The bracket that holds the guide for the door locking mechanic broke when it contacted some ice.
Weekly examiner found a roof fall at 8th South right return #6 entry at #6 x-cut. There had been a previous roof fall in the x-cut 6-7 and the intersection fell in approx. 20' x 15' x 7-8 feet high. The area was dangered off and timber set at both accesses. Does not impede ventilation and is not an escapeway.
At approximately 1:35 am the belt crew was moving belt when they heard a roof fall. Supervisor immediately went to the last open x-cut in #8 entry at #23 x-cut of 12th West section and found a roof fall 20' x 20' x 4.5' high. The area was dangered off and called to the shift foreman. Shift foreman notified superintendent and safety supervisor. Not above 5' bolts being used.
A non-injury accident, the unintentional stopping of the elevator at Camp Run Portal, entrapping one (1) person on-board the elevator occurred. The source of the malfunction was due to a loss of hydraulic fluid in the cylinder to the gripper.
The Kuhntown Elevator was down from 9:07am until 11:30am. There were no employees on the elevator at this time. The elevator failed due to a loose plank switch. Switch was replaced.
16-W tail section. #3 entry, 69 block, 100' inby intersection a roof fall occurred. Length of fall 50', 8' high in middle of the entry.
The continuation of a previous roof fall was discovered at approximately 1:00 AM. The continuation measured approximately 8 ft in heigth, 10 ft in width and 15 ft in length. No injuries to personel or damage to equipment occurred.
On 10/3/08 the roof set down at 3+70 in the tailgate of 2B longwall. Face was at 3+90. Roof didn't fall but as a result the approved blockage plan was put into effect. At the time that this was called in to Safety Supervisor, it was not known if it was a fall or not so the agencies were called just in case. It was found that it was not a roof fall.
A non-injury accident, the unintentional stoppage of the elevator at the Oakdale Portal entrapping 6 people on-board from 5:23 pm until 7:15 pm. A repairman from Schemdler was called to correct the malfunction.
An unintentional roof fall occurred in the # 1 entry of 6-Mains starting at SS# 3907 extending to SS# 3847 and continuing inby SS# 3847 into the #4 entry of 6-Left panel for approximately 80'. The fall dimensions were approximately 18' w x 6' h x 260' long.
At 11:25 am Mine Foreman called out to me that he had found a roof fall in the 6th West bleeder at x-cut 22 in the intersection of the #2 entry that measured 30' x 25' x 6'-7' high. Five foot resin bolts had been installed in this location. MSHA was notified at 11:30 am and a K-Order was issued verbally. Also called into 800 number at 11:33 am.

The fall has been dangered off and additional supports implemented on all sides. No one was injured, ventilation was not interrupted and no equipment was caught. This fall will not be cleaned up.
Roof fall at 107 BK Main West #9 heading. Intersection leading to 6N Seals. Fall 18' x 18' x 7' high. Area mined in 1970s.
A non-injury accident, the unintentional stoppage of the Camp Run Portal elevator due to an electrical component malfunction from approximately 4:05 PM until 8:00 PM.
A roof fall measuring approximately 60 ft. long x 18 ft. wide x 6 ft. height was discovered in the intersection of the #4 entry of 3 East Mains Bk. 4 at survey station # 18496.
An unintentional roof fall occurred at ss# 4215 in the mains bleeder #4 entry. The fall dimensions were approx. 35;L x 18;W x 6'H.
At 1:35 a.m. the Kuhntown elevator stopped operating. An employee was on the elevator from 1:35 until 3:10 a.m. The door switch was broken.
A roof fall was discovered on the Grassy Creek Mains in the #3 entry 20' inby s.s.#22650. The fall measured approximately 22' wide x 87' long x 10' high. No one was injured, no equipment was caught and ventilation was not disturbed. The fall has been dangered off and additional supports implemented on all sides. This fall will not be cleaned up.
The Oakdale elevator became inoperative at approximately 7:00 PM. The malfunction was due to a faulty / sticking electrical contactor. A representative from Schindler Elevator Repair service was called to correct the condition. No person or persons were injured in this event.
An unintentional roof fall was discovered on 4 north mains at crosscut # 93 in the #2 entry survey station # 8419. The fall measured approx 20' x 20' x 8' high. The fall has been dangered off and additional supports implemented on all sides. No one was injured, ventilation was not interupted and no equipment was caught. This fall will not be cleaned up.
23-M Section, #3 Entry at 13 block intersection, unintentional roof fall, ventilation not affected.
Unintentional roof fall in the walkway 1 block outby #5 seal in the intersection located in the right return on the main north main 185 blk. Ventilation not affected.
While making the required checks on the safety catches of the Page Service Elevator, one of the spare counterweights stored at the coal seam level was accidently knocked into the sump of the shaft striking one of the tail ropes resulting in broken wires & distortion. No one was injured & no other damage was reported. The damaged ropes were replaced.
The rib over the inby overcast wall located along 6 North belt at 71 block deteriorated and fell out breaching the separation of the air-courses. Fallen material measured 5 ft by 4 ft by 14 ft. No injuries to any employees.

12-M seals, between #2 & #3 seal. Rock fell above roof bolts, area supported & walkway rerouted outby to next X-cut and back # 3 Seal, this area is a weekly examination.
A roof fall between 215 & 216 blk on #4 belt. 3' X 5' fall above bolts.

The fall measured approx. 20ft wide x 50ft long x 12ft high.No one was injured,no equipment was caught and ventilation was impaired.Roof fall wont be cleaned up and the area has been timbered off.
An unintentional roof fall occurred on 3 east mains section in the #4 entry, brk. 127 at S.S. #25282. The fall measured approx. 20ft wide by 20ft long by 10ft high. No one was injured and no equipment was involded and ventilation was not impaired. The fall was timbered off and it will be cleaned up.

While performing a preshift examination of the recently constructed seals on 3 right off 3 east mains, the examiner discovered a thermal event within the #6 seals. Upon close examination of the seal, it was ascertained that a fire was within the seal. Efforts to extinguish the fire lasted approx. 28 minutes.
Face ignition lasting 2-3 seconds no injuries.
At 12:30 am the Kuhntown elevator stopped about 150' off the bottom. An employee was on the elevator at the time and was trapped from 12:30 to 2:00 am. A contactor was found to be bad and tripped the overload. The contactor was replaced, elevator inspected and put back into service after K-order was lifted.
A surface brush fire was discovered on mine property near the Neely Pump. Pictorial evidence gathered the following day indicated that vandals may have been burning off the insulation to a section of pump cable. Evidence from nearby spectators indicate two young men on 4-Wheelers left the scene just moments earlier. The nearby fire department extinguished the brush fire.

(Battery Charging Station Area) Miracle Run bottom battery car on fire due to a short in the cell compartment. No injuries reported.
An unintentional roof fall occurred on the 1 East Mains in the #2 entry adjacent to SS# 278. The fall measured 25'L x 25""W x 10' high. There were no injuries, no equipment was caught and ventilation was not disturbed. This fall will not be cleaned up.
An unintentional roof fall occurred in the 4 North Mains #3 entry at crosscut #96 at SS #18557. The fall measured 25'L x 25'W x 10'H. No one was injured, ventilation was not impaired and no equipment was caught. This fall will not be cleaned up. The area surrounding the fall was cribbed off.
A fall of roof was found on 6 north haulage #6 entry at 22 block. Fall starts at a 4 way intersection and goes inby. 8' H x 16' W x 40' L. 846' of cover. Developed Aug 1992.
The escape hoist at vent shaft #6 was found inoperable due to a damaged hub which connects the speed reducer to the hoist drum. This escape hoist does not serve the working sections or the longwall. The hoist will be repaired as soon as possible.
A roof fall measuring approx 40' long X 19' wide X 7' high was discovered in the #7 entry of 3 East Mains approx 55' inby survey station 20813. The fall was discovered after mine reopening, which had been closed for several months due to a mine related incident. The actual date of the roof fall can not be determined due to the mine being closed, so date is date of discovery.

A roof fall was discovered during mine rescue explorations on 1/31/08 on the Grassy Mains Bleeder entries at Brk #80, #6 entry-SS #14655. The fall was in an intersection and measured 30'L X 30'W X 9'H. The actual date of the roof fall cannot be determined due to the mine being closed, so the date of the accident is the date of discovery.
A roof fall was discovered during Mine Ruscue exploration (exploration due to incident at the mine which had it idled for several months). The fall was located on the 3 East Mains Section in the intersection of the #3 entry at survey staion 24992. The fall measured approx 25' long X 20' wide X 5' high. The fall fell across the ATRS pf, a Fletcher Dual Head Roof Bolter
A roof fall was discovered during mine rescue exploration on 1/30/08 on the 12 RT. section. The fall measured approx 50'L X 20'W X 9'H in the intersection of #2 entry at SS# 25192. The fall caught approx 10' of the continuous miner at the cutter head of the machine. This fall will be cleaned up.
Unintentional roof fall - old belt heading. All entries in area common. All stoppings in area intact.
A non-injury accident, the unintentional stoppage of the elevator at Camp Run Portal, entrapping 1 person on-board the elevator occurred. The source of the malfunction was in the circuitry when switching from a generator back to the main power source while testing.
A tail peice with motor and reducer damaged the side of the cage while trying to load it on the cage when it was on the bottom. The motor and reducer were taken off and the tailpiece taken out. The cage wall was further damaged when the tailpiece was unloaded on the surface. Repairs were made and inspection of hoist conducted. Returned to use 1/11/08 3:50 PM
A roof fall was found at 1:52 am by the midnite shift firebosses in 6th West at x-cut 14 in the #3 entry. The fall is approx. 20' wide x 30' long x 8' high. The fall has been dangered off on all sides and additional support set. It is not an escapeway and does not impede ventilation (intake). No injuries or equipment involved.
A death of an employee occurred on or in the Ohio River. The death is still under investigation & the autopsy report has not been released. The cause is unknown at this time. Report is being filed under protest due to jurisdictional issues yet resolved. The deceased was working on empty barge when last seen. Upon his disappearance, a search commenced & his body found approx 2:30am.
Employee was marking locations to install roof bolts, a piece of slickensided rock fell striking the employee's head and crushed his head against the drill station canopy.
Employee was involved in a fatal rail haulage accident. The accident investigation is on-going. EE-fatally injured while transporting two flats of supplies along the 3-west track haulage. Other ee discovered the victim laying between the clearance side of the track and the mine rib at No. 129. EE was unresponsive at this time. EMT arrived-unable revive the victim.
Boom truck being used to set concrete safety barriers beside driveway at the request of MSHA inspector. Boom assy. on truck had approx. 18' clearance under overhead power line. Operator of truck and deceased had just moved truck and reset up to have more clearance under the wires. On first lift the boom was extended too far and made contact with wires. Day was clear, dry and sunny.
The employee got his left middle and ring fingers caught between the headgate frame work and the shield clevis rail. The resultant injury was a partial amputation of the end of the middle finger. Both the middle finger and the ring finger required multiple sutures.
EE was setting shield #79 on the o-Panel longwall and his right foot was caught underneath the pontoon and relay bar of the shield which initally resulted in fractures of the foot and ankle then later required amputation of the right foot.
Pinched left index finger while positioning a wedge between a jack and the jeep that was being re-railed.
EE stated that while roof bolting in 2 South East, he was trying to pull a roof strap back to line up the hole to install a roof bolt. When doing so, he felt pain in his right shoulder. After further investigation, it was diagnosed that the EE had a muscle strain and on 12/9 it was determined that the EE missed time from work due to the condition and is now being reported.
EE was installing cable bolts in the belt entry of the 16 WHead section. When EE was removing the 4' drill steels from the bolt hole, the steels became stuck up in the hole. When EE pulled apart the steels, they fell striking his left middle finger. EE continued to work until 12/23/08/ At this time he became a LTA with a broken left hand middle finger.
Felt pain in low back while handling rockdust bags.
As the employee was positioing a motor coupler he felt a ""pop"" in his abdomen. The employee continued to work and was diagnosed with a hernia on 12-8-08 which is the first day that he missed work.
EE was operating a battery mantrip, when mantrip struck a broken rail. EE was thrown against breaker boxes, causing him to break left upper jaw, break upper teeth and bruise left rib cage.
EE was repairing a water leak on #69 shield. He removed a keeper pin on a 13mm. High pressure line. The hose blew and the pressure in the line cut his left ring finger and middle. Ring finger has 10 sutures and middle finger has 3 suture.
Ee fell off 12' ladder while repairing a door, He had severe pain in lower back and continued to work. On 12/27/08 He was cutting mine cars and felt pain in lower back.
Employee was walking when he stepped on uneven pavement and felt pain in his right ankle.
Employee felt pain in low back after falling when the come-along hook-up slipped.
EE was stepping out of landing machine on the 16wt section, EE caught his right foot on the bottom of the canopy, felt a pop in the back of his right knee.
11/2/08 EE slipped in a drainage ditch that was located along the mainline treck. EE was walking on treck and had a rail roller in his hand, when he slipped the bar hit him above the left knee, EE thought he had problem with the knee from a past injury, 11/15/08 MRI showed a broken petella.
EE was hosing up rib protectors on Alpine miner. When power was energized, the EE was stamping between rib protector and machine. The rib protector struck EE causing pain to his lower abdomen.
The employee states that as he was walking, he tripped and twisted his right ankle.
EE was stripping wood forms. He pulled the board and it slipped causing a puncture wound to his left knee from a nail in the board.
While walking an aircourse, the employee tripped and fell injuring his right ankle.
Employee was in process of picking up a timber and turned to place on a stack of timbers and felt pain in lower back. Employee had MRI and was diagnosed with lumbar sprain/strain on 10/21/2008.
The employee stated that while replacing a torque shaft in a miner, he felt pain in his lower back
Employee was pulling on miner cable to get slack to hang to the mine roof and felt a burn in his lower left abdomen.
Victim was roof bolting on the left side of the Apline miner in 38 8 North section. He had just completed drilling a hole and had a stick of glue in his right hand. He was reaching for the bolt when a piece of stone that was approx 1'x1'x3"" fell striking the knuckle of his left middle finger. This resulted in a laceration that required 4 sutures to close.
A metal cover that weighs an estimated 300 lbs was lying on the bottom. The scoop operator and victim were trying to flip the plate up on it's end with the scoop bucket. The plate was stood up on it's end. At that time, the victim tried to push it the rest of the way over when the plate slid (metal on metal) on the scoop bucket falling over on his right foot.
Ee was sitting on a ventilation tube along side the miner and was knocked into the miner when the loading machine arm struck the tubing.
Stepped down from the dozer onto a coal pile and twisted right knee. Reported to company on 9/22/08.
EE was raising up a coal hauler too put the third crib block under the frams of it, when it slipped and fell on the crib, caused it to flip up and strike him in the lower jaw.
The employee stated that he tripped and fell. While falling he struck his left arm and shoulder against the ventilation tubing.
EE was lifting and moving DMU rails and felt pain in lower back.
While removing a bottom belt roller, the employee got his gloved left middle finger caught between the roller and the frame of the roller assembly resulting in a fractured tuff and a laceration requiring 6 sutures.
The employee was in the process of removing a manifold off of a 1-1/2 inch fire valve, which was attached to a 4 inch water line at Break 17 on 4 North Mains. The manifold flew off striking the employee in the face causing lacerations to the face requiring sutures to close and a broken nose.
EE received a left rib fracture when the miner slid backwards while attempting to tram forward on an incline of an overcast, pinning him between the rib protector and the rib.
EE was in process of setting turn timbers in the #13 entry of Mains section for the right barrier lift, when two sections of roof broke loose from the roof, measuring from 0-2"" thick and 45"" X 28"" and 45"" X 27"". The roof fell out from around the roof bolts, striking him on back of the head, middle of back, top left hand and left elbow.
Employee was walking around the tail end of a conveyor belt. When he bent over to get around the ""X"" brace that crosses over the catwalk, he felt a sharp pain in the middle of his lower back. ""X"" brace was about 3 feet above the catwalk.
The employee lost his balance and fell against the mine coal rib while moving mine rails. The resultant injury was a contusion to the right shoulder. The accident became reportable on 8-20-2008 when the employee started missing work.
Avulsion of the left index finger to the first joint by removing a drill steel under lateral pressure.
While replacing a cover on a miner, the employee caught his gloved left middle finger between the cover and the frame resulting in a broken finger tip.
The injured employee and a Motorman put an empty supply car on the Contrary Service Cage in order to send to the surface. When the injured uncoupled the car and flagged the Motorman to pull away, one of the safety chains caught the horn on the motor coupling, which pulled the supply car on the right leg of the EE, resulted in fracture of the right tibia and fibula.
EE was operating left side of the roof bolter on the 12 Right Section in the # 1 entry approx. 50 ft. inby survey station 25484 & as he walked towards the end of the roof bolter a rib measuring about 76"" high x 84"" wide by 26"" thick rolled off behind the line curtain striking the employee against the bolter resulting in fractures to left shouder, ribs, sternum & back.
Felt pain in right shoulder after carrying cans of oil from a flatbed and thru a mandoor. Diagnosd with degenerative acromioclavicular joint change on 7/25/08.
While walking around the 15A feeder, the employee stepped on something and twisted his right knee.
EE was carrying conduit and slipped on loose gravel, causing him to fall and injure his right leg.
Felt pain in low back while shoveling belt.
Injured employee stepped in hole near upsweep of fan.
Employee stated that he was carrying a side cover for the Alpine miner (approx 2'x 2') in the track entry of 3B miner section. He stepped on a piece of coal causing him to turn his left ankle. This caused swelling and the doctor told him not to put on a miner boot.
Lifting belt rollers out of slope car, felt pain in rt. shoulder.
While pulling miner cable/waterline with shuttle car employee's left foot was caught between miner cable and water line, resulting in a fractured left tibula.
Employee's left hand thumb was pinched between a trailer and the latch on a pintle hitch causing a minor fracture and laceration.
Employee was walking on 10 Rt. section at #16 crosscut between #4 & #3 entry, slipped and fell. He tried to catch himself from the fall and injured his right knee. A MRI was performed and it was determined that a tendon was torn from the right knee cap. Surgery was performed to repair the tendon. Employee started missing work on 6/26/08.
A coworker picked up a piece of floor decking; a piece of 1"" X 3' to 4' rod fell from the decking to the next floor level striking the empoyee on his left hand.
Tin sheeting fell and struck employee on the hard hat.
Employee was going over dirt slope during in lighting storm when he slipped and fell and sprained left ankle.
Pulling rockdust hose and felt pain in groin area was diagnosed with a hernia and his first day of lost work is 5/29/08.
Employee was n the process of extending line curtain when a section of roof fell measuring 7.5'L x 6'W x 0-14"" thick from the unbolted face and extending approximately 16"" past last row of bolts to where he was standing under a permanently supported area striking him across both legs resulting in fracture to left big toe and tailbone.
EE felt pain in his lower back while loading roof nolts onto a bolt machine.
While walking from the miner to the 15A dinner hole, the employee states that he stepped on a rock, twisted his right ankle and fell.
Employee was pulling rail when it caught on the track and a car on the track. Tension was still on the wire rope that was pulling the rail. The employee used a pry bar to free the rail. When he did, the rail moved a few inches and forced the pry bar toward the car with the employee's right hand between the pry bar and the shelter that was on the car. EE worked until 5/12/08.
Employee stepped out of manbus. He stepped on a piece of coal and twisted his left knee, hyper extending it. He had pain and swelling in left knee.
EE was unloading posts and mashed the tip of his left index finger between two posts.
Employee was in process of removing broken pieces of a cast volute from a fine heavy media pump and as he tried with a bar using right hand, the broken discharge section of pump came loose and caught his left ring and middle finger between the broken discharge section and an assembly bolt resulting in lacerations and fractures to the tips of both fingers.
A piece of coal fell from roof while ee was sitting in a motor waiting to leave bottom area. The coal struck ee on left wrist and left leg causing a contusion on the leg.
EE stated that he was helping rib bolt with a twin boom bolter at #2 empty belt rehab project. He & another EE were going to rib bolt a corner but there was an oxy & acet. cart there. EE attempted to move cart & feet slipped causing him to fall to knees while cart moved forward. This caused his head to whip down forcing his nose into the cart resulting in nose fracture & cut.
Employee was struck by a section tailpiece on 3/24/08 which caused a contusion on the right knee. The employee started missing work due to the injury on 07/01/08.
The EE bent half-way over with the intention of picking up a crib and felt pain in his low back.
The employee was loading sections of 4"" waterline when the pipe suddenly turned. When the employee attempted to stop it, he felt pain in his low back. The incident became reportable on April 9, 2008 when medical evidence revealed that the employee would be missing work.
Set jack down on foot while unbooming pole.
Injured was responding to an emergency job when the mine had a tear in the overland conveyor. A steel cord conveyor had a 14"" tear for 7000'. Injured was assisting in removing the torn segment when a piece of the conveyor that was already removed and piled up fell against him and knocked him down causing him to strike his left arm against a piece of angle iron, fracturing arm.
EE was hosing in the slope head house when the hose got caught on a chain, He jerked the hose to free it up and felt pain in his lower back and right hip.
EE was moving bolt cable using his right foot, when the cable tightened and knocked him off of his feet, causing him to break his ankle.
Miner cable struck EE's head, shoulder and thumb while lowering it from a hanger. Evidence obtained from a compensation review on 6/27/08 dictated recording the incident.
He was shoveling snow out of truck bed. Fell down in the truck bed.
Rock fell through a wire mesh, hitting EE on the head/shoulder. Few minutes later he felt faint, tingling in right arm. Cervical Spine Strain. *Post treatment infection resulted in hospitalization and lost time.
Checking anchor bolts and slipped/fell on snow covered slope.
Employee stepped on an angle iron that was rusted. The angle iron collapsed causing employee to twist ankle.
The snatch block on the winch line slid down the winch line and struck the injured employee's left thumb and index finger, resulting in a contusion.
Walking around derrick truck, ankle twisted on rocks at V39 station.
EE was helping a core breaker on the alpine miner in 2 south east section when the fellow ee was hitting the core breaker with a 3 lbs hand sledge. He bent over to see if the core breaker was aligned properly and when doing so, but his left hand on the core breaker which was struck by the hammer. This result in a laceration to
Employee was throwing crib block thru mandoor and caught right index finger between door and crib block.
Employee was getting out of manbus. He got caught up as he stepped out of manbus and twisted his right ankle. Ground was level no tripping hazard.
Loading 4"" x 20' metal water line onto rail car, another piece of pipe already on the car rolled catching the middle finger on his right hand.
EE loading rails using hoist winch cable. EE pulled on winch rope so rail would clear trucks as the rail was being raised and the cable tightened up. EE pinched right hand and little finger between cable and rail (4 sutures).
Ee was assisting a mechanic in loading a hill block housing for a belt roller onto a vehicle and caught right hand, middle finger between mobile and roller housing.
The employee states that while cutting out date boards to use for fire boss use, the razor knife he was using slipped and he cut his right lower thigh just above the knee. The resultant injury required 2 sutures.
While center bolting, the employee reached up to straighten a bent roof, the bolt unexpectantly twisted around when the employee pushed against it. A sharp place on the bolt plate struck against the employee right wrist resulting in a laceration. The resultant injury required 7 sutures.
Employee was hosing spillage along belt. Three hoses were being used for cleaning spillage. When two hoses were shut off it increased water pressure in her line and tossed her right pinky finger against the metal belt structure.
The employee was struck by a rib roll. As he fell, he struck the rib with his right arm resulting in a laceration requiring 9 sutures.
A piece of wire mesh was sticking out the back of the scoop and when the EE walked by, it struck him on his right eye brow causing a laceration that required stitches.
Employee, was removing a piece of pipe - the pipe became wedged in the U-Bolt holding it. He advised a coworker to lift on the pipe which allowed the pipe to slide freely striking his left ""pinky' finger between 2 pipes. He was wearing metacarpal gloves - however the cut still required stitches.
Employee states that he was dragging a wash down hose, when it suddenly got stuck on a bench bottom, causing the hose to slip through his left hand. A repaired place in the hose had a exposed hose clamp which caused a laceration to the left middle and ring fingers. The resultant injury required 4 sutures to each finger.
Employee was operating Joy shuttle car and pulled behind the Joy Miner when the boom contacted the mine roof and shot a piece of coal into the shuttle car cab striking the employee in the head and neck causing a laceration on his right cheek that required 5 sutures.
EE was using a knife to cut a slot in the plastic 5 gal bucket. The knife cut thru the bucket and into his off-gloved left hand, resulting in 5 sutures between the forefinger and thumb.
EE stated that he was working in the crusher building and cleaning off a 24"" beam so that he could repair a hole in a chute. As he was doing this, a piece of stone fell, striking him on the left ear lobe.
EE was picking up a roof bolt by hand to place the roof bolt tightener in the chuck and cut his forearm with the bolt plate.
EE was hammering a pin back in on a mucker cat chain and a piece of metal chipped off and punctured the EE on the chin. Doctor surgically removed the metal on July 15, 2008 resulting in a recordable incident.
EE was in the process of recovering track rails on 10 right section and apparently lost his balance while positioning the rail with a slate bar. The slate bar struck the employee on his right temple area causing a laceration which required 2 stutures to close.
The injured EE was positioned in the jackline of O panel longwall near jacks 4-5 oberserving the shear cutting out at the head,when hydraulic hose fitting apparently broke,the hydraulic hose struck the employee on the left leg resulting in a fracture of the fibula.Indcident occured on May 11,08 and reported to mine mgmnt on May 15,08
While dressing a motor lead with a hand held grinder on a motor he was repairing, the employee struck himself with the grinder in the web of his left hand between the thumb and index finger. The resultant injury was a laceration requiring 5 sutures.
EE was hanging pipe in the shaft and when he went to cut the plastic cap off of the pipe his knife slipped and cut his left thumb requiring four stitches.
Employee stated that he was rib pinning from the miner. When drilling into the rib, the ee was struck with a drill steel extension that came loose during rotation. He was struck in the face causing a laceration to the mouth area. The accident is still under investigation.
While loading a belt drive motor the employee was struck by an unknown flying projectile in the right bicep causing a contusion and a puncture wound. The physician had to open up the wound to determine the extent of the damage which required staples to close.
At 9:50 AM EE was supervising another emp. as they were changing out a jig intake air valve. The unit was placed on the jig air chest that is approx. 24"" high. As emp. was cleaning the pocket, EE turned to get a wrench & as he did, he contacted the valve which caused it to fall hitting his lt. metatarsal.
EE was in dinner hole when a piece of rock fell from top knocking off his hat, more rock followed striking EE on bare head, resulting in laceration, which required staples.
The employee caught his gloved right ring finger between the top bearing surface of a ratchet jack and the jack body as he was moving the jack. The resultant laceration required 4 sutures.
On 3/21/2009, the mine received a notification of carpal tunnel syndrome claim diagnosis for the employees left wrist.
Employee was diagnosed with degenerated disk disease on 4/27/08.
Employee was granted an award for occupational pneumoconiosis.
Employee was seen by a physician and Dx with carpal tunnel in both wrists. A date of 3-31-2008 was determined by the doctor. However, the employee did not go off on surgery until 5-19-2008.
Medical evidence received that the employee was granted an award for noise-induced hearing loss.
EE had a heart attack while working. A clot was removed and a stent was put in. Not work related.
EE was struck by a small rock, which fell while he was installing a split set bolt, causing a laceration to his forehead which required two stitches.
