Accident type, without injuries

Miracle Elevator doors gapped open enough to activate door switch, which locked brakes and jolted over speed switch, activating it as well (discovered SOS has weak set spring, replacement ordered). 4 miners entrapped approx. 2 hours about 10 feet off bottom. No problems with training, ppe, mining methods or compliance issues.

Unplanned fall discovered reportable of brow of southern approach to 2 belt trench at the drive. Fall was above anchorage and 4' high, 8' foot long, 16' wide. Investigation discovered brow separated in area of damp, deteriorated clay material. Fell to trench excavation height. No problems with training, PPE, mining machines or compliance issues. Area mined in 1965.

During a pre-operational inspection, the hoist operator discovered that a crack in the load line drum caliper brake assembly. The hoist was immediately taken out of service and MSHA was notified.

A roof fall (66'L X 9'H X 18'W) was found on the old 6-North Track outby spad location 7810. This did obstruct the walkway but did not involve any other compliance issues. No equipment was involved and no other 50.20 criteria applies. The fall approaches will be supported and walkway rerouted.

The Metz elevator experienced a door fault error and engaged the safety brakes, entrapping 2 miners for 40 minutes approximately 175 feet from the top. The safety fault was reset and the elevator tested numerous times but would not reproduce the fault. No problems with training, PPE, mining methods or compliance issues.

A roof fall occurred above the anchorage point at survey station 57+35on the mainline near the river portal. The fall was 12 feet long and 10 feet high and the width of the entry. The fall occurred due to the age of the area, weather it didn't involve any compliance issues it did impede travel, no equipment was involved no other 50.20 criteria applies.

A roof fall was discovered in a non-active area of 6 North #4 belt at 285 block intersection. The roof fall was estimated at 16 ft in width and approx. 30 to 40 ft in length. The roof fall is approx. 10 ft above the coal seam. The area was initially bolted with 8 ft. 2 piece combination bolts. No injuries. No equipment damaged. No impared ventilation.

It was discovered the Sugar Run elevator was down for equipment damage after the MonPower substation at Harvey Run lost a phase causing damage to the elevator generator (4 resistor banks were burnt up and #5 line starter had damage to the tips). Repairs made by Schindler. No problems with mining methods, training, PPE or compliance issues.

Unplanned roof fall determined reportable on 24W longwall tailgate. Fall not above anchorage, but restricted travel. The travelway was impassable from 86+39 to the longwall face. 2-3' of dark, moist clay/shale was discovered above the wild coal in this location during the investigation. No problems with mining machines, training, PPE or compliance issues.

A Roof Fall (30' long X 6' high X 16' wide) was found in the Main North Left Return between 182 block and 184 block in the number 6 entry. There were no persons injured, ventilation not disrupted but the walkway is obstructed. No equipment was involved and no other 50.20 criteria applies. The fall will be supported and walkway reestablished.

A roof fall above the anchorage point occurred at 13+50 on the mainline at the river portal. The fall was 12 feet long and 10 feet high. The fall occurred due to the age of the area, weather and a rib sloughing knocking a crib over.it didn't involve any compliance issues it did impede travel, no equipment was involved no other 50.20 criteria applies.

A roof fall above the anchorage point occurred in the 2 East Longwall tailgate at engineering station 96+26. The fall was 67 feet long and 12 feet high. The fall occurred due to a weak roof structure of stack rock. It didn't involve any compliance issues, it did impede travel. No equipment involved, No other part 50 criteria applies.

Unplanned roof fall at the 47 block intersection of 4 belt, extending out towards the track. Fall approx. 20' high, 20' wide and 30' long. Small pocket of unconsolidated moist slick rock discovered above anchorage during investigation. Fall was cleaned up and re-supported with bolts, cribs and floor to roof support.

A Roof Fall (20' long X 10' high X 18' wide) was found in the 6-North Left Return,# 1 Entry at 21 Block spad number 8711. There were no persons injured, ventilation not disrupted but the walkway is obstructed. No equipment was involved and no other 50.20 criteria applies. The fall will be supported and walkway reestablished.

Unplanned roof fall in 21W discovered reportable at 2:56 am in the #2 entry between 3 and 4 blocks. The fall was approx. 16' wide, an unknown length and width, stopping about 5' into the #3 block intersection. Operator is retraining roof bolters at the mine and has submitted roof control plan changes.

A Roof fall (40' long X 8' high X 20 to 28' wide) was found on the Main North Belt at 150 block spad number 6631. There were no persons injured, ventilation not disrupted but the walkway is obstructed. No equipment was involved and no other 50.20 criteria applies. The fall will be supported and the walkway reestablished before mining resumes.

Unplanned roof fall discovered reportable at 12:10 am in 21W 2 block intersection of #3 entry. Fall was approximately 16' wide, 20' long and 10' high. The fall will not be cleaned up. No problems with training, ppe, or compliance issues. Mining method changed through roof control plan revision.

Unplanned roof fall discovered reportable at 5:23 pm in 22W 2 block intersection of the #1 entry. Fall was approximately 20' long, 16' wide and 13' high and covered a scoop. No problems with training, ppe or compliance issues. Mining method changed through roof control plan revision.

A roof fall occurred above the anchor point on the Whittaker portal mainline track at 26 crosscut. The fall was approximately 75 feet in length, 8 feet high, and 16 feet wide. No injuries occurred. The fall occurred due to the mine roof being deteriorated. The fall did impede travel. No other 50.20 criteria applies.

A Roof Fall (70' long X 8' high X 16' wide) was found in the 6-North Intake at 14 block, # 7 entry outby spad location 7427. There were no persons injured, ventilation not disrupted but the walk way is obstructed. No equipment was involved and no other 50.20 criteria applies. Approaches to the fall supported and the intake will be made common with the return air course..

A roof fall above the anchorage point occurred in the 1 East section beltline at nine crosscut Survey Station 3+41.23. The fall was 28 feet long 16ft wide and 8ft high. There were compliance issues and no other 50.20 criteria applies.

Roof fall was discovered at intersection of the inby recovery chute with the inby ladder located near 3 block of 22D. The fall was approx. 20' long, 16' wide, and 8-9' high. Possibly due to unforeseeable small area of unconsolidated shale with slickened sides just above the anchorage. No injuries. No problems with training, PPE, mining machines or compliance issues.

A roof fall (30' long X 6' high X 16' wide) was found in the 6-North left return between spad locations 7513 & 7842 in the # 2 entry at 18 block. This did obstruct the walkway but did not involve any other compliance issues. No equipment was involved and no other 50.20 criteria applies. The fall will be supported and walkway reestablished.

On 5-30-15 a roof fall occurred eight feet above the anchorage point at survey station 23+91. The fall was around 16 feet wide and 37 feet long. The fall occurred because of a sandstone transition in the roof. it didn't involve any compliance issues it did impede travel and the longwall stage loader was involved. No other 50.20 criteria applies.

While testing 9 South #4 escape capsule the chain which controls the level wind broke which also bent the mounting bolts of the sprocket. Ketchem Construction came and replaced chain and mounting bolts, returning capsule to service at 6:30 pm 5/7/15. No problems with PPE, training, mining methods or compliance issues.

A roof fall above the anchorage report occurred on 5-6-15 at 11:40pm. The fall occurred in the track entry in the old 5B longwall panel, due to deteriorating roof conditions & age of the area. The fall was 60 ft X 60 ft & 7 feet high. There were no compliance issues. It didn't involve job skills proficiency training or attitude. No PPE or protective devices on equipment involved.

A roof fall (15' l X 5'H x 14' W) was found on the old 6-North Track between location # 294.6 and 295.3 just outby the old 6-M. This did obstruct the walkway but did not involve any other compliance issues. No equipment was involved and no other 50.20 criteria applies. The cause of the roof fall is undetermined at this time. Fall will be supported and walkway reestablished.

The outby brow fell on the 9 transfer to 8 belt trench approximately 6 feet wide, 6 feet high, and possibly 30 feet long (most of which was alongside already installed steel sets down 9 belt). The area was resupported and cleaned up.

An alleged dust ignition appeared to have occurred around the cutter drum of the continuous miner at the face. No injury to the miners in the area nor damage to the mining equipment. The incident occurred while cutting in a belt trench with pyretic intrusions. Rules, regulations and policies were complied with.

Employee was inspecting elevator when a piece of ice fell off the shaft wall and struck the bottom overtravel switch, resulting in an entrapment until 4:30 am. No problems with training, PPE, mining methods or compliance issues.

A roof fall (75' long X 6' High) was found on the weekly examination in the 6-North Main left return between spad # 7842 & spad # 7513 in # 2 entry. This did not involve any compliance issues, no equipment was involved and no other 50.20 criteria. applies. The cause of the roof fall is undetermined at this time. Fall will bolted and walkway restored.

Unplanned roof fall discovered in 21D 1A entry at 2 block intersection. Fall approximately 50' long, 20' wide and unknown height. The fall will not be cleaned up, has been dangered off and the perimeter supported. No problems with mining methods, machines, training, ppe, or compliance issues.

Caught in, under or between a moving and a stationary object

Two employees were lifting a mine rail by hand, when one employee let go and the other employee got his finger caught under the rail when it hit the ground. This caused the employee a left middle finger distal tip amputation.

Caught in, under or between running or meshing objects

The employee stated that while drilling a roof bolt hole, the drill steel started to bend, as he reached out to grab hold of the drill steel, he accidentally put up pressure on the steel which caught his left hand between the drill steel and the bolt mast. The accident was caused by the failure of the employee to drop the drill mast instead of lifting it. The ee had on his PPE.

Over-exertion in lifting objects

The employee stated that while changing a belt take-up roller in the #2 dolly of 5 in the 22A belt takeup unit, he heard his right shoulder pop. The employee continued working and the incident became reportable when the employee began missing work on 04/15/2015. The accident occurred because the employee attempted to lift more than he was safely capable of lifting.

Over-exertion in pulling or pushing objects

Miner was using a chain fall to position the rock box for the 3 East transfer. He felt a pain in his left shoulder while pulling down on the chain fall trying to maneuver the rock box into its proper location. Given time off for shoulder pain. Was reaching overhead to pull down. No problems with mining methods, ppe, training or compliance issues.

Struck by falling object

Employee was performing belt work along a section belt entry when the ""J"" hooks holding the waterline up to the roof channels, failed causing the waterline to fall onto the employee's shoulder knocking EE to the floor. Employee recently began missing work to have surgery on rotator cuff due to this past injury. Employee didn't violate any regulations and was properly trained.

Injured picked up winch hook to move from behind bucket ram into the scoop bucket. While positioning the hook in the bucket he pinched his gloved middle finger between the winch hook and the bucket, receiving a fracture. Occurred in track heading of 1 East, 27 block. No problems with training, ppe, mining machines or compliance issues.

Over-exertion (Not Elsewhere Classified)

While bolting in the 2E beltline and dragging Z-Drill employee felt pain in right shoulder resulting in possible rotator cuff injury. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies.

Fall onto or against objects

Miner was turning water pressure back on at 2 block 2 east at the end of the belt spur (after installing booster pump). His hands slipped off handle and fell over backwards hitting the back of his head on the edge of a supply car causing a laceration that required staples. No problems with mining methods, ppe, training or compliance issues.

While de-trashing 24W transfer area he picked up a bottom roller that was leaning against rib, the shaft caught on the rib, pulled out like a plunger and then smashed down pinching his right middle finger between shaft and roller shell. No problems with mining methods, training, PPE or compliance issues.

Over-exertion in lifting objects

While lifting a shuttle car anchor, the ee overextended his left shoulder. The ee first began missing work on 12-11-2015 & is periodically attending physical therapy. The incident occurred because the ee failed to get assistance when lifting over his head. The incident did not involve the mining system nor any PPE. The ee was properly trained and has a good safety attitude.

Over-exertion in lifting objects

While carrying a pump, the employee slipped and stumbled and afterwards complained of pain in his right lower abdomen. The mine's Safety Department received notification on Dec 18, 2015 that this is a hernia and the employee will undergo surgery on Dec 29, 2015. The incident occurred because the employee was not aware of the bottom conditions.

Struck against stationary object

Employee was walking back to belt entry after taking equipment out to the track, he stepped on a piece of belting laying on the floor and twisted his ankle. Cause of this injury was employee awareness to the task at hand. Compliance with rules/regs. not a factor, miner proficiency could be a factor, PPE was not a factor, mining equipment/systems not a factor.

Over-exertion in lifting objects

Trolley pole of 126 jeep came off the wire while traveling Main West haulage. Injured leaned over to put pole back on wire and felt pain in lower back. Continued to work normal job until 11/28/15, when he missed 3 days due to the injury. No problems with mining methods, training, ppe, or compliance issues.

Struck by falling object

Using come along, slate bar and hammers to remove a section of handrail off the panline (near headgate in 24W). Injured was standing in front of the handrail prying it off and it landed on his left foot causing a contusion. No problems with ppe, training, mining machines or compliance issues. Body placement was very poor.

Fall from scaffolds, walkways, platforms

Injured replaced a piece of grating on slope catwalk at 2780, then walked down catwalk and a separate piece of grating failed and he fell striking his right shoulder on the concrete floor below. Started missing work much 11/23/15 for persistent shoulder pain. No problems with mining methods, training, ppe or compliance issues.

Bodily reaction, (Not Elsewhere Classified)

Walking bleeders from 24W to 17W and felt pain in lower back, going down both legs, when he got to bleeder fan. Miner has history of lower back issues (herniated lumbar disc) aggravated by walking bent over and hitting his head on the roof a few times. No problems with training, mining methods, PPE or compliance issues.

Fall to the walkway or working surface

While carrying a 2"" air pump in the bleeder walkway near 21W tap he stepped on a section of slick line and fell backwards, landing on his back. The location where he tripped was dry. He is experiencing localized pain in the upper lumbar and has been referred to a specialist. No problems with training, PPE, mining machines or compliance issues.

Over-exertion in lifting objects

After handling a miner cable, the employee felt pain in his back. The incident occurred due to the ee's improper lifting and handling of the cable. The ee had on the appropriate PPE, was proficient, properly trained and has a proper attitude. The incident did not involve the mining system or mine equipment nor any aspect of compliance with rules or regulations..

Contact with electrical current

The individual was welding with a DC Welder in a tight area when the left side of the neck came in contact with the stinger and received an electrical shock. This did not involve compliance issues, no equipment involved. Personal safety equipment being utilized. This did not involve job proficiency, or attitude. Individual went off work for surgery 1/21/2016.

Over-exertion in pulling or pushing objects

Individual not concentrating while moving the track rail bender along the rail when it became stuck. When pulling on the bender he felt a pop in the right shoulder. This did not involve any compliance issues. It did not involve jobs skills or attitude. Personal safety items were being utilized.

Fall to the walkway or working surface

Employee was carrying come-a-long on belt move when he tripped on chain and fell striking his right shoulder. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved

Over-exertion in lifting objects

While standing from a kneeling position, the employee felt his lower back ""pop"". The accident occurred due to the ee improper lifting after handling materials. The ee had on the appropriate PPE and was properly trained. The ee has a poor safety attitude.

Employee bent over to pick up a stopping panel when he experienced a sharp pain in his left lower back. This did not involve any compliance issues, job skills, training, or attitude. Personal safety equipment was being used.

Over-exertion in lifting objects

Employee was unloading rockdust from a supply car and stacking them. After stacking bags employee felt pressure and then extreme pain in lower back. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved. No other part 50 criteria applies.

Fall onto or against objects

While attempting to unload a roll of belt, employee slipped while using a pry bar causing him to fall in an awkward position twisting his left knee and fracturing his left ankle. This did not involve any compliance issues, job skills, training, or attitude. Personal safety equipment was being used.

Struck against a moving object

While sitting in a vehicle, it was struck by another vehicle causing the ee to fall to the floor. The incident was caused by a careless operator of the 2nd vehicle who did not observe the 1st. The injured ee was wearing the appropriate PPE but has a poor attitude. The operator was trained but received retraining in the safe operation.

Winching a car of belt with a belt winder on 7 belt. He pulled off rope to get more slack and got his foot caught in some hydraulic hoses. When he energized the winder, the hoses moved and tripped him. Diagnosed with fracture to distal end of fibula. No problems with training, mining methods, PPE or compliance issues.

Employee was injured on 10/04/2015 and continued to work until 11/03/2015 when doctor changed treatment and took him off work. He was reinstalling guarding when he turned and slipped hyperextending his right knee. All personal safety items were being used. It did not involve compliance issues, no equipment involved. It did not involve job proficiency or attitude.

Struck by falling object

Bolting (and screening) in 1East #1 entry, 17-18 block, in an area with exposed shale. The ATRS was set and he was starting to drill first hole when a rock (18""x24""x5"") broke loose outby the drill canopy, swung around and struck him in the lower back. Started missing work on 10/2/15 for contusions. No problems with training, PPE, mining machines or compliance issues.

Employee was helping shearer operator spin the shearer drum when the operator fell into him causing him to fall striking his neck on the panline. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.

Struck by falling object

Employee was bolting off of the continuous miner with the rock deflector up & the rib door extended when a rock (46""x35""x6"")fell from in front of the last strap hitting the rock deflector then hitting the rib protector. The rock then came into the bolter station striking EE in the back. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved.

Struck against a moving object

Employee was injured on 9/20/2015 and worked until 10/20/2015 when doctor recommended change in treatment and pulled him off work. While riding in BT24, ride picked the switch and derailed causing employee to strike his left elbow and shoulder against the side of BT24. It did not involve compliance issues, job proficiency or attitude. All personal safety items were being used.

Fall from machine

Individual was dismounting 24 track mobile not concentrating, caught his foot on the vehicle frame or safety chain causing him to fall (sprain / strain) to the left foot. This did not involve any compliance issues. No equipment involved. This did not involve proficiency or attitude. All personal safety items were being utilized.

Fall to the walkway or working surface

Walking through the dust chute on Metz Bottom and fell, spraining his right ankle. He is unsure whether he tripped, or if his problematic surgically repaired knee gave out and caused the fall. No problems with mining machines, training, PPE, or compliance issues.

Over-exertion in pulling or pushing objects

Individual was working in a low Roof Fall area prying down loose material when experiencing a pop in the upper back and shoulder area. This did not involve any compliance issues, no equipment involved. It did not involve job skills, training or attitude. Personal safety items were being utilized.

Employee had cleaned 15 tail and was walking back to his mantrip, when he stepped over the rail he rolled his right ankle, resulting in a sprain. He continued to work until 9/22/15, when he missed 3 days due to the injury. No problems with training, ppe, mining machines or compliance issues.

Individual had thrown the K-4 track switch, not concentrating on the switch throw, started walking back to the 20-ton motor when the switch throw sprung up from the hold down position striking the right knee. This did not involve any compliance issues no equipment involved. It did not involve job skills, training or attitude. Personal safety items were being utilized.

Individual was assisting in hanging 12470 cable on the Longwall Power move when the cable slid off the pump car creating a chain reaction of the cable. Because of lack of concentration the cable struck employee on right shoulder. There were no compliance issues or equipment, did not involve lack of training or attitude. Employee continued to work until 9/19/2015.

During a Part 50 audit and citation 8060588 being issued, this injury report is being filed. Employee was installing bolt when foot slipped and caused drill to spin striking employee in the face resulting in a facial fracture. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

During a Part 50 audit and citation 9123775 being issued, this injury report is being filed. Employee stepped on miner to see over it when shuttle car came up and struck left foot resulting in a fracture and sutures. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was attempting to pull cable slack by putting cable on shoulder when he went to put cable on shoulder he stepped in a rut and twisted his left ankle. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies.

Struck by rolling or sliding object

While operating the Shearer on the Long Wall Face, when passing 152 shield a spare leg jack that was stored on the shield pontoon broke a chain and rolled striking the individual on the lower right leg. This did not involve any compliance issues, job skills, training or attitude. Personal safety equipment was being used.

Walking by miner in 1 East #3 heading inby 7 block and a piece of loose rib (2' by 8-12"") fell and struck him in the head and neck. X-rays were negative, given time off for possible soft tissue damage. No problems with training, ppe, mining machines or compliance issues.

#28 bolter in 1 east lost suction, the injured had bolter operator open dust dump box to check the rubber seal. While checking the seal the dust box closed, pinching his right thumb and resulting in 23 sutures and a hairline fracture of the thumb. No problems with training, ppe, mining methods or compliance issues.

Bolting off the miner in 2 East while cutting a turn in #1 entry a rock fell, broke on the rib protector and rolled on the back of his left foot resulting in a blood blister and possible soft tissue damage. No problems with training, ppe, mining machines or compliance issues.

Attempting to pull face shield that would not pull in typical remote. Employee stood on toe of shield and was manually operating solenoids. While pulling shield he got his left foot caught between shield toe and pan line resulting in a sprain. No problems with training, PPE, mining methods or compliance issues.

While testing a water hose (for cutting purposes) at 46 blk Main West when a 6"" ball valve failed and struck him in the left knee. Initial x-rays were negative, employee had next two weeks off, however a later MRI (8/27/15) showed a hairline fracture to the proximal end of his left tibia. No problems with mining methods, ppe, training or compliance issues.

Absorption of radiations, caustics, toxic and noxious substances

After plastering a stopping, the ee indicated that he had a skin irritation develop on his left forearm. The incident happened because the employee allowed dry B-Bond to get inside his shirt sleeve. The ee was properly trained to use the material and knew the precautions that he was to follow. The incident did not involve any mine equipment nor the mining system.

Employee was carrying a water pump in the back of #1 charge chute when he struck his head on a bolt jamming his neck.There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies.

Began missing work for shoulder injury on 9/28/15, with a doctor slip putting him off work to await an MRI. No problems with training, PPE, mining methods or compliance issues.

Employee was hitting headgate sprocket with sledgehammer when a piece of metal came off of sprocket and hit employee in the upper part of his left leg causing a laceration to his leg. While moving the shear from Sugar Run to 24W the cable handler had to be removed at 55 blk St. Leo. They removed the bolts and when he tried to pick it up it rolled over and crushed the end of his little finger, resulting in a fracture and laceration. A small section of roof material fell from around the bolt he was installing striking the ee about the head and shoulders. The event became reportable when the employee reported the incident to management on 7-30-2015. The incident occurred because the ee failed to conduct an exam of the roof prior to drilling. The employee was properly trained and had on the appropriate PPE.

Contact with electrical current

Employee had set power up for the Fletcher Bolter at the Power Center. He then walked to the machine and while pushing the power button to start Bolter, received an electrical shock.

Employee was in the process of unloading a CLA pot for a Joy miner when it shifted and fell on his left foot resulting in three fractures. Individual was using Chain Tuggits to lift a 636 scoop canopy when the canopy twisted the Tuggit chain hook came dislodged from the chain allowing the canopy to fall striking the right hand.
