One miner was entrapped on the Miracle Run elevator for 1 1/2 hours. While riding elevator to top a small piece of ice fell off shaft wall and hit the linkage on the SOS switch, manually throwing it, causing a safety fault and the brakes to lock up. No issue with mining machines, methods, ppe, training or compliance issues.

While hoisting supplies, elevator failed to stop at bottom landing causing damage to the guide rail, and channel beams on the elevator itself. It did not involve any compliance issues. There was no employee, or persons involved.
A roof fall occurred at 37 block intersection 23 West track, approximately 27' long, 30' wide and 9' high. The fall was caused by a geologic abnormality of a layer of unforeseeable fractured shale above the anchorage point of the resin assisted point anchor bolts. The fall will be cleaned up & supported. No problems with mining machines, methods, PPE, training or compliance issues.
A roof fall occurred above the anchorage point in the 2 west #1 entry at survey station 23+90. The fall was 16 feet wide and 20 feet long and varied from 8 to 10 feet high. The fall occurred due to a weakened laminated roof structure. It didn't involve any compliance issues. No training issues, No equipment involved. No injuries involved. No other 50.20 criteria applies.
A roof fall occurred above the anchorage point at survey station 34+03. The fall was 16 feet wide and 12 feet high. It occurred because of reduced pillar sizes due to a four entry area. It didn't involve any compliance issues. No training issues, no equipment involved. No injuries involved. No other 50.20 criteria applies.
Around 2:50 pm it was discovered that a roof fall at 42 blk St. Leo between the track and belt deteriorated to be above the anchorage. It is 7-8' high, 16' wide, and an unknown length (there is a stopping on the belt side without a mandoor). It does not impede passage, does not disrupt ventilation and will not be cleaned up. No problems with mining methods or compliance issues.
A roof fall occurred above anchor point on 2 west longwall headgate between crosscut 10-11. Fall was 50' L by 16' W & it varied from 15-20' H. It did impede travel on LW face. It didn't involve any compliance issues. LW stage loader was involved in the fall. No other 50.20 criteria applies. The accident was caused by a weakened roof structure due to a laminated rock transition.
Kuhntown elevator broke down with 2 men on board in excess of 30 minutes. Employees were safely removed at 11:53 P.M. PLC reset and approved by Schindler Elevator service to fix problem.
Roof fall occurred at 26 block Main West from track to belt that rode over into the belt line. Approximate size, discovered during recovery efforts, was 120' long, 16' wide and 30' high. No problems with training, mining methods, machinery, PPE or compliance issues. Belt line is being re-supported with arches.
A roof fall occurred above the anchorage point on two west number one entry at survey station thirty nine plus seventeen. While mining towards the headgate near shield 171, the shearer cut into the pyretic mine top which resulted in an alleged ignition. The mine was cited for not following the mine's ventilation plan where 4 inches of head coal is required to be left.

Entrapment of a person on the Wana Service Elevator for 45 minutes. The cause of malfunction was a stuck slide bar lock switch. (replaced switch)

A roof fall occurred above the anchor point on the 1 East section. It fell in the face of #1 and extended into the 1-2 crosscut. Survey station 26+54. The fall was approx. 60 ft long by 16 ft wide and 8-10 ft high. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
Roof fall occurred on the 6-North haulage at 24 block intersection 16' wide, 12' high. No persons injured, ventilation not obstructed, track haulage, secondary escape way obstructed, Fall will be supported by arches and track haulage and escape way restored.
Around 6:30 pm a fire was discovered at the St. Leo substation that supplies power to St. Leo fan. It is believed an insulator was penetrated, or failed, causing a short circuit and throwing transformer oil, with the oil catching fire and burning poles and the transformer. Fire extinguished around 7:30 pm. No problem with training, PPE, mining equipment, or compliance issues.
A roof fall occurred above the anchor point on the old mainline track outby the Whittaker portal at 11 crossover. The fall occurred in a 3 way intersection. It was 20ft long by 16ft wide and varied from 6 to 8 feet height. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
At 19D 1A-1 4 block charging area two employees went to put a 650 scoop battery on charge. Mistakenly hooked up a 636 ""Y"" jumper to the 650 batteries, shorting the batteries. Sparking and a small flame occurred for less than 10 seconds until extinguished with a handheld fire extinguisher. Process continued a dozen times until the battery leads were cut at approximately 10:55 am.
One miner was entrapped on the Sugar Run elevator, one quarter way up from the bottom, from 12:35 pm to 2:19 pm. A door switch had an electrical fault causing the brakes to lock because the electrical circuits showed an open door. The switch was replaced. Schindler elevator services was contacted immediately.
A roof fall was discovered in the 6-North Right return at Spad # 7536. no persons injured, ventilation not disrupted, walkway is obstructed. Roof will be supported a walkway reestablished.
Rail mounted trash car caught fire at 123 block on the Main North haulage. Communications were disrupted until 10:50 a.m. Trash car was covered with a fire retardant cover. Fire was extinguished by fire extinguishers, rock dust and water. Ignition source unknown.
The Golden Ridge hoist stopped with 23 employees on it for over 30 minutes. The incident was caused by an outside power issue. The employees were on the hoist for 2.5 hours. It didn't involve any compliance issues. There was no damage to hoisting equipment.
The Whittaker elevator stopped with an employee one employee on it. The employee was on the elevator for over 30 minutes. There was no damage to hoisting equipment. There were no compliance or training issues. The elevator was affected by the in coming power supply.
An alleged fire occurred at the dupont raw coal stock pile on the east side. The area was approximately 5 ft by 10 ft. MSHA determined it was a fire and issued a k Order. It didn't involve any compliance issues. No equipment was involved.
5 men were entrapped on Sugar Run elevator from 6:00am to 7:40am. Ice built up on governor rope and tripped governor switch. Ice came from a small hole in the shaft that allowed a water seep 100' from top of shaft. RG Johnson came down and put a plug in the hole to stop the leak. Ice removed from rope and shaft wall.
5 men were entrapped on the sugar run elevator from 6:05 am to 8:35 am. Ice built up on the governor rope and tripped the governor switch. Ice removed from rope and wall beside governor and hung brattice by opening that was blowing snow onto the rope. Elevator returned to service at 1:15 pm.
1 person was entrapped from 12:55 to 2:55 am on Sugar Run elevator due to an electrical fault (governor switch tripped). The switch was reset, miner removed, the contacts were cleaned on the starting panel relay for the motor fields. Returned to service at 3:50 am. No injuries, no issues with company rules, regulations, PPE or training.
6 people were entrapped on Sugar Run elevator from 12:58 to 3:10 am due to an electrical fault (governor switch tripped). Switch was reset, men were removed and fault was unable to be reproduced that day. Later had to clean contacts on starting panel relay for motor fields. No issues with company rules, regulations, training or PPE.
The person responsible for the daily inspection of the elevator got stuck on the elevator near the bottom. Schindler Elevator Service was called to evaluate and correct the issue. Ice build up on a door switch was found to have caused the issue. The employee was freed at approximately 5:45 AM. The elevator was throughly examined, evaluated and returned to service.
A roof fall occurred on the 8 South mains in the #2 Entry at spad location 55+45.The fall was 16' wide, 10' high, and 18' long. A slip/clay vein in the intersection was a contributing condition. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
Pumper discovered roof fall above anchorage (13' high, 16' wide, for unknown length) on backside of small sump area by K-Seals #2 Seal. Neither ventilation nor travel was restricted. Area will not be cleaned up.
Schindler called for electrical fault on Sugar Run elevator. At 12:53 pm they discovered a retaining bolt for a disconnect switch had backed out (due to rust). Repairs made immediately, returned to service by 1:30 pm.
Using 25 ton hoist in Metz Motor barn to lift C motor by attaching to old slope chain bracket. EE was installing a bearing housing when slope chain bracket weld broke off motor frame, motor fell, partially amputating right thumb (between first and second joint) between the motor and bearing housing. Company block to final working height procedures were not followed.
While loading a roll of conveyor belt, the roll suddenly shifted and rolled back against the employee left knee. The cause of the accident was the employee's failure to position himself out of the way of the moving belt. His inexperience also contributed to the accident. He did not violate any rules or regulations. He had on his required PPE.
While carrying notch posts from 1A to #2 in 21W the employee felt a slight pain in stomach that got worse as the shift progressed. Diagnosed with a partial abdominal tear in the upper right quadrant of the abdomen. No problems with PPE, mining machines, methods, training, or compliance issues.
Employee was spotting scoop while the operator was trying to pick up can when pile shifted and can fell on left foot causing multiple fractures. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills,proficiency,training or attitude. No PPE or protective devices on equipment involved.
Individual was walking along the belt line when stepping on the side of joint of rock dust hose, twisted the right knee, this did not involve any compliance issues, no equipment involved, It didn't involve job skills, proficiency, training or attitude.
Employee had finished dragging rock dust hose at 99 block #8 entry 9 South when the hose energized unexpectedly, striking him in the groin. This caused him to fall down, twisting his left foot and tearing some of the calf ligiments. No problem with mining machines, methods, PPE, training or compliance issues.
While stringing up barges employee threw rope around timberhead on barge, pulled slack and felt pain in arm, shoulder and neck. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
The ee stated that while carrying roof bolting material, he stepped on uneven bottom and twisted his left ankle. The ee has previously experienced a non-work related severe ankle fracture which required the insertion of a metal plate, pins and screws to re-stabilize the ankle. The ee failed to take the necessary precautions as to his foot placement while working.
Individual was standing beside the Long Wall Power Center when the high pressure hose came uncoupled striking the right ankle area, individual suffered a fracture to the right fibula, This did not involve any compliance issues, no equipment involved, it didn't involve job skills, training or attitude.
The ee claims to have felt pain in her neck and back when the mantrip she was riding in d-railed. The accident was caused by the ee's failure to recognize the rails were slick and did not properly use the sanding devices to slow the mantrip. The ee had on the appropriate PPE.
The ee claims that as he was rockdusting, the rockdust hose plugged. As he tried to use pressure to unplug the hose, it suddendly jerked causing the ee to fall to the ground. The accident was caused by the employee's failure to replace the hose and used excessive air pressure in an attempt to blow the hose clear. The ee had on the appropriate PPE.
A large rock was stuck in the panline at 6 shield on 22 West longwall, being broken up by a sledge hammer. The injured was walking by and a piece of the rock flew out and hit him in the right eye causing a laceration to his cornea. No problems with mining machines, methods, PPE, training or compliance issues.
The ee claims that as he was assisting with cable handling, the suspended cable fell striking him across the neck and back. He fell to the ground and complained of pain in his back and left knee. The cause of the accident was, the employee failed to pay attention to the hung cable and should have asked for assistance prior to attempting to lower the cable.
While center bolting the ee placed his gloved hand over the chuck to check the suction. A drill steel fell from the drill hole striking the ee on the tip of the left middle finger resulting in a laceration and fracture of the finger tip. The cause of this accident was due to the ee failing to secure or remove the drill steel prior to placing his hand in a vulnerable position.
Injured was drilling a hole for a 16' cable bolt on the tight side of 23 West belt at 22 block. The drill hit a hard spot about 7' in, stopping the bit and torquing the gopher drill which pulled on his right shoulder resulting in a strain/sprain. No problems with mining methods, training, PPE or compliance issues.
On 24 West the injured was assisting the mechanic in making a cable splice. He was hitting the beat block with a hammer and hit himself in the thumb, resulting in commutated fractures to the distal phalange and a hairline fracture to the proximal phalange. No problems with mining methods, PPE, training, mining machines or compliance issues.
Miner was sitting in a turn from #2 to #3 at 74 block 23 West. Injured was putting in a test bolt underneath a rock brow when he noticed an oil leak on the bolt station. When he turned to yell for the mechanic the rock brow fell and struck him in the neck and right shoulder resulting in minor soft tissue damage. No problem with mining machines, training or PPE.
Employee was walking around machine and tripped caught hand on roll of mesh and fell. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved. The accident occurred because the employee was not aware of his surroundings and tripped.
Individual was ratcheting a 1 ton tuggit to straighten a belt take up roller when experiencing a sharp pain in the left shoulder, trained on use of hand tools, doctor's request no reaching, pulling and pushing until 11/10/2014.
While walking across the 22A longwall, the employee struck his head against a low shield resulting in a strain / sprain injury to his neck. This incident did not involve any aspect of compliance nor the employees job skills, proficiency, or attitude. The incident did not involve protective items of clothing nor equipment.
Individual was operating a 20 tons motor when struck by a trip of 6 empty supply cars which came uncoupled from the lead motor when coupler / choker chain broke. Individual task trained on motor. Individual suffered contusion to the left knee.
Employee was helping install roof plank jack insert when the insert fell causing a fracture to his hand. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved. The accident occurred because the employee did not have proper hand placement.
Injured was replacing belt structure at 13 block (4 belt). He turned around after setting the structure on the ground and struck a low hanging roof strap with his head, just above his right eye. Resulted in 6 sutures. No problems with training, PPE, mining methods, or compliance issues.
Individual was dragging a door kit alongside the supply train on 13 WHG section when he twisted his ankle on loose coal/rock. He was removed from work by Med Express Fairmont, Fairmont, WV for swollen ankle and pain.
Shuttle car canopy caught roof strap and bending it causing it to strike the left hand of the employee. Diagnosed as light contusions and abrasions. No problem with training, PPE, mining machine or compliance issues.
Employee was pulling empty wheel barrow down a set of small stairs, the wheel barrow caught on something, suddenly stopping, causing pain in lower back. Diagnosed with muscle strain to back. No problem with training, PPE, mining machines or compliance issues.
When placing steel roof strap across the continuous miner the right hand was pinched between the strap and coal rib when continuous was finishing mining cycle, individual trained on job task. Individual received 11 sutures to the right index finger.
Individual was cleaning rock from Bunker bin when rocks slid striking the head area. Individual continued to work until 10/08/2014 then removed from work on 10/09/2014 by doctors.
Lifting a scoop car motor with tuggits, tuggits failed allowing motor to pinch the right hand between the motor and scoop frame, mechanic trainee, trained on hand tools, contusion to the right hand.
Employee was walking along the stage loader on the 2 west longwall when he fell and tried to catch himself injuring his right index finger. The injury resulted in a fracture and laceration. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude.
He was installing plastic mesh with continuous miner when the roof strap they were installing fell and struck the ee's hand. No compliance issues involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved. The accident occurred because they were installing roof mesh and not watching where the roof strap was.
In 23D, cutting the turn for #1 at 60 block, the injured was bolting off the right side of the miner when a 4'x4' rock fell from the top and struck his right humorous resulting in a compound fracture. No problems with PPE, mining machinery, training or compliance issues.
Mining 2-1 at 60 block in 23D the miner operator was preparing to make another sump when a rock (1 1/2' square and 5"" thick, approx. 50 pounds) fell from the top (about 3 straps from face) and struck the miner operator in the back of the neck and arm resulting in possible ligament damage. No problems with training, mining machines, PPE or compliance issues.
Driving 37 jeep out of the 60# at Sugar Run, being followed by 61 Jeep. 37 jeep stopped and 61 jeep slid into 37, knocking the injured off his seat and striking his right shoulder on the handle of the crank brake, receiving a contusion. No problems with training, mining machines, methods, PPE or compliance issues.
Individual was baring over a track rail on rail car when the electric winch's steel cable back lashed allowing the tail to drop 12"" striking the left knee area, trained on job task.
While roof bolting on the left side of a continuous miner, the employee had pre-made a roof bolt assembly and placed it along the right rib. When the continuous miner backed up, the rib protector caught the roof bolt, which then struck the employee with some force, along the right side of the employee's head just in front of the ear and temple.
Employee was taking the miner cable out of a hanger when he caught his left index finger between the cable and the hanger. The injury resulted in a laceration. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. Employee started missing work on 9/9/14
Replacing leg check block on 150 shield (22D longwall). While installing the bottom bolts, the block slid off the upper mounting bolts and pinched his left small finger. Received avulsion near the proximal end of the distal phalange bone requiring 3 sutures to close. No problem with mining machines, methods, training or compliance issues. EE was performing a two man job by himself.
Employee was picking up a 5 gallon can of hydraulic oil by the outby airlock doors of 22D (longwall) and felt pain in his lumbar area. No problems with training, PPE, mining methods, machinery or compliance issues.
While leveling new track installation in 23D the employee carried a 5' crib, while setting it down it slipped and struck him just above the left knee resulting in a contusion and swelling. No problems with training, PPE, mining methods, machinery or compliance issues.
While dismounting a diesel motor near the end of track in 23D the injured's metatarsal bones in his right foot felt like they shifted. Employee heard pop and felt pain. No problems with mining methods, machinery, training or compliance issues. Diagnosed with metatarsal sprain.
The employee stated that as he was picking up a PMCR - shield control unit, he felt a pain in his lower right side abdomen as he twisted with the box. The employee is suspected of suffering a hernia. This accident did not involve any aspect of compliance with rules, regulations or policies. It did not involve any Personal protective equipment.
Walking down the 23D belt line to install 2"" waterline and walked through a damp area (waterline was previously flushed out) and stepped on a piece of coal. Employee felt like knee twisted, continued to work and perform regular job duties until 8/27/14, started missing work on 8/28/14. No training, mining method, PPE or compliance issues involved.
Employee was changing out the can on an automatic greaser when he felt a pain in his lower back. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude.
1 E Sect-Employee was standing by the boom on left side of miner when a rock fell 30""X24""X2"" thick struck him on the head, neck, shoulder, & left arm. He stated that he was unconscious for a few seconds. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

The employee claims that as he was lifting and twisting cement block over chest high, he felt a pain in his middle - lower back. The accident did not involve any aspect of compliance with rules, regulations or policies, nor any mine equipment nor mining system. The accident did not involve any protective personal equipment.
Employee was carrying a roof strap from a supply car and was placing it on a scoop when he caught his right ring finger between the two straps causing a fracture and a laceration. He was wearing gloves. It didnt involve any compliance issues. No equipment involved. It didnt involve job skills,proficiency, training, or attitude.
While handling materials, the employee indicated that he felt a sharp pain in his scrotum. This accident was not reported by the employee until 08/19/2014. This accident did not involve any aspect of compliance with rules, regulations or policies, nor did it involve any mining equipment nor the mining system. The employee had the necessary PPE.
Ventilation tubing fell striking the left knee area, trained on lifting procedures, chain hooks stretched with the weight of the tubing allowing tubing to fall. Individual continued to work until 8/16/2014 when doctor removed him from work due to pain and swelling.
When swinging a sledge hammer to bust up a rock on top of miner, caught left middle finger between handle and rock, trained on hand tools and C M Utility, wearing proper PPE.
Struck head on long wall shield tip experienced neck pain, trained on job task, wearing proper PPE,
Lifting a bolter ATRS Jack felt pain in the lower back, Trained on lifting procedures, wearing proper PPE.
Hand dusting on 3 conveyor belt line between 1 and 3 block. Employees right hand hit the belt structure causing a laceration.
Employee was cleaning/shoveling around old loaded track switch. When he went to throw switch, the switch became stuck. When this happened he felt a pop in his shoulder. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.
While assisting with a belt move, the employee claims to have tripped and fell. In falling, he struck his knee on the edge of a cement block. The injury did not involve any aspect of compliance with rules, regulations, any mining equipment, nor any personal protective equipment.
At the end of afternoon shift, the injured was showering in the women's bathhouse at Sugar Run. She was barefoot and fell out of the shower, twisting left ankle, striking right knee on floor and hitting head on the bench. Sustained sprained ankle. No problem with training, mining machines, PPE or compliance issues.
In 23D at 39 block 2 to 1 (track chute) the employee was on the right side of the Joy 14ED25 miner installing the last strap in the entry (after holing). After finishing the drill hole, he lowered the timber jack and a piece of roof rock (18""x12""x1"") fell and struck his right elbow causing small lacerations and contusions. No issue with mining methods, compliance, PPE or training.
Injured was walking down the belt side of the track entry in 22D, moving the jumper cables outby. He stepped on a piece of coal and twisted his knee while falling down. Diagnosed with a sprain of his right knee. No problems with mining methods, machinery, PPE, training or compliance issues.
While hanging messenger wire for the high voltage cable, the employee claims that later near the end of his shift, he felt a pain in the left side of his neck. The employee missed 3 days, worked the next 2 days, missed the next 2 days, worked the next 2 days and has missed the last 3 for unknown reasons.
Injured was climbing down off of 22D middrive when he lost his footing and his left leg went under the wing on the middrive, twisting his left knee. There was no problem with mining methods, machinery, training, PPE, or compliance issues.
Removing belt structure when striking the bed rail pin with an axe (hammer side) struck the right hand. Trained on hand tool, wearing proper PPE, Individual continued to work until 8/09/2014 when the doctors removed him from work to repair a plate from a previous injury on same hand.
Injured was carrying rock dust in 23D in crosscut 2-1 at 36.5 block, when a piece of head coal (4' long, 3' wide, 1"" to 8"" thick) fell and struck the employee knocking him to the ground. Miner sustained contusions to the top of his head and some neck pain. No problems with mining machines, methods, training, PPE, or compliance issues.
A roll of conveyor belt accidentally became dislodged and rolled against the back of the employee's left foot resulting in multiple fractures of the foot. This incident did not involve any mining equipment, nor any aspect of compliance with rules or regulations. The employee failed to review his surroundings prior to moving the roll of belt.
Injured had tested a mine rescue apparatus (Draeger BG4) and placed it back in its container. When he picked up the container to place the unit back in storage he felt a sharp pain in his lower back. Diagnosed with strain/sprain of lower back. No problems with training, PPE, compliance issues, mining machines or methods.
Employee was standing along side the miner when a piece of rock fell from the mine roof and struck him on the head, left shoulder, and left arm. It resulted in a contusion. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
While walking the employee stated that his foot got stuck in the mud. As he pulled his foot out, he twisted his left knee and felt a popping sensation and a sharp pain. The accident did not involve any aspect of compliance with rules or regulations, nor any mining equipment. The incident did not involve any personal protective equipment.

It was determined by a medical examiner that he had a MASSIVE HEART ATTACK. Is not a work related injury/illness death. Cause of this death was a massive heart attack while the person was sitting in a chair.
