At approximately 11:20 pm in the 1st South Pillar section, MMU 004-0 the roof fell on top of the continuous miner. There was a combination of top coal and rock that fell on the miner causing it to be trapped. MSHA was notified and investigated. The continuous miner was recovered with minimal damage by 7:00am on 12/30/2005. There were no injuries.

A METHANE IGNITION OCCURRED IN THE PRODUCTION SHAFT DUE TO A SPARK OR SLAG FROM A CUTTING TORCH. THE WORK WITH THE CUTTING TORCH WAS ON THE UPPER DECK OF THE PROD SHAFT HEAD FRAME, APPROX 650' ABOVE WHERE THE FLAME WAS DISCOVERED IN THE SHAFT. THE FLAME WAS APPROX 6"" LONG COMING OUT OF THE SHAFT WALL & WAS EXTINGUISHED BY A FIRE EXTINGUISHER. NO INJURIES OR EQUIP DAMAGE.

An unintentional roof fall was discovered on 3 East Mains, #4 Entry, Brk. 12, 150 feet inby spad #18990. The fall measured approximately 20 ft. wide, 60 ft. long, and 6 ft. high. This fall will not be cleaned up. No equipment was involved, ventilation was not impaired and there were no injuries.
KUHN TOWN ELEVATOR - DAMAGE TO HOISTING FACILITY THAT CAUSED IT TO BE OUT OF SERVICE FOR MORE THAN 30 MINUTES. IT IS EXPECTED THAT THE COUNTER WEIGHT CABLE SHEAVE WHEEL BEARING FAILED, CAUSING THE SHEAVE TO STOP TURNING RESULTING IN CABLE SLACK - RESULTING IN 1 BROKEN 1/2"" CABLE.
Oakdale portal elevator was inoperable from the start of the midnight shift until about 4:00 a.m. The problem was a malfunctioning gate latch. Repairs were made by elevator repair contractor. No entrapment at this time. However, at 4""10 a.m. the elevator malfunctioned again and stopped about 100' down the shaft with 18 employees on board. Entrapped for 30 minutes. No injuries.

ON THE 084-0 MMU, 9A LONGWALL SECTION, AN ALLEGED IGNITION OCCURRED. AN ORANGE-RED COLORED FLAME, ROUND IN SHAPE ON THE HEADGATE END OF THE SHEAR DRUM. FLAME ENVELOPED DRUM WHICH IS 62"" HIGH AND 42"" WIDE. LASTED 3-5 SECONDS. SELF EXTINGUISHING. NO INJURIES TO PERSONS OR DAMAGE TO PROPERTY.
Kuhntown Portal shaft elevator out of service for more than 30 minutes with an employee on it. Out of service due to an electrical problem (transformer) in the head house.
ON OCTOBER 12, 2005 AT APPROXIMATELY 6:30 AM A ROOF FALL WAS DISCOVERED BY THE WEEKLY EXAMINER IN THE APPROACH TO SCABRIGHT RETURN FAN AT SURVEY STATION 0+ 30 OF #3 ENTRY. THE FALL WAS APPROXIMATELY 17' WIDE AND THE HEIGHT AND LENGTH COUNT NOT BE DETERMINED. THE AREA WAS SUPPORTED WITH 8' ROOF BOLTS WITH 2"" WOODEN PLANK AND POSTS. THIS AREA WAS MINED IN JANUARY OF 1972.

AT APPROXIMATELY 10:30 AM ON SEPTEMBER 17, 2005 A ROOF FALL WAS DISCOVERED AT THE 3 + 00 SURVEY STATION OF THE MAINLINE JUST INBY THE BENTWOOD PORTAL DRIFT OPENING. THE FALL MEASURED APPROX. 17' WIDE X 40' LONG X 15' TO 20' HIGH. THERE WERE NO INJURIES REPORTED AS A RESULT OF THE FALL.
The auxiliary mini cage located at the production shaft was removed from service due to damage to the head frame structure, that resulted from a loaded skip going back down the shaft. There were no injuries as result of this incident.
As a skip was being brought to the surface to be dumped, the hoist experienced an underwind fault. At which time the brake system set as it should. The brake system was unable to hold the loaded skip, which went back down the shaft resulting in damage to the head frame structure, structure in the shaft, ropes, and the hoisting building. 5:15am Ee noticed 79 x c along main east track was taking weight. He then knocked the trolley power and called tex rader acting shift foreman and informed him of the condition. rader then sent a man to guard the area from 8 blocks away until rader arrived.He stated that at 6:17 am the intersection at 79 xc 82 + 68 on main east track fell. the fall was approx. 20'lx16;wx13'h.

A roof fall was discivered at 9:30 am. No injuries, did not effect ventilation - #9 belt main north 88 block - fall area was approx 6' high 30' long and 22 to 24' wide. Area originally mined in 1971, supported with 5 foot conventional bolts. Supplemental 8 ft install bolts remained.
un-intentional roof fall-6 north #1 entry, at approx. 29 1/2 block entry. Area initially potted out around a de-gas borehole and on 8-4-05-determination made that it became msha reportable: approx. 30'l x 16'w x and up to 7' high area previously bolted.
A fall of roof was discovered on the main west track entry at #38 block. The fall area measures 4 feet high- 20 feet long and 40 feet wide at the 4-way intersection.
The Miracle Run elevator stopped about 100 feet up from the bottom. The Schindler Elevator Company repairman found a shorted lead in a power control cable.
The page portal service elevator overtraveled due to a bad electrical relay. The elevator was down for 20 hours, from 8:00 PM on 7/21 until 4:00 PM on 7/22/05. The relay was replaced, the elevator was tested and the service cage was put back into service. There were no people on the service elevator at the time of the incident.

The contrary service cage was down from 11:20 pm until 1:00 am to repair the grating that was damaged due to a chain that was being used as a supply car safety chain getting fouled at the surface landing, which bent the grating. There were no injuries due to this incident.

A FIRE OCCURED INVOLVING #1 PAN BEING USED TO HAUL RESURE FROM REFUSE BIN TO THE REFUSE DUMP AREA. THE PAN OPERATOR WAS LOADING THE PAN WITH REFUSE WHEN HE DETECTED FLAMES COMING FROM THE FRONT ENGINE COMPARTMENT
A methane ignition occurred at the face of 5 right crosscut approx. 90 ft to the right of survey station number 2931 on the 001 unit. There were no injuries or property damage as a result of the ignition and the flame was self extinguished.
AT APPROX 1:35 PM THE MIRCLE RUN ELEVATOR LEFT BOTTOM WITH ONE PASSENGER. THE ELEVATOR STOPPED AT ABOUT MID SHAFT. THE ELEVATOR WAS OPERATED TO THE SURFACE AND THE EE EXITED. THE S-O-S SWITCH WAS RE-SET AND ELEVATOR RETURNED TO OPERATION AT 4:05 PM.
THE MIRACLE RUN ELEVATOR STOPPED IN MID SHAFT ON THE WAY TO THE SURFACE. ELEVATOR REPAIRMAN FOUND AN INOPERABLE CONTACTOR IN THE MAIN CONTROL PANEL. ONE PERSON WAS INSIDE OF THE ELEVATOR FOR 20 MINUTES.
Miracle Run elevator stopped in mid shaft on the way to the surface. The Shindler elevator repairman found a damaged steel cable that closes and opens the automatic doors and a bent sheve wheel that rolls up the excess.
The Miracle Run elevator was located at the surface landing. When the automatic doors would not close. A contact lead was replaced at 4:30 pm.
On the 088-0 MMU 6 north mains section, an alleged ignition occurred - an orange flame, round in shape, 18"" to 24"" in size, lasting 2 to 3 seconds. Self-extinguishing was observed. No injuries to persons or property.

AREA BEGAN FALLING OUT AROUND BOLTS ON FEB 2, 2005 BUT NOW BECAME REPORTABLE ON 2-18-05. NO PASSAGE OR VENTILATION AFFECTED.
THE ELEVATOR AT OAKDALE PORTAL WAS INOPERATIVE FROM 6:30AM TO 8:50AM. NO INJURIES. NO ENTRAPMENT, LEAD IN TRAILING CABLE, GROUNDED, RE-ROUTED TO CORRECT
The 001 longwall shearer was preparing to ""cut out"" on the tailside of the face when a bump occured that apparently forced a pocket of methane from the gob onto the face at the shearer resulting in an ignition that caused a mine fire.
An unintentional roof fall occurred int he number 3 entry, 2 breaks inby survey station 1604 in the belt entry. the fall was approx. 60 ft long x 20 ft widex 5 ft high.
Methane ignition occurred in face of #2 left crosscut on Grassy Creek Mains section, 78' left handed of survey station #23273. Ignition occurred as miner was loading the std. shuttle car ont he left side of the face opposite of curtain. flame duration was approx. 3 minutes, and was extinguished with the wash down hose off the miner and fire ext. were no injuries or equip.
THIS ELEVATOR AT OAKDALE PORTAL WHICH WAS INOPERATIVE FROM 11:10AM TO 12:20PM. NO INJURIES, NO ENTRAPMENT. DUE TO ELECTRICAL PROBLEM WITH DOOR SWITCH.
An unintentional roof fall was discovered on 4 North Mains in the #2 entry at survey station #17941 at approx. 10am. The size of the fall was approx. 20'W x 20'L, height is unknown. No equipment was caught, no ventilation was disturbed & there were no injuries as a result of this fall.
The victim was walking in front of the #11 scoop that was traveling in #1(intake) entry of the 14A longwall to deliver posting material to the worksite. The operator of the scoop stated that he saw the victim walking on the side opposite of the operator. He then looked back & did not see him. He exited the cab & walked to the front of scoop & found the vicim injured on the ground.
EMPLOYEE WAS WORKING ON 6 NORTH MAINS SECTION - IN PROCESS OF CUTTING OVERCAST - EMPLOYEE WAS HANDING AN 8 FT BOLT TO ROOF BOLTER OPERATOR - INSTALLING BOLT IN SCREEN - A PIECE OF ROCK FELL FROM ARCH - MEASURED 4 FT LONG X 2 FT WIDE X 3 INCHES TO 0 IN THICKNESS. STRUCK EMPLOYEE'S RIGHT HAND JUST BEHIND KNUCKLES - RESTING ON RIB PROTECTOR - AMPUTATION OF RIGHT RING FINGER.
AT 23-M CONVEYOR BELT TAKE-UP/TENSIONER AREA, EMPLOYEE WAS ASSISTING TO PREPARE TO LOOSEN THE TENSION OF THE MOVABLE TENSIONER CARRIAGE. HE GOT HIS LEFT HAND CAUGHT BETWEEN THE CARRIAGE DOLLY WHEEL & THE RAILING IT RUNS ON.
Injured was using a 4 ft bar to steady an H-beam that was being jacked back into place on an overcast, as pressure was being applied to the track jack that sitting on top of a 4 ft crib (top layer solid) the cribbing and jack creeled out catching his left little finger against the overcast wall, resulting in finger tip being surgically removed.
On the surface at Vent Shaft #8, The injured ee and one other ee was attempting to lift one end of metal grating (36 in. x 20 ft). It slipped catching the injured ee right middle finger tip between the grating and the asphalt. Resulting in the amputation of the tip of his right middle finger.(Yes Amputation Tip per call 5/23/05)
LOCATED ON THE TAILGATE #2 DEVELOPMENT (GATE) SECTION, THE EMPLOYEE WAS ATTEMPTING TO REMOVE A BIT INSERT FROM THE VOEST-ALPINE MINER CUTTER HEAD. AFTER STRIKING THE INSERT WITH A SLEDGE HAMMER, A SLIVER OF METAL FROM THE INSERT WENT THROUGH HIS GLOVE AND BECAME IMBEDDED IN HIS RIGHT MIDDLE FINGER. BEGAN MISSING WORK ON 12/31/05.
Employee stated that he was trailing empties into the mine on #104 motor when he hit his head on the top at approximately 22+50. After further investigation on 12/29/05 it was determined that this incident would be classified as a lost time incident.
EMPLOYEE WAS TAKING DOWN CABLE HANDLER MON-O-RAILING FROM MINE ROOF IN 6-N LONGWALL SECTION, #3 ENTRY, WHEN A PIECE OF COAL RIB BROKE OFF KNOCKING OUT A ROOF SUPPORT TIMBER, CAUSING THE TIMBER TO STRIKE EMPLOYEE ON SHOULDER (LEFT SIDE).
EE was operating the continuous miner (Alpine) through lunch on the 10-A section. While taking a short break a piece of roof coal fell and struck him on the hard hat. He looked up to check the roof conditions again and another piece had already fallen. A 5""x10"" piece of coal struck him in the face causing a laceration to his upper lip and chipping a tooth. Cut required 5 sutures.
THE INJURED WAS A PASSENGER IN A COVERED MANBUS WHEN THE DRIVER OF THE MANBUS ATTEMPTED TO STOP & THE MANBUS SLID INTO A PARKED MANBUS, CAUSING EE TO INJURE HIS NECK. THIS EE BEGAN MISSING WORK ON 12/28/05.
BECAME REPORTABLE ON 1/24/06 (1ST DAY OFF). ON 12/15/05 EE WAS REMOVING A METAL PANEL FROM A STOPPING WHEN A PIECE OF COAL RIB FELL, STRIKING HIM ON HIS RIGHT ANKLE AND TWISTING HIS KNEE (MUSCLE TEAR - RIGHT KNEE). EE CONTINUED WORKING UNTIL 1/24/06.
While working to remove a sheave wheel from its spindle on the 1st floor of the Prep Plant, the wheel broke free and caused the employee to quickly move out of the way. Employee struck the edge of his left foot against the concrete base of an electric motor. He sustained a fracture to the 5th metatarsal of his left foot.
Employee was helping unload a metal tool box from the compartment of a track jeep when he experienced pain in rt. side of low back and rt hip.
While dismounting the endloader in the supply yeard, the ee slipped off of the bottom step, He grabbed the handrail which caused him to spin around off of the step. His lower back and backside region struck the tire of the endloader. Began missing work on 12-9-05.
The employee was dismounting the front end loader at the Sugar Run Portal. As he stepped to the ground he slipped, falling onto his right side. Diagnosed on 12/19/05 with muscle sprain, knee contusion, bursitis of shoulder and arthritis of knees.
While exiting mantrip, employee twisted right knee stepping on lump of coal. He underwent surgery 02-09-06 on rt knee.
Crew was setting muck car on rail using a crane. Injured employee got inside the car to unhook the crane. Injured jumped off the car and landed on a small rock spraining his knee.
EMPLOYEE WAS ASSISTING IN HANDLING SECTIONS OF MONO-RAIL IN THE BELT ENTRY WHEN HE STEPPED OFF A WOODEN WALKWAY AND TWISTED HIS ANKLE. (6-N SECTION 0-69 MMU).
Became reportable on 02-02-06 when he was removed from work by physician prior to surgery. Employee was getting off of continuous miner, slipped (didn't fall), injured his shoulder when he caught himself.
THE INJURED EMPLOYEE WAS CUTTING OPEN A BAG OF ROCKDUST WHEN HIS KNIFE SLIPPED RESULTING IN A LACERATION TO HIS RIGHT THUMB. THE LACERATION REQUIRED 7 SUTURES.
Employee threw concrete form, form caught his vest.
IN THE PROCESS OF WALKING AND WORKING ALONG SIDE THE #3 BELT CONVEYOR, THE EMPLOYEE'S LEFT HAND CAME IN CONTACT WITH A BELT CONVEYOR TOP ROLLER.
WHILE TRAVELING IN MANBUS AS A PASSENGER, THE OPERATOR SPPLIED THE BRAKES AND THE MANBUS SLID CAUSING THE MANBUS TO SLIDE INTO A PATRKED TRAM, RESULTING IN AN INJURY TO LOWER BACK.
Laborer was loading his truck. He was climbing down the ladder from the bin he was loading from and hit his elbow on the cage around the ladder. The employee did not report the incident or see a doctor until 12/19/05. The office was not aware that this injury occurred at a mine until 1/6/05.
CONCRETE PUMP WAS BEING CLEANED OUT AFTER A CONCRETE POUR. THE EMPLOYEE WALKED IN FRONT OF THE DISCHARGE WHILE RETRIEVING A SHOVEL, AS CONCRETE WAS BLOWN OUT. DEBRIS HIT HIM IN THE LEG.
INJURY BECAME A REPORTABLE LOST-TIME INJURY ON 11/15/05. EMPLOYEE WAS GETTING OUT OF THE OPERATIONS COMPARTMENT OF A BATTERY POWERED SCOOP TRACTOR WHEN HE TWISTED HIS KNEE. EE CONTINUED WORKING UNTIL 11/15/05.
EE was operating a loading machine in 28-B North in #3 entry. He had just finished loading a shuttle car and when he looked back, he saw that the shuttle car had caught the loader cable and was dragging it around the corner. He got out of the shuttle car as the buggy stopped and as he straddled the loader cable the buggy started back up, pinning his left thigh against the loader.
EMPLOYEE WAS WORKING AT SLOPE BELT DRIVE. IN PROCESS OF RAISING BELT FAR ENOUGH OFF OF MINE FLOOR, TO PULL END OF BELT AND THREAD THROUGH DRIVE ROLLERS, AS RAISING BELT, SPINDLE END BROKE FREE CAUSING BELT TO DROP AND CATCH EMPLOYEE'S RIGHT HAND BETWEEN ANOTHER ROLL OF BELT. LACERATIONS TO RIGHT MIDDLE AND RING FINGER.
Employee was standing beside a concrete pump pipe. He removed his safety glasses to clean them. Some concrete hit him in the face thru a leaky joint. He received a bruise, and complained of eye irritation. Eye was cleaned and ee went to dr. Dr gave him 3 days off. No serious injury suspected.
PRODUCTION SHAFT MINE LEVEL, EMPLOYEE WAS CUTTING A GUIDE ROPE, AS THE ROPE WAS CUT IT BEGAN TO TWIST CAUSING LOOSE COAL TO FALL FROM ABOVE. THE EE DID NOT KNOW WHAT WAS FALLING AND JUMPED OUT OF THE WAY FALLING AND LANDING ON HIS RIGHT SHOULDER. ORG INJ DATE WAS 9/24/05. EE DID NOT START MISSING WORK UNTIL 1/6/06
THE INJ EE SMASHED & LACERATED RIGHT THUMB ON A DOOR LATCH MECHANISM ON THE MIRACLE RUN BOTTOM. FRACTURE & LACERATION WITH 10 STITCHES TO RT THUMB PAD.
The ee was struck on left knee by a piece of falling rock.
EE STATED THAT WHILE HELPING INSTALL A COVER OVER THE CUTTER MOTOR FOR THE NEW LONGWALL SHEAR OF 1B 8 NORTH LONGWALL, HE WAS HOLDING THE COVER IN PLACE WHILE TRYING TO PUT A BOLT IN IT TO SECURE IT WHEN THE COVER SHIFTED CATCHING HIS LEFT RING FINGER BETWEEN THE COVER & A HOSE FITTING THAT WAS UNDER THE COVER.
Employee stated that while carrying a bottom belt roller on #12 belt at 109 block 3 west, his feet became entangled with a compressor hose. This caused employee to lose balance and start to fall forward. He caught self. Employee is experiencing pain in left shoulder.
The injured employee was assisting another employee at moving a section of longwall cable trough out of the way on the longwall set-up. The employee injured his right arm in the bicep area. Diagnosed with ruptured tendon in elbow.
BECAME REPORTABLE 9/19/05 (1ST DAY MISSED DUE TO INABLITIY TO WORK) WHILE SHOVELING LOOSE COAL ALONG 6 N CONVEYOR BELT, EE TWISTED RIGHT KNEE
Employee was in process of unloading an ITE box at the mouth of 9A longwall section on the supply track entry. While lifting he felt pain in right lower back. Began missing on 8-30-05. Was informed by employee diagnosed with strain - experiencing muscle spasms.
THE EMPLOYEE WAS ASSISTING IN UNLOADING CRIB BLOCK OFF MATERIAL CAR WHEN HIS RIGHT INDEX FINGER WAS SMASHED BETWEEN 2 OF THE CRIB BLOCKS. RECEIVED 16 SUTURES.
EMPLOYEE STATED THAT HE WAS CARRYING A KENNEDY WALL DOOR WHEN HE SLIPPED AND FELL ON HIS RIGHT ARM CAUSING PAIN IN HIS LEFT KNEE AND RIGHT SHOULDER
The employee was installing a single hydraulic jack at the headgate alongside the stageloader. The jack ""kicked out"" at the top causing his right index finger to be smashed.
AT 7-M SECTION TRACK, EE WAS ASSISTING IN RE-RAILING A LOCOMOTIVE & FELT PAIN IN LOW BACK
Was performing rail/track installation in 7-n section. As he pulled a rail into place he noticed a burning sensation in his lower abdomen.
The emp. was on the inby side of the #50 motor in the process of chaining down the motor. The motor was contacted by another motor, caused #50 motor to knock the injured emp. to the ground.
Employee was taking a scoop off charge in the #5 entry on the Mains Section and stated that he twisted his right knee in the mud at the scoop charger station.
The emp. was lifting miner cable when he strained his lower back. 1st day missed was 7/28/05.
The emp. was on top of #3 belt removing a section of bagged down screen wire. A rock slid onto belt from the bagged down area. Emp. was diagnosed on 7/22/05 with hernia. Scheduled surgery 8/1/05 - 1st day missed 8/1/05.
Developed a discomfort in groin area during his normal duties thru the shift. no specific accident, or incident to cause the discomfort.
Tripped, and knee struck a rock on the ground near roadway.
EE WAS WEARING LEATHER BOOTS, WORKING IN WATER THAT WAS ANKLE DEEP. EE COMPLAINED OF PAIN IN HIS LEFT FOOT; LEFT GREAT TOE BECAME INFECTED. ACCIDENT REPORTED 7/26/05.
WHILE CARRYING A JENNY PIN IN HIS RIGHT HAND, EMPLOYEE TRIPPED & FELL STRIKING HIS RT. THUMB AGAINST A METAL PLATE. THIS RESULTED IN A BROKEN BONE & A LACERATION OF HIS RT. THUMB. THE LACERATION REQUIRED 10 SUTURES.
EMPLOYEE WAS DRAGGING TWO METAL ROOF SUPPORT STRAPS WHEN HE TRIPPED (AND FELL) WHILE CROSSING A TRACK RAIL RESULTING IN A TORN ACL LIGAMENT IN KNEE.
Became, reportable on 8-1-05, the first day missed as a result of the 6-15-05 incident. Ee was picking up a belt roller when he experienced pain in his upper left arm.
No specific accident. While doing normal duties, while handling a roof support strap, employee experienced a slight pain in his right shoulder.
EMPLOYEE WAS ALONG SIDE THE CONTINUOUS MINER OBSERVING THE MINING CYCLE WHEN HE WAS CAUGHT BETWEEN THE ATRS CONTROLS AND THE RIGHT COAL RIB RESULTING IN A HAIRLINE FRACTURE TO HIS LEFT PELVIC AREA.
Became reportable on 8-4-05, the first day missed for 6-7-05 incident. ee was walking and twisted his left knee, stepped in small hole while rockdusting face area.
EE WAS WORKING ON 8A SECTION. SHEARER HAD SET BITS AT HEADGATE - STARTED CUTTING TO TAIL. EE ACKNOWLEDGED START UP BY SHEARER OPERATORS. EE WAS PULLING SHIELDS. LEFT FOOT CAUGHT BETWEEN RELAY BAR & PONTOON (NO INJURY). RIGHT FOOT CAUGHT UNDER PONTOON - RIGHT FIBULA BROKEN.
THE EMPLOYEE WAS WALKING TO THE TRACK ENTRY AT 72 BLOCK OF ST. LEO WHEN HE TWISTED HIS RT. ANKLE. 1ST DAY MISSED 5/26/05.
9A SECTION - 88 BLK. EE WAS WALKING TO FACE OF #2 ENTRY. A PIECE OF HEAD COAL FELL FROM BETWEEN ROOF STRAPS, STRIKING EE ON TOP OF HIS HARD HAT AND SHOULDERS. APPROX. SIZE OF HEAD COAL WAS 2' X 3', VARYING IN DEPTH FROM 1-6"". EE COMPLAINING OF PAIN IN NECK AND BACK.
EMPLOYEE STATED THAT WHILE HELPING PUT A COVER BACK ON #85 MOTOR, FELT A PAIN IN HIS LEFT BICEP. CONTINUED TO WORK AND WORKED THE NEXT DAY. ON 21 MAY WENT TO THE HOSPITAL TO FIND THAT HE HAS A TORN BICEP MUSCLE. AT THIS TIME, IT WAS NOT KNOWN IF THIS INJURY WAS DUE TO A PREVIOUS SURGERY TO THIS SAME AREA. FINAL DIAGNOSIS DETERMINED THAT INJURY WAS NOT DUE TO A PREVIOUS SURGERY.
While standing on a ladder in a crosscut from #2 to #3 entry at 77+30 in 18-8 North miner section, he was attempting to lower the miner cable and water line out of orange hooks when he felt pain in his right abdomen area. He came to work 5/25 and worked 6 hours then came out of the mine and went to the hospital to find that he had a hernia on his right side.
While using a hand operated-pneumatic powered gopher roof drill to install 6' rppf blts in a rehabilitation area in a conveyor entry. the bolt wrench steel broke while tightening an installed roof bolt, causing the drill the continue upward striking the ee on face.
EE felt pain in his left knee while bending to pick up a bucket of miner bits. stated that he heard his left knee pop. he stated that he did not slip, trip or fall. began missing work on 4-27-05-received diagnosis on 5-2-05 degenerative changes in the medical joint compartment.
Using a pry bar to pry torque arm back on D&M Jeep #220 when bar slipped catching left thumb between bar and jeep hand brake handle, causing a fracture and laceration of left thumb.
The employee fell against locomotive while hooking up rockduster. Injured lt. shoulder-was diagnosed with muscle tear-shoulder was surgically reapried on 2/20/06. first day missed 2/20/06.
THE INJURED EMPLOYEE WAS STRUCK ON THE LEFT ELBOW AS HE WAS ATTEMPTING TO REMOVE THE SUSPENDED PIECE OF MONORAIL. FIRST DAY MISSED MAY 13, 2005.
The injured employee was in the process of setting bits in the LW shearer while spinning the cutting drum his foot slipped off causing pain in rt side from buttocks to calf. 1st day missed 4-26-05.
The employee had been unloading two inch plastic pipe. Later in the shift he reported to the foreman that his back and rt knee were hurting.
The ee was dragging a 1 inch was down hose thru 1st floor of plant. the hose hung on a motor base. The ee felt pain in right side diagnosed with hernia.
INSTALLING BELT ON MAIN EAST 3# BELT, BELT PULLED INTO POSITION AND WAS IN THE PROCESS OF BEING CUT.EE WAS HOLDING THE UPRIGHT BELT AGAINST THE RIB WHEN THE BELT WAS CUT, IT MOVED CAUSING EE TO LOOSE BALANCE AND FALL BACKWARDS, TRIPPING OVER THE DRAW BAR OF THE BELT DOLLY CAUSING EE TO FALL ON RIGHT ELBOW AND RIBS.EE WORKED 6-8, THEN DID NOT RETURN TO WORK TIL 14TH WITH DR EXCUSE
EE was roofbolting. as he pushed a 8' roofbolt up into a 4' bolt hole, the bolt bent catching his left thumb between the bent bolt and the bolt machine roof jack- resulting in injury to left thumb.
Became reportable as lost time on 1-9-06, the first day off. EE was prying up on a track rail when he felt pop in left shoulder, kept workinguntil his surgical repair on 1-9-06
Employee stated while operating 23 motor - pole dislodged from the trolley wire - as he reached to put pole back on wire - felt pain in lower abdomen and right side. EE continued to work until 3-30-05 received diagnosis of bulging disc in back.
EMPLOYEE FELL WHILE SHOVELING AROUND THE FEEDER OF 9 SO MAINS. THE EMPLOYEE HAS BEEN DIAGNOSED WITH A TORN RIGHT ROTATOR CUFF, SCHEDULED FOR SURGERY ON 5/10/05. 1ST DAY MISSED 5/4/05
EE CLIMBED ONTO THE SHELVING TO ATTACH A COME-A-LONG TO SECURE THE SHELF SO THAT THE FORKLIFT COULD BE REMOVED. WHEN GETTING DOWN IS WHEN EE SLIPPED ABOUT 3 FT OFF OF THE FLOOR CAUSING EE TO FALL HITTING UPPER LEFT RIB CAGE ON A BIT
Employee was assisting fellow employee remove stuck cement blocks in the loading machine conveyor, using a 20' long 2"" pipe to push on the blocks. Employee was in front of the loader holding the pipe against the stuck block as the loader moved forward causing the pipe to bend down against his leg.
THE EMPLOYEE WAS USING A TRACK HAMMER LUGGING A RAIL TO A STEEL CROSS TIE WHEN HIS RT. FOOT SLIPPED. THE EE EXPERIENCED DULL PAIN & TIGHTNESS IN HIS LOWER BACK.
EE felt pain in his left shoulder while assisting in loading a 60 lb rail on 3-1-05 and continued to work. The co was notified on april 28, 2005. when the ee began missing work that the ee had diagnostic testing which revealed degeneratives changes to his left shoulder there are no traumatic pathology.
EE helping to lace mine conveyor belt near tailgate #2 section, when he lacerated his r palm (1"" laceration). at base of little finger on r hand as a spindle cover for a belt clip cut thru his work gloves.
Employee stated that he was walking along the portal bus on 1b section when he stepped on a piece of coal causing him to lose his balance, when starting to fall he went to grab for the portal bust to catch himself and in doing so, hit his right hand on the corner of the portal bus. This resulted in a fracture to his right 5th metacarpal.
The ee was pushing a roof bolt into a bolt hole using the drill boom of the bolter. The bolt hung, bent and smashed the ee's left middle finger. Lacerated underside of finger- 14 stitches.
EMPLOYEE WAS ROOF BOLTING ON THE LEFT SIDE OF THE WEST ALPINE CONTINUOUS MINER/BOLTER, WHEN A PIECE OF ROOF ROCK/COAL FELL ALONG THE RIB LINE KNOCKING THE VENTILATION FIBERGLASS TUBING DOWN. THIS PUSHED THE EE AGAINST THE SIDE OF THE MACHINE RESULTING IN A LACERATION TO HIS CHEEK.
EE WAS LAYING 60LB MINE RAIL ON THE TAILGATE #2 SECTION. AS EE WAS USING SLATE BAR TO ROLL RAIL INTO POSITION, POINT OF BAR SLIPPED OUT OF HOLE, CAUSING STRAIN TO RIGHT HAND AND WRIST, BEGAN MISSING WORK ON 02/15/05
The employee was in the process of cleaning out dust tanks on the left side of Voest/Alpine miner bolter. As the auger rotated the employee was struck by a portion of the auger when the auger broke. Fourteen stitches in nose.
EE WAS WORKING ALONG #12 BELT AT 94 BLOCK, ROCK DUSTING BELT LINE. AS HE WAS WALKING AND MOVING HOSE SIDE TO SIDE, END OF HOSE WAS CAUGHT BY TOP ROLLER JERKING RIGHT FOREARM AND LEFT HAND BETWEEN ROLLER AND BELT BEFORE HE COULD RELEASE GRIP. RUPTURED TENDON LEFT MIDDLE FINGER
The employee dismounted #58 jeep, turned right ankle causing dislocation and fracture to three bones in right lower leg. She was hospitalized for surgery.
The ee was using a bar to move a track rail into place when he strained his left shoulder.
The employee was operating D motor (20 ton) on the tail end of two other cars. The D motor ran off track causing the employee to fall from the operator's seat striking his left shoulder and upper back.
EMPLOYEE WAS MOVING TROLLEY WIRE INTO 9A SECTION, HAD OPENED OUTBY HAULAGE DOOR AND PULLED WIRE THROUGH. AS HE WAS IN PROCESS OF CLOSING HAULAGE DOOR, END OF TROLLEY WIRE SPRUNG BACK AND STRUCK HIS LEFT KNEE, CAUSING A CONTUSION.
At 6N conveyor belt track spur, employee was prepairing to switch-out an empty rail flat car. As he stepped off the car, it moved causing him to lose his balance and fall against the coal rib resulting in a right shoulder dislocation.
Accident occurred at crosscut #41 on the main line. It is believed he raised his head too high and struck the first plank, knocking his hard hat off. He continued to travel out by and struck approx 4 additional planks. He was transported by ambulance to hospital, treated for a laceration above the left eye and across top of head.
EE was unloading some tires out of a pickup and heard a pop and felt pain in his right shoulder. He saw a doctor at the Medical Center and was released back to work. He continued to have pain and recently (on 7/18/05) went off for shoulder surgery. He worked from the accident date to the date of surgery.
BASED UPON PREVIOUS HISTORY AND MEDICAL DOCUMENTATION RECEIVED ON 3/3/05, EE WAS OFF DUE TO THIS CONDITION. DIAGNOSED DEGENERATIVE DISCS C3-4 IN ADDITION TO DISC DESICCATION AND BILATERAL FACET HYPERTROPHY IN AN EARLIER DIAGNOSIS
Supervisor was assiting ee in opening 2'x2' metal door from the jig vessel bin when the door un-expectedly blew open after bolts were removed, due to water. pressure. behind door. employee was push back against a stationary ladder resulting in a shoulder injury.
While operating a scoop the ee lt. middle and ring fingers were pinched between a stop block and the outside wall of the scoop cab. ee fractured the tip end of lt. ring finger, 8 stitches to pad of middle finger and 6 stitches to pad of ring finger.
Employee was working in prep plant replacing fly hts in a-bath. new flights were banded together. as ee broke bankd 1 flyht rolled and dropped 18 to 22 inches-striking the ee right foot just behind metatarsal-xray revealed no breaks-contusion.
THE EMPLOYEE BENT DOWN TO PICK UP ROCK DUST HOSE. FEET GOT TANGLED UP IN HOSE, TWISTED RIGHT KNEE. DIAGNOSED ON 3/4/05 WITH TORN ACL OF RIGHT KNEE. FIRST DAY MISSED WAS 3/4/05.
After filing sanders of his 50 ton motor located on the #2 empty track, he walked to the wire side of the track where he slipped on packed snow between the rails. He fell, landing on his right elbow and shoulder. At the end of his shift he felt a little sore and filed a report in case something became of it later. He worked until an MRI revealed that the rotator cuff was damaged.
THE INJURED EMPLOYEE WAS JUST BEGINNING TO GO UP AN EXTENSION LADDER, LOST HIS BALANCE & FELL FROM THE 3RD RUNG OF THE LADDER. THE EE STRAINED HIS LT. GROIN AREA.
EE was pulling miner cable when he felt a pain in his groin. He told the miner operator and worked his full shift. He worked the next day and was in a little more pain at the end of his shift. He told his supervisor on this day. The next morning he was in a lot more pain and decided to get it checked. He was diagnosed with a hernia and referred to a surgeon.
ee was installing I-Beams across intersection of slope when he strained right shoulder after attempting to lift beam over his shoulder
Employee was preparing to begin bolting on the left side of the miner in the #1 entry of 8 North Faces. He observed the top, set his ATRS and then reached for his starter auger when a rock fell out from under the first strap outby. This rock hit the chuck of the miner bolter and then struck him in the left thigh.
THE INJURED EMPLOYEE WAS CHANGING BIT INSERTS WHEN A SLIVER OF METAL WAS KNOCKED OFF STRIKING THE EE IN THE RIGHT FOREARM. THE SLIVER OF METAL WAS SURGICALLY REMOVED, THIS REQUIRED 4 SUTURES.
The injured employee was in the process of rolling a bent track rail out of travelway before moving a scoop. The rail rolled unexpectedly causing the employee's right ring finger to get caught between the rail and side of scoop car.
EMPLOYEE WAS WORKING AT 49 BLOCK MAIN NORTH TRACK ENTRY. HE WAS IN THE PROCESS OF LOADING DEBRIS INTO MINE CAR. EMPLOYEE HAD PICKED UP AN OLD 5 FOOT BOLT WHICH HAD A TROLLEY HANGER ATTACHED TO THE END. EMPLOYEE THREW BOLT INTO EMPTY MINE CAR. END OF BOLT BOUNCE BACK AND STRUCK EMPLOYEE ON LEFT CHEEK CAUSING A LACERATION.
The employee was operating the track bolter and sugar run. As the Emp. lowered the drill pot the pot contacted a stored drill stem. The stem flipped upwards and contacted the emp. in the mouth, chipped and broke front tooth.
Employee was pushing an I-Beam off of a Mucker when little finger on right hand got caught between the beam and the roof. Employee went to doctor received stitches and returned to work with no restrictions the next day.
ON 23M SECTION, TRACK ENTRY, EE WAS HELPING UNLOAD TRACK RAILS WHEN HE GOT A FINGER CAUGHT BETWEEN 2 RAILS, RESULTING IN INJURY TO FINGER.
Employee was standing at the headgate control box on 7 right longwall. a rock 18"" wide by 19"" long and 0 to 8"" thick fell from between the bolts and struck him on his right wrist just above the glove cuff on the thumb side, resulting in a laceration that required 5 sutures. The ee was wearing metacarpal gloves.
Employee was working on 8A section on tear down- in process of using a rope ratchet. Hand slipped causing ratchet to swing around, handle struck employee under left eye on face (cheek) resulting in a laceration requiring 2 sutures.
The emp. was carrying water pipe through man door when he smashed his rt. ring finger between end of pipe and stopping. 4 stitches.
Employee was repairing crawler on shovel. When he was aligning the thrust washer, it slipped off of dowel catching his finger.
EMPLOYEE WAS WORKING ON 6 NORTH MAINS SECTION. WHILE RETRIEVING BOLTS FROM SUPPLY CAR, RIGHT FOREARM CAME INTO CONTACT WITH A METAL STRAP USED TO SECURE ROOF BOLTS IN BUNDLE. RESULTED IN A LACERATION REQUIRING 7 SUTURES. RETURNED TO WORK HIS NEXT SCHEDULED SHIFT.
EMPLOYEE WAS CUTTING A PIECE OF BELTING USING A UTILITY KNIFE. THE KNIFE SLIPPED, LACERATION ON LEFT LITTLE FINGER. 5 SUTURES.
AT THE 5-WEST CONVEYOR BELT TRANSFER AREA, EMPLOYEE FELL BACKWARDS OFF AN IDLED CONVEYOR BELT AND STRUCK HIS LEFT FOREARM AGAINST THE MINE FLOOR, RESULTING IN A SMALL LACERATION REQUIRING 4-SUTURES.
THE INJURED WAS HELPING CHANGE OUT A 2"" WATER LINE ON 1 EAST MAINS BELT AT BREAK #32, WHEN HE SQUATTED DOWN TO TAKE THE BOLTS OUT OF THE VICTOLIC COUPLING HE STRUCK HIS LEFT KNEE AGAINST A BOTTOM BELT DROP DOWN HANGER, RESULTING IN A LACERATION REQUIRING 5 SUTURES.
Employee was helping load 6"" steel water line. While one employee was trying to dislodge a piece of water line off the stack, the injured employee reached in to help free it just as the other one moved the pipe causing his right thumb to be caugth between the pipe and a track rail, resulting in a fracture to his right thumb.
An airhose connected to a sandblast pot broke its coupling, striking employee in the arms and left leg. Whip checks were in place and did prevent more serious injury.
Joy loader in 1B section was blocked up for purpose of changing cross shaft for traction assembly. EE was under this area watching bearing that also needed changed. Due to bearing being difficult to remove, it needed to be removed in 3 sections. EE was observing removal. As a section was being removed a coworker was pounding on it, section came loose & hit EE in lip.
ON THE 3RD FLOOR OF THE PREP PLANT ATTEMPTING TO PULL A CYCLONE INTO PLACE. AS EE REACHED TO GRAB THE CHAIN, EE USED OTHER HAND TO GRAB A HANDRAIL, WHICH HAD BEEN REMOVED TO FACILITATE THE JOB, INSTEAD GRABBED A YELLOW RIBBON THAT HAD BEEN INSTALLED AS A WARNING. EE FELL 54"" STRIKING MOUTH ON A FLUME, RESULTING IN LACERATION WHICH REQUIRED 4 SUTURES
EE was roof bolting on a cont miner. The drill steel became plugged and he took the bolt wrench and was hitting the drill steel to try to clear the steel. When doing this, the socket of the bolt wrench flew off, striking the drill steel and bouncing off striking the ee in the right forehead just above the eyebrow resulting in a laceration.
This ee was working on a track switch at the neck of 12A-8 north. He raised the throw of the switch to align the connecting pin. As he was trying to insert pin, the switch throw handle fell, catching his right index finger against the switch housing
Employee was installing a 20"" diameter fiberglass ventilation tube at the face. The exhaust intake air pressure of the auxillary fan pulled his face toward the end of the ventilation tube causing his chin to strike the end of the tube resulting in a laceration.
EE WAS HELPING REMOVE A GUARD ON THE CLARIFIED PUMP, LOCATED ON THE BOTTOM FLOOR OF THE PREP PLANT. AS ANOTHER EE SPRAYED PENITRATING OIL ON THE BOLTS, SOME OF THE OIL ENTERED INTO THE INJURED EE LEFT EYE. EE WAS WEARING SAFETY GLASSES
CARPAL TUNNEL SYNDROME, BOTH WRISTS
Ee was given a diagnosis of pneumoconiosis by dr. on 10/27/05 and the safety department was notified 11/7/05. He last day work was june, 2003. He retired on this date.
Hearing loss safety department recieved notice on 11-7-05.

Employee was diagnosed with 4.3% noise-induced hearing loss.
OCCUPATIONAL ILLNESS. MEDICAL DIAGNOSIS (MRI) INDICATES EE HAS DEVELOPED BACK DEGENERATION AND ARTHRITIC CONDITION AS A RESULT OF WORKING IN MINE ENVIRONMENT. (DETERMINATION MADE 3/18/05.)
