long and 6' high. The area was supported with 5' fully grouted resin bolts.

The Kuhntown Elevator went down while an employee was doing his safety checks. Three rectifiers were found to be bad and were replaced. Also a DC connection was loose and tightened. The elevator was back in service 12-22-07 at 1:40 p.m. No injuries occurred.

An unintentional roof fall occurred approx one crosscut inby SS# 3779 in the #4 entry of the Mains section. The fall dimensions were approx 20'W x 45'L x 6'H.
Hoist went into overspeed conditions due to software fault. MSHA notified via Immediately Reportable Hot Line.
A methane ignition occurred on Mains section in the face of #3 entry while mining with Joy 14cm15 continuous miner approximately 136' inby SS #4101.
The wana cable went down shortly after midnight. Nobody was trapped or hurt. It was found. The overtravel switch was dirty. Ones cleaned the cabb was fully opertional
Methane ignition occurred in the face of #3 entry of 6-Left section while mining with Joy 14cm15 continuous miner approximately 30' inby SS #4084.
10/03/07 Shift foreman reported @ 11:22pm Whittaker elevator would not run. Employee was entrapped on elevator. At 11:52 pm State & Federal agencies were called to report situation. At 11:58 pm EE arrived on surface and agencies were recalled. No damage to elevator & no injuries to employee. Door switch was problem.
Elevator at Miracle Run stopped as it was ascending to the surface with 4 people. Total entrapment time was 35 minutes.
Door stop switch replaced and cleaned contactors.
5 men trapped for 18 minutes as elevator was ascending. The governor switch on the miracle run elevator was replaced.
The Camp Run Portal elevator was inoperative from 8:25 PM until 9:25 PM when the govenor rope tripped a pit switch. No damage was done to the elevator. The governor rope pulley was relocated to alleviate the nuisance tripping.

Notified by an MSHA employee an unintentional roof fall had occurred approximately 100' inby SS# 3817 in the #1 entry of the abandoned and not active workings 4-Left panel. The fall dimensions were approximately 5' high by 20' wide. Length could not be determined due to area not being accessible.
Outer door of elevator, door wire rope came off pulley at surface landing. Rope re-attached and adjusted. Elevator running at 3:00 a.m.
Elevator stopped 5' from surface landing for approximately 5 minutes, then continued to landing with no adjusting or attention made to the elevator.
Roof fall above anchorage on 6 North 45 block between the stopping and walkway on tight side of the belt.

The micro switch for the elevator braking system was repaired on the sugar run elevator and elevator brake adjusted

Methane inundation occurred at SS#3906 in #2 entry of 5-Left.
A hoist malfunction occurred which allowed the empty muck bucket to fall to the bottom of the shaft.
A roof fall above anchorage was detected in 5 West at Block between. Date of Developement - May '05 Cover - 1049' 8' conventional resin bolts 10' center bolts
The day shift electrician was making electrical checks on the wana cage. While the cage was on the shaft bottom, the over travel switch knocked power out. The cage was able to run to get the electrician off at 10:30 am. The brake fully set relay contact failed and was replaced.

Electrical tips were found to be dirty, tips were cleaned and elevator operated properly. Total time of entrapment was 30 minutes on Miricle Run Elevator.
A non-injury accident, the unintentional stoppage of the elevator at Camp Run Portal, entrapping 11 persons on-board the elevator occurred. The source of the malfunction was due to the unintentional activation of the car top hatch switch.
Two seismic events occurred in previously mined areas of the active Longwall District which created pressure waves that disrupted ventilation. The event is still under investigation by the mine and agencies.
An unintentional roof fall occurred at SS# 3460. The fall dimensions were approximately 30' L X 60' W X 8' high.

A roof fall occurred in the 6th West #2 entry at #8 x-cut in an adjacent intake entry that is not a manway, travelway or escapeway. The fall was measured at 25' by 15' by 8.5' high. There were no injuries or equipment damage.
A methane ignition occured in the face of the #2 entry on the 0-Panel section (00-7 MMU), approx. 60 ft inby survey station #25117. The ignition occured while the continuous miner was cutting sandstone bottom. The ignition occured int he lower right hand corner of the face. Flame duration was approx. 30 seconds and was extinguished with a wash down hose. There were no
The elevator stopped after traveling from surface for a distance of 8-10'. 17 people were aboard. EE's were removed at 6:10pm.
The elevator stopped after traveling a few feet with people aboard. The elevator repair was at the surface & immediately brought the elevator to the top landing.Entrapment lasted 5 mins.
The Oakdale portal elevator was inoperative from approximately 5:45 PM until approximately 8:00 PM. No entrapments - No injuries. Schindler's Elevator Service was called to the site and repaired a door closed. The elevator was returned to service at approximately 8:10 PM.
An unintentional roof fall occurred approximately 100 feet outby SS# 3839 on 4-Left panel. The fall dimensions were approximately 6' H X 120' L X 20' wide.
In 14th West Pillar section in the #5 entry a roof fall occurred at approximately 12:45 pm. It fell 18' wide x 31' long x 6' high. 5' resin bolts were installed during development. The cave buried two Mobile Roof Supports. There were no injuries. Two MSHA inspectors had been in the section observing mining prior to the fall.
A parked company vehicle caught fire behind the Oakdale air shaft. The smoke from the burning vehicle was pulled down the shaft to the Oakdale bottom area. No injuries. Fire was extinguished on surface within 20 minutes.
The elevator for the Oakdale shaft was taken out of service by mine management to make mechanical repairs. No entrapment. No injuries.
Wana service cage located at wana lost power with nobody on the cage. The cage had just been lowered onto the chairs when the kellum grip broke free causing the trailing cable to pull loose from it's connection. The trailing cable was replaced, new kellum grips installed and power restored.
On the 084-0 MMU, 11A longwall face near the 153 shield, an alledged ignition occurred near the mine roof at the tail shearer drum. The alledged ignition was self extinguishing. No injuries to persons or property. The WVOMHST and MSHA agencies were immediately notified.
A methane gas inundation occurred on the active pillar line of the 4-left section due to a pillar fall inby the pillar line in the active gob.
An unintentional roof fall occurred on top of a Joy 14-15 continuous miner in the face of the #7 entry of 4-Left approximately 30' inby SS# 3813. The fall dimensions were approximately 20' L x 20' W x 6' high.
The Oakdale portal elevator was inoperable from 7:52 AM until 9:35 AM. No entrapment - No injuries. Schindler's Elevator Service was called to the site and repaired a door closer slide. The elevator was returned to service at 9:40 AM.
The Miracle Run elevator doors failed to open while the elevator was at the surface landing. The Shindler repairman installed a new motor and relay switch. The elevator was returned to service at 12:30 pm.
The Oakdale portal elevator was inoperative from 8:55 PM until 11:10 PM on 3/31/2007. No injuries and no entrapements. Repairs were made by Schindler Elevator Service. An oil type overload device had leaked and had to be refilled.
The miracle run elevator doors failed to open when the elevator returned to the surface landing. The shindler repairman determined that a door guide wheel pin had broken. The repairs were made and elevator returned to service at 1:55 am.
The page portal service shaft elevator was taken out of service at 10:05 am on 3/21/07 in order to change the main hydraulic pump. The pump was changed and the elevator was put back in service after test runs at 11:15am om 3/21/07. There were no people on theelevator and no injuries.

The Oakdale Portal Elevator was shut down and inoperable from 7:50 am to 10:30 am. The intentional shut down was to perform repair work. No entrapment, no injuries. Schindlers elevator service repaired a door closer cable. Elevator was returned to service at 10:30 am.
A roof fall was discovered on 4 north mains in #2 heading at brk. 89 to 91, inby spad #18288. This fall does not impeade ventilation, no injuries, and no equipment was involved. The fall was timbered and dangered off at all approaches nad it will not be cleaned up.
The page portal service shaft elevator went down at approx. 6:05 pm on 3/15/07 due to a blown 0-ring ont he brake release valve. There were no people ont he elevator at time of incident. The o-ring was replaced and the elevator was tested and put back in service at 7:05 pm.
The Miracle Run elevator doors would not open. The Schindler repairman determined that a steel cable that opens & closes the automatic doors was broken. The cable was replaced & the elevator was returned to service @ 1:30 pm.
An untintntional roof fall was discvered at approx. 6:30 am at brk 67 on 4 north mains in #2 heading. There was no injuries, no equipment involved and ventilation was not impaired. This fall will not be cleaned up. The fall area was timbered and dangered off at all entrances.
Elevator on surface with doors in opened position. Doors would not close. Bad door closure motor. Elevator operational @ 9:26pm.
Elevator on surface with doors opened. Could not get doors to close. Found door closure motor bad. Replaced motor; elevator operable again 3:05 pm.
The Miracle Run elevator would not return to surface landing from Miracle Run bottom. The Schindler repairman determined that the brushes and bearings needed to be replaced in the hoist motor.
A roof fall was detected above the anchorage point on 22-M longwall section. The fall was in the belt entry in front of #1 & #2 shield, along the solid side. 7'L x 10'W x 12'H.
An unintentional roof fall was discovered on Grassy Creek Mains, # 7 entry Brk. 81 at S.S. # 14680. The fall measured approximately 20 ft wide X 64 ft long and 10 ft high. This fall will not be cleaned up. No equipment was involved, ventilation was not impaired and there were no injuries.
The Miracle Run automatic doors would not open. It was determined by the Shindler Elevator repairman that the door opening motor was bad. The motor was changed out with the elevator returning to operation @ 12:30pm on 2/5/07.
At 5:56 pm the Miracle Run elevator stopped in the shaft @ approx. 50' down from the surface landing. The elevator had 20 day shift employees on board. Elevator was restored to operation at 7:15 and the employees were removed. It was determined that a loose fuse clip caused the interruption in power.
Alleged face ignition occurred: By statements received from the headgate shear operator, tailgate shear operator and shieldman; saw a small orange ball of flame lasting less than 4 seconds and self-extinguishing. No injuries. Sulfur in roof and face.
An ignition occurred on the longwall face on the 8 right section at shield #85. The ignition occurred while an employee was cutting the face chain in the panline. The longwall was in the process of moving and the section was in a non-producing status. The flame lasted 2 to 5 seconds and was extinguished. No injuries.
An unintentional roof fall occurred on top of an Eimco 2810 continuous miner in the unbolted face of #7 entry approximately 35' inby SS3 3640 on 3-Left section. The roof material dimensions were approximately 20'L x 20'W x 3'H.
Located on the 11-A longwall section, an alleged ignition occurred while afternoon shift was waiting to be relieved by midnight. A small orange ball of flame was witnessed by the tailgate shearer operator - lasted less than 1 second, self-extinguished. No injuries (sulphur in the roof).
6 North emergency escape hoist was inoperable from 11:30 am until 12:27 pm. Repairs were made to correct a stalling problem with the motor. No damage, no injuries.
The Oakdale portal elevator was inoperable from approx. 10:30 pm on 1-6-07 until 6:30 am on 1-7-07. Repairs were made to the doors: latches & hinges that were damaged in an apparent collision from the battery-powered supply cart. No injuries. Repairs made by Schindler Elevator Service.
While inspecting the elevator on midnight shift, the power to the elevator tripped. The fan attendant conducting the inspection was entrapped for appox 40 minutes. Power breaker was reset and attendant returned to the surface. No injuries or damage.
While changing a pump the employee positioned himself in the pump hole to position the pump. As he stepped out of the pump hole, he felt discomfort in his left knee. The employee has a history of knee problems. The incident became reportable on 2-22-2008 when medical evidence revealed that surgery was required.
EE was performing miner bolter duties at the face of #1 entry in 2 south west section. He was bolting on the side opposite the operator when he noticed that the roof strap had fallen off of the plank rack. He walked toward the strap to put it back on the rack. It caught on something & sprung back hitting him in the left forehead. Miner was tramming forward. Received 7 stitches.
The resultant injury was diagnosed as a rib contusion and strain of the intercostal muscles caused by the employee resting against a slate bar. Employee stated he felt pain in the abdomen.
EE was riding in the jump seat of #390 portal bus enroute to 2 South section when a piece of roof material 14' x 3' x 4"" fell approx 8' from between roof bolts striking him in the right forearm, right knee, left wrist & the chin. Resulted in a hairline fracture to his right wrist. Occurred at survey station 300+50 on the mainline. No roof bolts were dislodged.
Prying track rails with a bar onto a track tie felt pain in right shoulder. Loveridge was given a diagnosis of injury on 12/19/07.
EE mashed left hand while unloading roof straps from a plat in 9s. mains
Injured was going in mines on D.C. Mantrip. He was sitting on wire side. Trolley pole caught on something on mine roof, pulling pole out of base. Pole fell, hitting EE on right shoulder, causing bruise.
EE was walking down 20-m set up face. He stepped in a water hole and twisted his right knee. EE saw a doct that day and was released to return to work. Later a MRI showed a torn meniscus. EE worked from time of accident until 1/3/08. On 1/3/08 he had knee surgery.
Re-railing the FCT unit when the come-along came unhooked and struck him in the shin.
EE was hanging line curtain and tripped/fell fracturing left fibula.
Smoke inhalation from toxic fumes.
The employee states that he got caught between a 50-ton motor and a miner (equipment move) when he tryed to hook-up a chain.
EE was in process of tramming miner out of #2 entry on 6-Left when a bucket started to fall off miner. He stopped miner and reached to grab bucket and inadvertenly contacted tram controls on remote box, causing the miner to move partially, catching his right hip between the cable stand off on miner and coal rib.
EE was hoisting a set of mantrip trucks that became lodged. In an attempt to free them, they unexpectedly teetered and struck his left foot causing compound fractures.
54 inch roller slid from a sled hitting the EE on the left hand, requiring sutures to two fingers.
The employee states that while removing a face conveyor motor, a motor covering unexpectantly slid forward when he moved the shield relay bar. The employee's lower right leg was caught between the motor covering and the relay bar connection point of the headgate assembly.
Bent drill steel while inserting into bolter chuck and pinched his finger between the drill steel and ATRS mast.
Employee was in the process of turning on water hose at #6 belt drive and as he turned valve on from the hard line going to the hose he was using it jerked his right arm. Employee was diagnosed with a torn bicep tendon and repaired on 10/11/07.
Laceration on left arm caused by contact of sharp end of a metal strap.
Pry bar slipped while positioning a belt drive motor, hitting the EE on the right cheek.
Employee was attempting to move a rock off of the #6 belt tailpiece when he felt a strain in his groin area.
This report is being filed to abate Citation 7101717. An ee suffered a sprained left ankle when he was struck by a rib roll. He went to 2 medical facilities with 2 conflicting reports. The operator chose to send the ee to an Orthopedic Specialist who could not see the ee for several days. The diagnosis was consistent with the 1st diagnosis of a left ankle sprain.
Ran mantrip through air-lock door installation.
Employee was attempting to move a wet brake, when he felt pain in left side of his neck. Hospital diagnosis with possible shingles. Later diagnosis with muscle stain, possible tear, on July 30th.
EE was carrying belt clamp on left shoulder with his arm extended and hand on one end. He twisted his torso, and in doing so, the end of the clamp came into contact with the tripper car. The tripper car forced the clamp into the rib, pinching his left middle finger.
He lost his balance while rockdusting and fell against the rib, cutting his forearm.
Employee was in process of positioning a distribution transformer onto a concrete pad using a boom truck. The transformer was approximately 1-2"" above concrete pad when the frame mounted clack bolts for the Manitex boom sheared and the individual jumped from the operator's deck from a height of 42"" landing on the ground, twisting his left ankle fracturing it.
Employee was walking to a survey control station when he slipped and fell on a concrete flume.
Employee was operating a motor and while dismounting vehicle. She placed her foot on motor step and twisted around to step down. When she did she twisted and felt pain in her right ankle. She could not get in to see an orthopedist until 7/20/07 and was dr with a fracture of the fibula.
While hanging a cable employee felt pain in right shoulder and arm.
Employee was working on pump discharge line, fixing a leak. He was pulling tape off pipe when a hose clamp band came through his glove and cut the palm of his left hand below his thumb. It took six sutures to close cut.
EE was carrying parts & bucket to put in 30 mobile.He lost his balance & fell into the corner of the mobile striking his left lower leg. Causing skin to tear and a non-displaced fracture to the neck of the fibula.
Stepped out of a mantrip and fell, injuring left knee.
Employee was standing along side the miner when his left knee was struck by a piece of coal from the rib.
The injured was pulling rails on the 9 Right section. A section of rails had separated & a scoop was used to pull the separated rails back together. Once together, the scoop moved into the crosscut & crossed the loose rail causing the rail to flip, striking the injured in the lower left leg resulting in a fractured fibia.
Employee got his right foot caught between a ram jack relay bar clevis and the lower portion of the cable trough of the panline when he stumbled as he was attempting to remove a horizontal pin. The employee suffered a fracture to his lower right leg above the ankle.
Employee was landing starting ring and was struck in the knee.
Hanging vent tubing and felt pain in left shoulder. No diagnosis as of 6/9/07.
After further investigation, a diagnosis was finally revealed that confirmed that employee's time off work was due to a muscle strain of his left shoulder from jumping down off the top of continuous miner while holding onto a drill steel.
Cutting on steel, the steel fell on his right foot (side) and broke foot.
Unspooling wire rope, rope got away & was unspooling down uncontrollably, employee grabbed it and pulled his back in a twisting motion and sprained/strained muscles.
EE had just finished scaling roof and rib down before attempting to install bolt. When he put the bit to the roof and started to rotate, a piece of rock fell from upper left corner. The rock came over the rib protector and struck him on top of left foot.
Walking around boom on miner, EE stepped on a small pile of coal, twising his right ankle.
On 9 Right Longwall Section, EE was helping to install a crusher guard, when the bottom of the guard came in contact with the crusher shaft & the come-along hook came loose from the guard. The guard fell over, striking him on left side head, neck & shoulder area, resulting in contusions to neck & left shoulder. Management was notified of this injury 06/24/2007.
Employee was taking down ventilation tubing when he fell onto right wrist. Diagnosed with sprained wrist.
EE was pulling shields using infared system. He was bent over looking at #161 shield to see why it didn't pull all the way in. He stood up with his back to #160 shield when it started to pull because he did not pause the system. The shearer op noticed it so he hit the quick-stop. However, the shieldman had rt foot pinned under the pontoon & partially between pontoon & relay bar.
While roof bolting in 2 South, employee was trying to free a stuck drill steel and in doing so, felt a strain to his right groin. After further diagnosis, it was revealed that the employee suffered a hernia.
While loading roof straps onto the miner, a bottom strap became fouled and the upper strap that he was pushing on continued to slide over the fouled strap. The momentum carried him forward, allowing him to slide his right wrist along the sharp edge of the roof strap. Laceration requiring 6 sutures.
Emp standing 5' away from borehole pipe waiting for shotcrete line with air blowing through to shut off. Line broke at top of borehole - falling - one of the joint pipes hit employee in head/face.
Employee was attempting to straighten a pallet of cinder block by using the bucket of scoop and as he backed up the bucket of scoop came off pallet causing the scoop to bounce. His hard hat came off and he struck the left side of forehead on canopy of scoop resulting in a laceration.
EE was using a jack bar while re-railing a moble on & experienced pain in abdomen. He con't to work then took off on 4/9/07 for hernia surgery.
Employee was dragging a piece of wire mesh along side of the miner in 2-South section. As he was dragging it, it caught on something and when it did, he gave a tug on it. When he did this, it caused him to lose his balance and he started to fall. While falling, he put his left hand down to break his fall and as a result fractured his left wrist.
The employee stated that he experienced rt. shoulder pain after handling exhaust tubing. He was diagnosed on 3/22/07 with rotator cuff syndrome.
EE slammed his left hand in the scoop door.
EE was operating trail motor when lead motor and trip lost traction and was struck by trailing motor.
Employee felt pain in lower abdomen/groin when re-railing off-track motor. Diagnosed with inguinal hernia.
EE was crawling over gob @ #1 shield and slipped, injuring his left hip and left knee.
While bolting on the belt line, employee's helper dropped a suspended steel on injured employee's left index fingertip.
The employee tripped and fell while walking along the track entry in 9 so. at #17 block. The employee fractured the humerous bone of rt. arm.
EE was walking down entry, tripped on a gunite ledge and fell. He put out his arms to catch himself, struck flat car with left arm and shoulder.
Employee was climbing out of man door & fell injuring left knee and left wrist.
The employee felt pain in low back and right hip while engaging a line switch on the trolley line.
An unknown mechanical failure occurred in the Clyde frame 10 hoist being used for shaft steel construction that allowed the work platform in the shaft to briefly freefall prior to being stopped by the overspeed protection on the hoist. Employee suffered 2 cracked vertebrae and cracked sternum from impact of sudden stop.
Employee was firebossing for a construction company. He and three other workers were coming out of the shaft in a construction bucket when the bucket suddenly dropped aprox. 10 to 30 feet and then immediately stopped causing his legs to be driven against the bottom of the bucket and broke both his legs below the knee.
An unknown mechanical failure occurred in the Clyde Frame 10 hoist being used for shaft steel construction that allowed the work platform in the shaft to briefly freefall prior to being stopped by the overspeed protection on hoist.
EE had just finished hanging tubes and began bolting when top started to drip. He turned and headed toward tail of miner to get away, when a piece of rock came down and struck him on top of left hand. He suffered a puncture wound on top of the left hand, which required 4 sutures to close.
The injured employee was struck on the Lt. ear by a roof bolt strap laying on top of the miner. A rib rolled out contacting the strap, the end of the strap then pivoted striking Lt. ear. Employee received 10 stitches.
Employee was helping load a section of impact bed on a flat car at 3 East # 3 belt head. One end of the impact bed was on the car and while loading the other end the employee's rt. index finger was caught at a hinge point resulting in a fracture, avulsion which was closed with nylon sutures.
Employee was struck in the face (left side) while installing a buggy cable anchor with the center bolter. While straightening the mast to the upright position, a drill steel became wedged. It released and struck him causing 5 fractures to the left side of his face (orbital bone).
Employee was operating unihauler while pushing disabled scoop on the surface, when the hauler bucket was raised up approximately one and one half feet and dropped back down to the ground causing him to be shaken in deck. He complained of pain in back, neck and right shoulder. He started missing work on 12/27/07 and medical diagnosis not received until 1/8/08.
Employee stated that while pulling shuttle car cable he struck the roof with his hard hat causing irritation to his neck while on the Mains section. Employee was diagnosed with neck spasms and started missing work on 10/23/07.
While mucking, employee was holding the hose when a rock fell and hit his left wrist, causing a puncture/laceration.
Employee was inspecting and replacing a worm gear. The worm gear shifted and caught left hand between gear and drive motor. Left hand was crushed with broken bones and laceration. Employee was taken to hospital for surgery at time of this filing.
Employee was putting gasoline in a water pump and felt a pop in his back.
Employee lifted a belly pan for a 630 mucker and he strained his lower abdomen which caused bleeding from his rectum.
Employee was working on workdeck, when a full muck bucket was raised to the workdeck level. Employee was to remove a peice of drill steel, but left the drill steel in the muck bucket, muck bucket was then raise out of the shaft, the drill steel caught on collar steel and drill steel fell back down shaft, stricking employee.
While preparing to lower the curb ring, they were closing the work deck doors when a board became entangled in the door assembly causing an impact injury to right knee.
Employee was moving materials, sat down for break and felt pain in lower back.

Employee was wearing leather welding gloves that the wheel hit before contacting his forearm. He was grinding on the bottom of the sheet metal with the grinder being held upside down.
EE was walking from his vehicle in the parking lot of Whittaker Portal and as he was crossing a guardrail, his back foot caught the guardrail, causing him to lose his balance and fall onto his face.
Employee was making a belt splice using a template pounding rivets with a 4 lb. hammer when the rubber handle grip came off of the hammer and his right hand continued downward hitting edge of template causing laceration to hand requiring 10 sutures.
Employee was setting a timber (post) when another employee used an axe to tighten the wedge and the axe slipped out of his hand striking employee on the mouth causing a laceration that required sutures (9). The employees gloves were wet and oily.
While firebossing the slope feed belt, the employee removed her gloves to make an entry in her notepad. As she passed through a mandoor in an oversized stopping, she caught her left middle finger between the frame and the door.

Employee cut his left hand with a razor knife
Cutting 4"" X 8' X 1/4"" plate out of chute, plate released, striking EE's hand. Discovered on 7/23/07 that case was beyond 1st aid because stitches were involved.
Employee stated that he was moving a jeep on the surface. He had moved approx. 40' when his trolley pole came off of the trolley wire & when he went to grab for the pole, his gloved hand hit a trolley hanger causing a laceration to his left index finger. The doctor applied 1 suture.
While dropping fan cable from a cable hanger on 6 North Mains, the employee's gloved right middle finger got caught in the hanger hook. As he attempted to pull free, the hook penetrated his glove and finger resulting in a laceration requiring sutures.
Employee was welding and cutting metal and he got flash burn in both eyes.

Located on the slope seciton: employee lost his balance and fell into the miner bolter. His left hand caught in the mast while his right hand landed on the control levers causing his left hand to get caught between components of the bolter assembly.-laceration L ringer finger required 13 sutures medical no lost time.
Hoist dropped 10-20 feet, then stopped suddenly when overspeed caught and / or operator applied brake. Strained knee.
EE was walking in 22-M track heading by the tool car. A piece of head coal fell out between the straps, stiking the EE in the head. He was knocked to his knees and his hat fell off. When he attempted to stand up, more coal and rock fell, hitting him on the exposed head. EE suffered two separate cuts on top of the scalp.
EE took hold of top belt structure on scoop and slid structure back toward himself. The structure came faster than what he thought it would and he pinched his left thumb between the structure and the sled. He suffered cuts on both sides of his thumb, which took 5 sutures to close.
The employee was diagnosed with noise induced hearing loss. 10.27% permanent partial disability was awarded. Loveridge was notified on Sept 20, 2007.
